    Abstract Resumen: Introducción: el objetivo del estudio es analizar la aceptabilidad de un modelo de formulación diagnóstica integrativa y multidimensional, implementado como estrategia educativa en el primer año de una residencia de psiquiatría de niños y adolescentes, por parte de un supervisor clínico dentro de una sala con espejo unidireccional. Material y métodos: se utiliza como metodología el análisis de contenido cualitativo de entrevistas realizadas a residentes, quienes participaron consecutivamente de la supervisión clínica entre 2018 y 2021. Para el análisis, se emplean 5 constructos teóricos derivados de la Teoría del Comportamiento Planeado y del modelo MODE de Fazio: actitudes, norma subjetiva, control percibido, motivación y oportunidad. Resultados: veintiséis códigos temáticos emergidos a partir del análisis influyen sobre la aceptabilidad: 8 códigos actitudinales (por ejemplo, aceptabilidad favorecida por la utilidad del modelo para analizar teóricamente los casos, pero con un alto tiempo invertido); 4 códigos normativos (por ejemplo, aceptación o desaprobación al modelo por personas ajenas a la supervisión); 7 códigos de control percibido (por ejemplo, la autoeficacia percibida aumenta con el tiempo); 4 códigos motivacionales (por ejemplo, deseo de aprender) y 3 códigos de oportunidad (por ejemplo, deliberación conjunta con pares). Conclusión: los resultados muestran información valiosa para el desarrollo de las estrategias educativas en el programa de especialidad, al aproximarse a factores cognitivos y afectivos significativos en el aprendizaje durante el período de formación. Se identifican aspectos de la estrategia que resaltan como favorecedores del proceso educativo, junto con otros aspectos que pueden perfeccionarse para una mayor aceptabilidad de la herramienta docente.     Abstract Introduction: Autism Spectrum Disorder (ASD) is a neurodevelopmental condition with alterations in social inte raction and communication, and restricted and repetitive patterns of behavior and interests. Gender Dysphoria (GD) refers to the incongruence between the gender identity and the sex assigned at birth, together with a strong and persistent desire to be of the opposite sex. During global personal identity construction, the development of sexual identity is essential.
    Objective: To analyze an adolescent clinical case of co-occurrence of ASD and GD from a psychodynamic understanding of his identity.
    Clinical case: A 15-year-old male diagnosed with ASD and borderline intellectual capacity. He mani fested feminine behaviors and interests in girlish games since preschool age, imitating female figures, and verbalizing the desire to be of the opposite sex. A multidisciplinary evaluation of the patient concluded that he presented a diffuse general identity with ambivalent gender identity. Therefore, he did not fulfill the requirements for hormone therapy. Psychological therapy was suggested to favor grea ter exploration and cognitive flexibility to develop a better-defined gender identity.
    Conclusions: In cases as ASD, where identity development is compromised, the affected subject often delays or does not consolidate sexual identity, leading to a diffusion of identity and gender, as in the reported case. The co-occurrence of ASD and GD requires a multidisciplinary evaluation that includes a complete assessment of the patient's global identity before considering any definitive therapeutic orientation regarding gender.

    Abstract Introduction: atlas assimilation can occur alone or in association with other craniocervical junction malformations and deformities. The etiological and pathological mechanisms are not clear.
    Case presentation: patient in her sixth decade of life, who was treated at the "Eugenio Espejo" Hospital, with asymmetric pyramidal symptoms after a low-energy kinetic fall. The condition completely reverted with conservative management. The patient is currently under periodic observation.
    Conclusion: the atlas assimilation, a congenital malformation, often asymptomatic; it is part of an embryological compensation when it is associated with other malformations. The appearance of deformity is necessary to reach atlantoaxial instability that it is not early. The knowledge of the concepts and distinction of the craniocervical junction abnormalities allows to indicate the best treatment in order to obtain the most suitable results with the individualization of each case.
