    Abstract Background: Ureteropelvic junction obstruction (UPJO) and renal calculi are associated in 20 to 30% of cases and treatment is mandatory. The simultaneous surgical management is a therapeutic challenge that is still a source of controversy. We describe our technique combining robot-assisted laparoscopic pyeloplasty and transcutaneous retrograde flexible ureteroscopy (fURS), assessing the feasibility of simultaneous treatment through an original technique.
    Methods: This single centre series reports our initial experience with 12 patients. From January 2014 to September 2018, 12 patients underwent robot-assisted laparoscopic pyeloplasty with simultaneous fURS for UPJO and renal calculi. Mean age was 46 years (24-68). 92% had multiple renal stones and the mean cumulative stone diameter was 31,3mm. Robot-assisted pyeloplasty was performed with peroperative transcutaneous retrograde fURS through a ureteral access sheath introduced in an incision on the bassinet through a subcostal trocar. Stone extraction was performed using a basket.
    Results: All patients underwent surgery successfully, achieving UPJ repair and complete stone extraction. Mean operating time was 92,5min (85-110). All reported Clavien-Dindo complications were grade 1. Non-contrast enhanced abdominal CT performed 1 month after surgery confirmed the absence of residual stones in all patients. Mean follow-up time was 10 months with no recurrence of UPJO.
    Conclusion: This small series confirms the feasibility with good surgical results of concomitant robot-assisted laparoscopic pyeloplasty and transcutaneous retrograde fURS stone extraction. No major complications were observed. This technique is easily reproducible but requires 2 experienced urologists to be achieved in a contained operative time.

    Abstract The idea that earthquake processes occur on a continuum from aseismic (stable sliding) to seismic slip (event nucleation) is not new, however the mechanisms causing any evolution between the two end-members is not fully understood. Possible mechanisms may relate to dynamic fault weakening, static stress transfer, the concentration of shear stress at the boundary of an aseismic region, some combination of these, or a mechanism we have yet to detect. Moreover, the detection of aseismic processes is inherently difficult, adding to the challenges in detecting and understanding this evolution. Here, we investigate evidence for a critical state transition within the subsurface using data from several recent seismic sequences in western Canada. The first case investigates a potential change in seismic event rate following prolonged fluid injection at a water disposal well, suggesting that initial deformation in the subsurface related to the injection was aseismic. Twelve months into the injection program, a large change in the seismic event rate was observed, along with a change in the source characteristics of the events, suggesting a transition from dominantly aseismic to seismic slip. Another case investigates seismicity in a historically quiescent area where there is ongoing fluid injection. A M5.3 mainshock was followed by an unusual aftershock sequence, which showed limited evidence of Omori-like decay, and instead, remained persistent. We investigate these sequences using an array of techniques including ambient seismic noise, repeating seismicity and the identification of tremor-like signals to determine whether there is evidence of a critical state transition at depth.

    Abstract Background: Cigarette smoking rates remain disproportionately high among low income populations with unmet social and behavioral health needs. To address this problem, we sought to develop and evaluate the feasibility, acceptability, and preliminary effectiveness of a novel smoking cessation program for community health centers that serve these populations.
    Methods: We implemented a randomized pilot trial of two smoking cessation programs in three county operated community health center (CHC) sites: (1) a systematic assessment of smoking habits and standard tools to assist with smoking cessation counseling ("Enhanced Standard Program" or ESP), and (2) another that added a structured assessment of social and behavioral barriers to smoking cessation, ("Connection to Health for Smokers" or CTHS). Clinical outcomes were evaluated between 10 to 16 weeks, supplemented with interviews of patient participants and health care team members.
    Results: 141 adults were randomized and 123 completed the intervention (61 in ESP, 62 in CTHS). At follow-up, over half of participants reported ≥1 quit attempts (59.7% ESP and 56.5% CTHS; adjusted
    Conclusion: Lessons learned from this pilot study may inform the development of effective smoking cessation programs for CHCs that combine elements of both interventions.

The urban areas of this nation present high levels of residential segregation, represented in its main city, Santiago. This article presents results of an exploratory analysis of the relationship between residential segregation, immigration rate by educational institutions and changes in school performance.     Abstract Background: Gender differences in psychosis are a topic that has been studied considering different aspects. Although some available evidence would point to a possible better prognosis in women, this claim is far from conclusively established.
    Methods: We propose an analysis of gender differences in the risk of readmission to an acute hospitalization unit, an indicator related to prognosis.
    Results and conclusions: We found that although the risk of readmission at 1 year is lower in women, this seems to be explained by other confounding factors.
