    Abstract Background: The need for home-based palliative care is accelerating internationally. At the same time, health systems face increased complexity, funding constraints and global shortages in the healthcare workforce. As such, ambulance services are increasingly tasked with providing palliative care. Where paramedics with additional training in palliative care have been integrated into models of care, evaluations have been largely positive. Studies of patient and family carer experiences of paramedic involvement, however, are limited.
    Aim: To explore patient and family caregiver experiences of paramedics' contribution to palliative care at home.
    Design: Qualitative interview study. We analysed data within a social constructionist epistemology using reflexive thematic analysis.
    Setting/participants: Participants receiving specialist palliative care in the community of a metropolitan city of Australia who requested an ambulance between January and August 2018, inclusive.
    Results: Participants considered paramedics with expertise and experience in palliative care as an extension of the specialist community palliative care team and held them in high regard. Participants highlighted the importance of: critical palliative care at home and a timely, responsive approach; person-centred paramedics; as well as safety and security.
    Conclusion: Patients and carers feel safe and secure when they know that highly responsive skilled professional support is available when an unexpected problem or sudden change arises, especially out-of-hours, and that support is delivered in an empathic and person-centred manner.

    Abstract Who feeds the world? Two billion small-scale farmers who in addition to feeding themselves also produce surpluses for local markets — these are the food producers for a global population. Domestic markets along with the food consumed by the producers’ families constitute more than 70% of the world’s food consumption and are often overlooked in the food security debate. The importance of these producers to overall global food security is not in dispute, but can these farming systems continue to perform at current or improved levels, considering the influence of human-induced land and water degradation and associated effects on ecosystem services? Soil erosion, depletion of nutrients and soil organic matter, salinisation and surface and groundwater pollution are challenges that have confronted agricultural and urban communities for decades, and still do. Land degradation associated with inappropriate and unsustainable land use practices is estimated to affect 5–10 Mha annually; 34 Mha of global irrigated areas is affected by salinisation; it is estimated that 25% of global freshwater storage capacity will be lost in the next 25–50 years unless measures are taken to control sedimentation in reservoirs; approximately 2 Mt of waste is dumped into rivers, lakes and wetlands each day; and it is estimated that there are now 12,000 km3 of polluted water on the planet, a volume greater than the contents of the world’s ten biggest river basins. This litany of land and water degradation issues represents a diminished ability of ecosystems or landscapes to support functions and services required to sustain livelihoods. Small-scale farmers, the engine of global food supply, are the mainstay of most developing country rural economies and often occupy marginal and vulnerable lands. It makes sound economic sense to address this ‘slumbering giant’ of degradation through increased conservation investments in land and water resources within this sector. Whilst technologies, technology packages and management practices have been developed that demonstrate the practicalities of addressing these resource degradation issues, adoption at scale has been disappointing. Government institutions and development and research organisations are tasked with sustainably securing future food supplies. Their central challenge is to develop greater insights into constraints inhibiting adoption of productivity-enhancing and conserving interventions, and to identify the driving factors and relevant levers to address these constraints. Time may not be on our side in addressing land and water degradation, central to one of the nine thresholds that define ‘a safe operating space for humanity’.

    Abstract The value of ponds in urban areas historically has been overlooked. While some recent studies have described considerable biodiversity in urban areas, it is unclear as to how far this extends to different urban habitats. The aims of this study were to determine the condition of 21 urban ponds in Bradford (northern England) and to quantify the connectivity of wetlands in the district. The study showed that macroinvertebrate and plant biodiversity was substantially lower than would be expected based on pristine reference sites. Of the 21 ponds surveyed, 15 were found to be classified as having very poor ecological quality, with 5 being classed as poor and just 1 was classed as moderate. The number of aquatic plant species found in the ponds ranged from 0 to 6 and the number of macroinvertebrate families found ranged from 4 to 13. It was suspected that the aquatic plant diversity was low due to management techniques such as the removal of emergent vegetation. The average distance to a wetland was found to be higher in urban areas (533 m) compared to rural areas (448 m) although this difference was small, which indicates that the low diversity found in urban ponds is likely due to habitat variables.

    Abstract We demonstrate how niche theory and Hubbell’s original formulation of neutral theory can be blended together into a general framework modeling the combined effects of selection, drift, speciation, and dispersal on community dynamics. This framework connects many seemingly unrelated ecological population models and allows for quantitative predictions to be made about the impact of niche stabilizing and destabilizing forces on population extinction times and abundance distributions. In particular, the existence of niche stabilizing forces in our blended framework can simultaneously resolve two major problems with the dynamics of neutral theory, namely predictions of species lifetimes that are too short and species ages that are too long.
