    Abstract The lockdown and related measures implemented by many European countries to stop the spread of the SARS-CoV-2 virus (COVID-19) pandemic have altered the economic activities and road transport in many cities. To rigorously evaluate how these measures have affected air quality in Europe, we developed Bayesian spatio-temporal (BST) models that assess changes in the surface nitrogen dioxide (NO₂) and fine particulate matter (PM₂.₅) concentration across the continent. We fitted BST models to measurements of the two pollutants in 2020 using a lockdown indicator covariate, while accounting for the spatial and temporal correlation present in the data. Since other factors, such as weather conditions, local combustion sources and/or land surface characteristics may contribute to the variation of pollutant concentrations, we proposed two model formulations that allowed the differentiation between the variations in pollutant concentrations due to seasonality from the variations associated to the lockdown policies. The first model compares the changes in 2020, with the ones during the same period in the previous five years, by introducing an offset term, which controls for the long-term average concentrations of each pollutant during 2014–2019. The second approach models only the 2020 data, but adjusts for confounding factors. The results indicated that the latter can better capture the lockdown effect. The measures taken to tackle the virus in Europe reduced the average surface concentrations of NO₂ and PM₂.₅ by 29.5% (95% Bayesian credible interval: 28.1%, 30.9%) and 25.9% (23.6%, 28.1%), respectively. To our knowledge, this research is the first to account for the spatio-temporal correlation present in the monitoring data during the pandemic and to assess how it affects estimation of the lockdown effect while accounting for confounding. The proposed methodology improves our understanding of the effect of COVID-19 lockdown policies on the air pollution burden across the continent.

    Abstract Background: Airflow limitation is a hallmark of chronic obstructive pulmonary disease, which can develop through different lung function trajectories across the life span. There is a need for longitudinal studies aimed at identifying circulating biomarkers of airflow limitation across different stages of life.
    Objectives: This study sought to identify a signature of serum proteins associated with airflow limitation and evaluate their relation to lung function longitudinally in adults and children.
    Methods: This study used data from 3 adult cohorts (TESAOD [Tucson Epidemiological Study of Airway Obstructive Disease], SAPALDIA [Swiss Cohort Study on Air Pollution and Lung and Heart Diseases in Adults], LSC [Lovelace Smoker Cohort]) and 1 birth cohort (TCRS [Tucson Children's Respiratory Study]) (N = 1940). In TESAOD, among 46 circulating proteins, we identified those associated with FEV
    Results: After false discovery rate adjustment, serum levels of 5 proteins (HP, carcinoembryonic antigen, ICAM1, CRP, TIMP1) were associated with percent predicted levels of FEV
    Conclusions: A signature of 5 circulating biomarkers of airflow limitation was associated with both impaired lung function growth in childhood and accelerated lung function decline in adult life, indicating that these proteins may be involved in multiple lung function trajectories leading to chronic obstructive pulmonary disease.

    Abstract Mit der Entschlüsselung des menschlichen Genoms waren Erwartungen im Hinblick auf die medizinischen Anwendungen der neuen Erkenntnisse verbunden, die sich nicht in dem erhoffen Ausmaß erfüllt haben. Der klinische Nutzen von Gentests im Bereich von familiär gehäuft auftretenden Krebsarten für die gesunden Familienmitglieder ist unumstritten, wenn sich hierdurch das Erkrankungsrisiko mit Hilfe präventiver Interventionen substantiell senken lässt. So lässt sich das Brust- und Eierstockkrebsrisiko von Trägerinnen einer BRCA1- oder BRCA2-Mutation dank prophylaktischer Mastektomie oder Ovarektomie substantiell senken. In den letzten Jahren beschäftigte sich die Forschung aber vor allem mit der Entschlüsselung der genetischen Ursachen sporadisch auftretender, komplexer Krebsarten. Mit Hilfe von genomweiten, hypothesenfreien Assoziationsstudien gelang es, verschiedenste Chromosomenregionen mit der Entstehung von Krebs in Verbindung zu bringen. Auf diese Weise wurden zwar neue Erkenntnisse über Krankheitsmechanismen gewonnen, die später grundlegend zur Entwicklung von Diagnostika und Medikamenten beitrugen. Es fehlt jedoch bislang die Evidenz, dass sich die ermittelten, assoziierten SNPs (Single Nucleotide Polymorphisms) auch zur individuellen Risikovorsagen oder zur Stratifizierung der Bevölkerung in Gruppen mit unterschiedlichem Präventions- oder Screening-Bedarf verwenden lassen.

    Abstract The promises resulting from the decoding of the human genome have not been fulfilled to the extent as expected. At the same time it is fair to say that the results of recent genetic research have not been useless. In the area of ​​​​familial cancer the clinical benefit of genetic testing for healthy family members is very convincing, where the risk of disease can be reduced substantially through preventive interventions. For example, prophylactic mastectomy and premenopausal ovariectomy can reduce the cancer risk in carriers of BRCA1 and BRCA2 mutations dramatically. In recent years, the research has moved towards the decoding of the genetic causes of sporadic cancers as well. Genome-wide and hypothesis-free association studies have linked different chromosome regions to cancer. By this, new insights into disease mechanisms could be gained, an important requirement for the development of diagnostics and drugs. However, what is missing is the evidence that the associated SNPs (Single Nucleotide Polymorphisms) could be useful for individual risk calculation or for stratification of the population into groups with different preventive or screening needs.

    Abstract Background: Recognized public health relevant risk factors such as obesity, physical inactivity, smoking or air pollution are common to many non-communicable diseases (NCDs). NCDs cluster and co-morbidities increase in parallel to age. Pleiotropic genes and genetic variants have been identified by genome-wide association studies (GWAS) linking NCD entities hitherto thought to be distant in etiology. These different lines of evidence suggest that NCD disease mechanisms are in part shared.
    Discussion: Identification of common exogenous and endogenous risk patterns may promote efficient prevention, an urgent need in the light of the global NCD epidemic. The prerequisite to investigate causal risk patterns including biologic, genetic and environmental factors across different NCDs are well characterized cohorts with associated biobanks. Prospectively collected data and biospecimen from subjects of various age, sociodemographic, and cultural groups, both healthy and affected by one or more NCD, are essential for exploring biologic mechanisms and susceptibilities interlinking different environmental and lifestyle exposures, co-morbidities, as well as cellular senescence and aging. A paradigm shift in the research activities can currently be observed, moving from focused investigations on the effect of a single risk factor on an isolated health outcome to a more comprehensive assessment of risk patterns and a broader phenome approach. Though important methodological and analytical challenges need to be resolved, the ongoing international efforts to establish large-scale population-based biobank cohorts are a critical basis for moving NCD disease etiology forward.
    Summary: Future epidemiologic and public health research should aim at sustaining a comprehensive systems view on health and disease. The political and public discussions about the utilitarian aspect of investing in and contributing to cohort and biobank research are essential and are indirectly linked to the achievement of public health programs effectively addressing the global NCD epidemic.

    Abstract Lung function, strongly associated with morbidity and mortality, decreases with age. This study examines whether poor adult lung function is associated with age accelerations (AAs). DNA methylation (DNAm) based AAs, lifespan predictors (GrimAge and plasminogen activator inhibitor 1-PAI1) and their related age-adjusted measures were estimated from peripheral blood at two time points (8-to-11 years apart) in adults from two cohorts: SAPALDIA (n=987) and ECRHS (n=509). Within each cohort and stratified by gender (except for estimators from GrimAge and PAI1), AAs were used as predictors in multivariate linear regression with cross-sectional lung function parameters, and in covariate-adjusted mixed linear regression with longitudinal change in lung function and meta-analysed.AAs were found cross-sectionally associated with lower mean FEV1 (Forced Expiratory Volume in one second) (AA-residuals:P-value=4x10

    Abstract Background: Low lung function has been associated with increased body mass index (BMI). The aim of this study was to investigate whether the effect of BMI on lung function is mediated by DNA methylation.
    Methods: We used individual data from 285,495 participants in four population-based cohorts: the European Community Respiratory Health Survey, the Northern Finland Birth Cohort 1966, the Swiss Study on Air Pollution and Lung Disease in Adults, and the UK Biobank. We carried out Mendelian randomisation (MR) analyses in two steps using a two-sample approach with SNPs as instrumental variables (IVs) in each step. In step 1 MR, we estimated the causal effect of BMI on peripheral blood DNA methylation (measured at genome-wide level) using 95 BMI-associated SNPs as IVs. In step 2 MR, we estimated the causal effect of DNA methylation on FEV
    Results: In step 1, we found that BMI might have a small causal effect on DNA methylation levels (less than 1% change in methylation per 1 kg/m2 increase in BMI) at two CpGs (cg09046979 and cg12580248). In step 2, we found no evidence of a causal effect of DNA methylation at cg09046979 on lung function. We could not estimate the causal effect of DNA methylation at cg12580248 on lung function as we could not find publicly available data on the association of this CpG with SNPs.
    Conclusions: To our knowledge, this is the first paper to report the use of a two-step MR approach to assess the role of DNA methylation in mediating the effect of a non-genetic factor on lung function. Our findings do not support a mediating effect of DNA methylation in the association of lung function with BMI.
