    Abstract Monogenic diseases, although rare, should be always considered in the diagnostic work up of vascular dementia (VaD), particularly in patients with early onset and a familial history of dementia or cerebrovascular disease. They include, other than CADASIL, Fabry disease, Col4A1-A2 related disorders, which are well recognized causes of VaD, other heritable diseases such as mitochondrial encephalopathy, lactic acidosis, and stroke-like episodes (MELAS) and cathepsin-A related arteriopathy strokes and leukoencephalopathy (CARASAL). MELAS, caused by mtDNA (80% of adult cases m.3243A>G mutations) and more rarely POLG1 mutations, has minimum prevalence of 3.5/100,000. CARASAL, which is caused by mutations in the CTSA gene, has been described in about 19 patients so far. In both these two disorders cognitive features have not been fully explored and are described only in case series or families. This review paper is aimed at providing an update on the clinical manifestations, with particular focus on cognitive aspects, but also neuroradiological and genetic features of these less frequent monogenic diseases associated with VaD.

    Abstract Cancer cachexia is a complex multifactorial syndrome whose hallmarks are weight loss due to the wasting of muscle tissue with or without the loss of adipose tissue, anorexia, systemic inflammation, and multi-organ metabolic alterations, which negatively impact patients' response to anticancer treatments, quality of life, and overall survival. Despite its clinical relevance, cancer cachexia often remains an underestimated complication due to the lack of rigorous diagnostic and therapeutic pathways. A number of studies have shown alterations in gut microbiota diversity and composition in association with cancer cachexia markers and symptoms, thus supporting a central role for dysbiosis in the pathogenesis of this syndrome. Different tools of microbiota manipulation, including probiotics, prebiotics, synbiotics, and fecal microbiota transplantation, have been investigated, demonstrating encouraging improvements in cachexia outcomes. Albeit pioneering, these studies pave the way for future research with the aim of exploring the role of gut microbiota in cancer cachexia more deeply and setting up effective microbiota-targeting interventions to be translated into clinical practice.

    Abstract The goal of estimating a soundscape index, aimed at evaluating the contribution of the environmental sound components, is to provide an accurate "acoustic quality" assessment of a complex habitat. Such an index can prove to be a powerful ecological tool associated with both rapid on-site and remote surveys. The soundscape ranking index (SRI), introduced by us recently, can empirically account for the contribution of different sound sources by assigning a positive weight to natural sounds (biophony) and a negative weight to anthropogenic ones. The optimization of such weights was performed by training four machine learning algorithms (decision tree, DT; random forest, RF; adaptive boosting, AdaBoost; support vector machine, SVM) over a relatively small fraction of a labeled sound recording dataset. The sound recordings were taken at 16 sites distributed over an area of approximately 22 hectares at Parco Nord (Northern Park) of the city Milan (Italy). From the audio recordings, we extracted four different spectral features: two based on ecoacoustic indices and the other two based on mel-frequency cepstral coefficients (MFCCs). The labeling was focused on the identification of sounds belonging to biophonies and anthropophonies. This preliminary approach revealed that two classification models, DT and AdaBoost, trained by using 84 extracted features from each recording, are able to provide a set of weights characterized by a rather good classification performance (F1-score = 0.70, 0.71). The present results are in quantitative agreement with a self-consistent estimation of the mean SRI values at each site that was recently obtained by us using a different statistical approach.

    Abstract We have performed a detailed analysis of the soundscape inside an urban park (located in the city of Milan) based on simultaneous sound recordings at 16 locations within the park. The sound sensors were deployed over a regular grid covering an area of about 22 hectares, surrounded by a variety of anthropophonic sources. The recordings span 3.5 h each over a period of four consecutive days. We aimed at determining a soundscape ranking index (SRI) evaluated at each site in the grid by introducing 4 unknown parameters. To this end, a careful aural survey from a single day was performed in order to identify the presence of 19 predefined sound categories within a minute, every 3 minutes of recording. It is found that all SRI values fluctuate considerably within the 70 time intervals considered. The corresponding histograms were used to define a dissimilarity function for each pair of sites. Dissimilarity was found to increase significantly with the inter-site distance in space. Optimal values of the 4 parameters were obtained by minimizing the standard deviation of the data, consistent with a fifth parameter describing the variation of dissimilarity with distance. As a result, we classify the sites into three main categories: "poor", "medium" and "good" environmental sound quality. This study can be useful to assess the quality of a soundscape in general situations.

    Abstract Stroke remains a major cause of death and disability worldwide. Identifying new circulating biomarkers able to distinguish and monitor common and rare cerebrovascular diseases that lead to stroke is of great importance. Biomarkers provide complementary information that may improve diagnosis, prognosis and prediction of progression as well. Furthermore, biomarkers can contribute to filling the gap in knowledge concerning the underlying disease mechanisms by pointing out novel potential therapeutic targets for personalized medicine. If many "conventional" lipid biomarkers are already known to exert a relevant role in cerebrovascular diseases, the aim of our study is to review novel "unconventional" lipid biomarkers that have been recently identified in common and rare cerebrovascular disorders using novel, cutting-edge lipidomic approaches.

    Abstract "Click" reactions have revolutionized research in many areas of science. However, a disadvantage of the high stability of the Click product is that identifying simple treatments for cleanly dissociating the latter under the same guiding principles, i.e., a "Clip" reaction, remains a challenge. This study demonstrates that electron-deficient alkynes, conveniently installed on methionine residues, can participate in well-known Click (nucleophilic thiol-allene addition) and subsequent Clip reactions (radical thiol-ene addition). To illustrate this concept, a variety of bioconjugates (peptide-peptide; peptide-fluorophore; peptide-polymer; and peptide-protein) were prepared. Interestingly, the Clip reaction of these bioconjugates releases the original peptides concurrent with regeneration of their unmodified methionine residue, in minutes. Moreover, the conjugates demonstrate substantial stability toward endogenous levels of reactive species in bacteria, illustrating the potential for this chemistry in the biosciences. The reaction conditions employed in the Click and Clip steps are compatible with the preservation of the integrity of biomolecules/fluorophores and involve readily accessible reagents and the natural functional groups on peptides/proteins.

    Abstract Ring Finger Protein 213 (RNF213), also known as Mysterin, is the major susceptibility factor for Moyamoya Arteriopathy (MA), a progressive cerebrovascular disorder that often leads to brain stroke in adults and children. Although several rare RNF213 polymorphisms have been reported, no major susceptibility variant has been identified to date in Caucasian patients, thus frustrating the attempts to identify putative therapeutic targets for MA treatment. For these reasons, the investigation of novel biochemical functions, substrates and unknown partners of RNF213 will help to unravel the pathogenic mechanisms of MA and will facilitate variant interpretations in a diagnostic context in the future. The aim of the present review is to discuss novel perspectives regarding emerging RNF213 roles in light of recent literature updates and dissect their relevance for understanding MA and for the design of future research studies. Since its identification, RNF213 involvement in angiogenesis and vasculogenesis has strengthened, together with its role in inflammatory signals and proliferation pathways. Most recent studies have been increasingly focused on its relevance in antimicrobial activity and lipid metabolism, highlighting new intriguing perspectives. The last area could suggest the main role of RNF213 in the proteasome pathway, thus reinforcing the hypotheses already previously formulated that depict the protein as an important regulator of the stability of client proteins involved in angiogenesis. We believe that the novel evidence reviewed here may contribute to untangling the complex and still obscure pathogenesis of MA that is reflected in the lack of therapies able to slow down or halt disease progression and severity.

    Abstract The-growing influence of urbanisation on green areas can greatly benefit from passive acoustic monitoring (PAM) across spatiotemporal continua to provide biodiversity estimation and useful information for conservation planning and development decisions. The capability of eco-acoustic indices to capture different sound features has been harnessed to identify areas within the Parco Nord of Milan, Italy, characterised by different degrees of anthropic disturbance and biophonic activity. For this purpose, we used a network of very low-cost sensors distributed over an area of approximately 20 hectares to highlight areas with different acoustic properties. The audio files analysed in this study were recorded at 16 sites on four sessions during the period 25-29 May (2015), from 06:30 a.m. to 10:00 a.m. Seven eco-acoustic indices, namely Acoustic Complexity Index (ACI), Acoustic Diversity Index (ADI), Acoustic Evenness Index (AEI), Bio-Acoustic Index (BI), Acoustic Entropy Index (H), Normalized Difference Soundscape Index (NSDI), and Dynamic Spectral Centroid (DSC) were computed at 1 s integration time and the resulting time series were described by seven statistical descriptors. A dimensionality reduction of the indices carrying similar sound information was obtained by performing principal component analysis (PCA). Over the retained dimensions, describing a large (∼80%) variance of the original variables, a cluster analysis allowed discriminating among sites characterized by different combination of eco-acoustic indices (dimensions). The results show that the obtained groups are well correlated with the results of an aural survey aimed at determining the sound components at the sixteen sites (biophonies, technophonies, and geophonies). This outcome highlights the capability of this analysis of discriminating sites with different environmental sounds, thus allowing to create a map of the acoustic environment over an extended area.

    Abstract Objective: Restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) is the surgical gold standard in patients with ulcerative colitis (UC). Results are generally satisfactory but there is a significant rate of patients who experience postoperative complications. The aims of our study were to identify the pre- and intraoperative risk factors and their correlation with the reported outcomes.
    Patients and methods: A retrospective study was conducted on the medical records of all consecutive patients undergoing restorative proctocolectomy with IPAA for UC in our center from 2010 to 2021. Pre- and intraoperative factors were examined and correlated with pouchitis, endoscopic pouchitis, pouch failure, anastomotic leak, postoperative complications classified according to Clavien-Dindo score and stoma outlet obstruction. A univariate and multivariate statistical analysis was performed.
    Results: Out of 75 patients undergoing 3- or 2-stage IPAA surgery, the coexistence of extraintestinal clinical manifestations and preoperative topical rectal stump therapy for active proctitis were significantly associated with the occurrence of pouchitis (OR=4.4, p=0.03 and OR=7.6, p=0.01). Endoscopic pouchitis was found to be related to preoperative topical rectal therapy (OR=10.2, p=0.007), but not to extraintestinal manifestations of disease. Anastomotic leak was found to be significantly related to pouch failure (OR=22.7, p=0.007). Surgical indication for malignancy increased the risk for early complications (Clavien-Dindo >2) (OR=16.0, p=0.04). Young age was associated with the occurrence of outlet stoma obstruction in patients with recent IPAA surgery (OR=0.97, p=0.05).
    Conclusions: Based on observed results, an appropriate preoperative patient assessment aimed at detecting specific risk factors is crucial to identify early or prevent worse outcomes in patients undergoing IPAA surgery.
