    Abstract Observations from the Local and Non-local Fog EXperiment field campaign are used to investigate the formation and initial development of radiation fog. Data from six locations have been selected: five sites in Shropshire, and one in Bedfordshire, UK. These data show that during night-time clear-sky conditions, thermally-stable radiation fog only forms when the vertical velocity variance near the ground (at 2 m) decreases to 0.002–0.005 m2 s−2 or less. The variation in this threshold appears to be a function of the vertical relative humidity gradient near the ground. When the vertical velocity variance exceeds these values, saturation may still occur near the ground, but fog formation is inhibited due to the more efficient transport of moisture to the ground in the form of dew. Furthermore, once stable radiation fog forms, any increase in the vertical velocity variance to values greater than 0.002–0.005 m2 s−2 may cause the fog to dissipate. The subsequent development of shallow stable fog into deeper, weakly-unstable fog is also examined. Possible mechanisms for this are discussed, including in situ processes and non-local larger scale processes. Clear evidence of the latter is presented, but the relative importance of in situ and non-local processes for fog deepening requires further clarification.

    Abstract An observational study has been conducted concerning atmospheric dewfall and fog-droplet deposition with application to the formation and maintenance of fog layers. The relationship between dew and fog is discussed together with the challenges and requirements to measure representative values of their deposition to the surface. A practical instrument developed at the UK Met Office Research Unit, Cardington, is described. The instrument is a small portable device that uses a load cell to measure the weight of a pan upon which various types of natural and artificial canopies can be placed, and can measure dewfall and fog-droplet deposition to an accuracy of 0.0005 mm. Dewfall results from this device are shown for a selection of nights under varying conditions. On a given night the overriding factor determining the amount of dew deposition appears to be location. Several dewmeter devices were placed at different locations around the 18 ha Cardington field site for various clear nights and it was found that dew amounts varied significantly, depending on location: canopies with a more open aspect experienced more deposition by up to a factor of two. The results also suggest that the hygroscopic effect of a canopy, whereby water is absorbed into the canopy and topsoil layer before dew formation begins, is also important for the removal of atmospheric water vapour. Results indicate this effect can be of a similar magnitude to dew deposition. Measurements of fog-droplet deposition showed total water deposition rates did not change when thin radiation fog formed. When optically thick adiabatic fog formed, deposition rates were seen to decrease with time or be generally lower than for thinner radiation fog. Further observations are required to establish if the behaviours found are typical for all fogs.

    Abstract The granulation pattern of somatotroph adenomas is well known to be associated with differing clinical and biochemical characteristics, and it has been shown that sparsely granulated tumours respond poorly to commonly used somatostatin receptor ligands (SRLs). We report a challenging case of acromegaly with a sparsely granulated tumour resistant to multiple modalities of treatment, ultimately achieving biochemical control with pasireotide. A 26-year-old lady presented with classical features of acromegaly, which was confirmed by an oral glucose tolerance test. Insulin-like growth factor 1 (IGF1) was 1710 µg/L (103-310 µg/L) and mean growth hormone (GH) was >600 U/L. MRI scan showed a 4 cm pituitary macroadenoma with suprasellar extension and right-sided cavernous sinus invasion. She underwent trans-sphenoidal pituitary surgery. Histology displayed moderate amounts of sparsely granular eosinophilic cytoplasm, staining only for GH. Postoperative investigations showed uncontrolled disease (IGF1:1474 µg/L, mean GH:228 U/L) and residual tumour in the cavernous sinus. She received external beam fractionated radiation. Over the years, she received octreotide LAR (up to 30 mg), lanreotide (up to 120 mg) two weekly, cabergoline, pegvisomant and stereotactic radiosurgery to no avail. Only pegvisomant resulted in an element of disease control; however, this had to be stopped due to abnormal liver function tests. Fifteen years after the diagnosis, she was started on pasireotide 40 mg monthly. Within a month, her IGF1 dropped and has remained within the normal range (103-310 µg/L). Pasireotide has been well tolerated, and there has been significant clinical improvement. Somatostatin receptor subtyping revealed a positivity score of two for both sst5 and sst2a subtypes.
    Learning points: Age, size of the tumour, GH levels on presentation, histopathological type and the somatostatin receptor status of the tumour in acromegaly should be reviewed in patients who poorly respond to first-generation somatostatin receptor ligands.Tumours that respond poorly to first-generation somatostatin receptor ligands, especially sparsely granulated somatotroph adenomas, can respond to pasireotide and treatment should be considered early in the management of resistant tumours.Patients with membranous expression of sst5 are likely to be more responsive to pasireotide.
