    Abstract Over the past few years, social media has become integral to surgical communication, research, and patient education. Furthermore, they have revolutionized how academic congresses have been held over the past 10 years. There are several benefits resulting from social media integration in scientific meetings, such as disseminating content, increasing engagement, highlighting important messages, and educating patients and physicians. Furthermore, social media platforms provide free and equal engagement of all users; no matter the experience, the level of training, or the socioeconomic background, all participants are free to express their opinion and discuss with world-renowned experts-an opportunity that was available only for the few and privileged ones in the past. Nevertheless, when talking about social media, one must bear in mind that there is a huge variety of platforms that are not all equally used or universally accessed. Besides the benefits highlighted above, this new social media-enhanced reality has some disadvantages as well: lack of any control of content along with anonymity can sometimes lead to dangerous and hostile behaviors as well as set the playground for personal feuds, ulterior financial motives, and bullying. Furthermore, the accuracy of content could be somewhat controversial, especially when pieces of presentations are taken out of context. Violations of intellectual property and copyrights are also to be considered. Social media platforms are a great tool for enhancing academic congresses when appropriately used. It is undebatable that social media has changed how information, content, and material are shared and promoted. Nevertheless, as with everything in life, their use needs to be in "good measure"-appropriate, mindful, and respectful-so that social media can be a powerful tool instead of a dangerous weapon in inappropriate hands.

    Abstract There is a bidirectional relationship between climate change and health care. Climate change threatens public health, and health care contributes to climate change. For example, surgery is the most energy-intensive practice in the health-care sector, and gastrointestinal conditions are responsible for a substantial environmental burden. However, environmental costs associated with health care are often overlooked. This issue has been examined more closely in current times. Emerging data are mainly focused on surgery, as the most resource-intensive practice. However, there is still a lack of global awareness and guidance on sustainable surgical practices. This Perspective aims to reassess the evidence on health care and surgery carbon footprints, focusing on gastrointestinal conditions, identify issues that need to be addressed to achieve a more sustainable practice and develop perspectives for future surgical procedures. The proposed framework to mitigate the environmental effects of surgery could be translated to other health-care sectors.

    Abstract Aim: We describe two options for colorectal anastomosis suitable in cases when the colon would reach the pelvis under tension.
    Method: Deloyers procedure and the retro-ileal colorectal anastomosis are presented, focusing on practical tips and tricks to perform them. Insights on patients who underwent the procedures are provided to demonstrate the advantages and feasibility of the techniques.
    Results: Each step of both techniques is detailed. Ten patients underwent Deloyers procedure and nine underwent retro-ileal anastomosis at our unit. A minimally invasive approach was attempted in 13 patients, of whom five required conversion to open surgery due to the technical complexity of the abdominal procedure. Colorectal anastomosis was successfully performed in all patients. There were no major intra-operative complications, although five patients had postoperative complications requiring further treatment.
    Conclusions: Both techniques are effective in patients at risk of receiving a colorectal anastomosis under tension, and a minimally invasive approach can be used. However, owing to the complexity of surgery in this group of patients, the perioperative morbidity is not negligible. Careful postoperative management is advisable, and patients should be informed of the risks. In expert hands, the outcomes are acceptable, avoiding an ileorectal anastomosis and its constraints.
