    Abstract Trigger and functional substrate are related to the tone of autonomic nervous system, and the role of the autonomic nerve is more significant in paroxysmal atrial fibrillation (AF) compared to non-paroxysmal AF. We have several options for neuromodulation to help to manage patients with AF. Neuromodulation targets can be divided into efferent and afferent pathways. On the efferent side, block would be an intuitive approach. However, permanent block is hard to achieve due to completeness of the procedure and reinnervation issues. Temporary block such as botulinum toxin injection into ganglionated plexi would be a possible option for post-cardiac surgery AF. Low-level subthreshold stimulation could also prevent AF, but the invasiveness of the procedure is the barrier for the general use. On the afferent side, block is also an option. Various renal denervation approaches are currently under investigation. Auditory vagus nerve stimulation is one of the representative low-level afferent stimulation methods. This technique is noninvasive and easy to apply, so it has the potential to be widely utilized if its efficacy is confirmed.

    Abstract Background: Despite their growing popularity, the extent and nature of workplace alcohol and other drug (AOD) policy and support services in the United States remain unclear. The present study examined the availability of AOD policies and support services in workplaces. Also, the associations between a combination of these workplace measures and the current AOD consumption (focusing on binge drinking, use of marijuana and other illicit drugs, and nonmedical use of prescription drugs) were assessed.
    Methods: Pooled Data from the 2015-2019 National Survey on Drug Use and Health (NSDUH) were utilized to examine a nationally representative sample of 131,751 employed adults. First, the rates of those who reported having a written AOD policy at their workplaces among the total sample and subpopulations across socioeconomic status, and those who reported an employee assistance program or other counseling services for substance use problems were estimated. Then the associations between a combination of the two workplace measures and individuals' current AOD consumption were tested using multinomial logistic regression analyses.
    Results: Although 77.2% of employed individuals reported having a written AOD policy at their workplaces, only 50% of the sample had access to support services for substance use problems. The limited access to support services was most notable among traditionally disadvantaged groups (e.g., females, Hispanics, and individuals with limited English proficiency). The models suggest that having both a workplace AOD policy and support services was associated with significantly lower rates of marijuana and other illicit drug use compared to having neither of the two measures or only support services.
    Conclusion: Regardless of workplace AOD policies, the limited availability of support services, especially for disadvantaged workers, raises concerns. While further investigation is needed to understand the effectiveness of the comprehensive prevention approaches at workplaces, it is also important to advocate for the potential benefits of having employee assistance or other services available along with AOD policies.

    Abstract Trigger wrist, characterized by a clicking or snapping sensation around the wrist joint during finger or wrist motion, and bifid or trifid median nerve, which occurs in carpal tunnel syndrome along with anatomical variation of median nerve, are rare conditions. We report the case of a patient with a thickened tendon caused by severe tenosynovitis and flexor tendon subluxation to the hamate hook due to bowing of the flexor retinaculum, thereby resulting in trigger wrist as well as an anatomical median nerve variation (bifid median nerve in the right wrist and trifid median nerve in the left wrist). A 59-year-old housewife visited our hospital with bilateral fingertip numbness, tingling sensation, and aggravated severe night cramping that began 2 months ago. She also complained about trigger wrist during small finger flexion. Based on magnetic resonance imaging, ultrasonography, and nerve conduction study, trifid median nerve and bilateral severe median nerve neuropathy of the wrist were diagnosed; therefore, transverse carpal tunnel release and exploration under wide-awake anesthesia were planned. Intraoperative findings showed trifid and bifid median nerves in left and right wrists, respectively. Additionally, bowing of flexor retinaculum and severe flexor tendon tenosynovitis were observed. Tenosynovitis with thickened flexor sheath resulted in subluxation of the small finger flexor tendon above the hamate hook. After transverse carpal ligament release with antebrachial fascia release and tenosynovectomy, subluxation of the flexor tendon was resolved. At 6 months postoperatively, the tingling and dullness in fingertips also resolved, and no trigger wrist or any other complications were noted.

    Abstract The present study examines the phonetic and phonological status of vowel reduction in Brazilian Portuguese. In order to compare the effects of duration and metrical structure, we tested the influence of duration on the realization of /a/ in five prosodic positions: word-initial pretonic, word-medial pretonic, tonic, word-medial posttonic, and word-final posttonic. The results revealed that, while both phonetic duration and prosodic position had effects on F1 values for /a/, the categorical effect of prosodic position was much stronger and more reliable. In particular, F1 values for /a/ were best predicted by a two-way distinction between posttonic and non-posttonic syllable positions. Correlations between a vowel's duration and its F1 frequency were statistically significant but generally weak in all positions. We argue that these findings suggest that vowel reduction in Brazilian Portuguese primarily reflects phonological patterning rather than phonetic undershoot, although there was also evidence for some amount of undershoot. Brazilian Portuguese can therefore be said to have a mixed system of phonological and phonetic reduction. The present study discusses the results in the context of Brazilian Portuguese metrical organization, sound change, and the relation between phonetics and phonology.

    Abstract This review essay examines the ambitious thirty-volume series Science and Civilization in Korea (SCK), published between 2010 and 2022. Input from over sixty Korean scholars traces the evolution of Korean science and technology, from elementary tools to advanced semiconductor technology. Inspired by Joseph Needham's series Science and Civilization in China, SCK seeks to reveal the "universal value" embedded in Korean civilization, extending to the tumultuous eras of the nineteenth and early twentieth centuries. This review essay probes SCK's implications and boundaries, elucidating the influences molding its narrative and identifying omissions. It also considers alternative narratives. Albeit rooted in use-centered historiography, such narratives would not be restricted to the local but underscore an array of practices striving for compatibility with global resources. Moreover, they could bridge the "imagined discontinuity"-the notion of rupture around 1900-between "tradition" and "modern" and thus cultivate a more seamless chronicle of Korea's history of technology.

    Methods: A total of 44 sites (15 shallow, depth<3.5 mm; 29 deep, depth>5.5 mm) were analyzed in 3 canine left ventricles under general anesthesia. A custom-made bipolar needle electrode was used to simulate a focal activation source. A mapping catheter with an electrode tip size of 2 mm, band electrode size of 1 mm, and inter-electrode spacing of 2-10-2 mm was placed at the mapping area.
