    Abstract Introduction: The aim of this report was to assess the skeletal, dental and soft tissue changes in a juvenile patient with a severe class III malocclusion. This case report describes a novel method of class III treatment using skeletal anchorage for maxillary protraction and Alt-RAMEC protocol.
    Patient concerns: The patient did not have any subjective complaints before treatment and there was no family history of class III malocclusion.
    Clinical findings and primary diagnoses: Extra-orally, the patient had a concave profile with a retrusive mid-face and prominent lower lip. The intra-oral examination revealed angle class III malocclusion with a -3-mm overjet. There was no anterior displacement on closure when the patient was assessed clinically. According to the cephalometric analysis, the sagittal jaw relation and Wits appraisal were reduced due to a retrognathic maxilla and prognathic mandible.
    Interventions: The treatment plan involved maxillary protraction, Alt-RAMEC protocol for 10 weeks and upper molar distalisation with a hybrid hyrax distalizer in combination with a mentoplate. The active treatment time was estimated to 18 months followed by 6 months retention with the appliance.
    Outcomes: The sagittal jaw relationship was increased by approximately 9°, mainly due to maxillary advancement of 8 mm and a positional change of the mandible anteroposteriorly. Natural decompensation of the lower incisors was also observed. In addition, both the facial profile and the smile became more harmonious after treatment. The treatment analysis revealed that the changes achieved were mainly skeletal and it was possible to avoid adverse effects on the dentition.
    Conclusion: In conclusion, treatment with a hybrid hyrax distalizer combined with mentoplate using the Alt-RAMEC protocol is effective in correcting the anteroposterior discrepancy in a juvenile class III patient and it is possible to achieve maxillary advancement of 8 mm.

    Abstract Transient Receptor Potential (TRP) channels are multifunctional sensory molecules that are abundant in the skin and are involved in the sensory pathways of itch, pain, and inflammation. In this review article, we explore the complex physiology of different TRP channels, their role in modulating itch sensation, and their contributions to the pathophysiology of acute and chronic itch conditions. We also cover small molecule and topical TRP channel agents that are emerging as potential anti-pruritic treatments; some of which have shown great promise, with a few treatments advancing into clinical trials—namely, TRPV1, TRPV3, TRPA1, and TRPM8 targets. Lastly, we touch on possible ethnic differences in TRP channel genetic polymorphisms and how this may affect treatment response to TRP channel targets. Further controlled studies on the safety and efficacy of these emerging treatments is needed before clinical use.

    Abstract Background: Parathyroid carcinoma is exceedingly rare. This cancer does not metastasize in a predictable manner and has in fact been noted to spread both hematogenously and lymphatically. There are currently no cases in the literature that report metastasis to a contralateral level II lymph node.
    Case description: We report a case of recurrent parathyroid carcinoma in which the patient underwent several surgeries for right-sided parathyroid carcinoma and hyperparathyroidism, including 2 redo parathyroidectomies, 4 neck dissections, and auto transplantation and re-excision of the autotransplanted tissue. Almost a decade after initial surgery, parathyroid hormone (PTH) levels rose significantly, but nuclear medicine (NM) sestamibi scan, positron emission tomography-computed tomography (PET-CT), and ultrasound (US) could not identify the source of recurrent disease. Finally, neck US revealed a suspicious contralateral level II lymph node; fine needle aspiration (FNA) of the lymph node revealed parathyroid tissue, and PTH aspirate noted an elevated PTH washout. After left lateral neck dissection, pathology confirmed recurrent metastatic parathyroid carcinoma, and serum calcium and PTH levels improved. This case was further complicated by chronic kidney disease and pT1N0M0 colon cancer, all before the age of 45.
    Conclusions: In cases where a sestamibi scan is not able to localize a metastatic focus in a patient with known parathyroid carcinoma, FNA and PTH washout are important secondary studies.

    Abstract Background: Primary hyperparathyroidism (PHPT) is classically treated by conventional parathyroidectomy, an open neck surgery. Radiofrequency ablation (RFA) has been shown as a safe minimally-invasive alternative to parathyroidectomy for the management of PHPT and has been shown to be effective in 60-90% of cases. Here, we present a patient successfully treated for persistent PHPT by RFA with simultaneous intraoperative parathyroid hormone (IOPTH) monitoring.
    Case description: A 51-year-old female with a past medical history of resistant hypertension, hyperlipidemia, and vitamin D deficiency presented to our endocrine surgery clinic with PHPT. Neck ultrasound (US) localized a 0.79 cm lesion suggestive of a parathyroid adenoma. Parathyroid exploration resulted in the excision of two masses. IOPTH levels dropped from 259.9 to 204.7 pg/mL. No ectopic parathyroid tissue was found. Three-month follow-up demonstrated elevated calcium levels, suggesting persistent disease. A repeat neck US one-year post operation localized a suspicious hypoechoic sub-centimeter thyroid nodule, which was subsequently determined to be an intrathyroidal parathyroid adenoma. The patient elected to proceed with RFA with IOPTH monitoring, citing concern for increased risk of redo open neck surgery. Operation pursued without complication and IOPTH levels dropped from 270 to 39.1 pg/mL. The patient's only three-day post-operative complaints, occasional numbness and tingling, were completely resolved by her three-month follow up. The patient had normal PTH and calcium levels at seven months post-operation visit and was without complaint.
    Conclusions: To our best knowledge, this is the first reported case of RFA with IOPTH monitoring used to manage a parathyroid adenoma. Our work adds to the growing literature suggesting minimally-invasive techniques, such as RFA with IOPTH, as a potential management option for treating parathyroid adenomas.
