    Abstract In the context of inflammation, T cell activation occurs by the concerted signals of the T cell receptor (TCR), co-stimulatory receptors ligation, and a pro-inflammatory cytokine microenvironment. Fine-tuning these signals is crucial to maintain T cell homeostasis and prevent self-reactivity while offering protection against infectious diseases and cancer. Recent developments in understanding the complex crosstalk between the molecular events controlling T cell activation and the balancing regulatory cues offer novel approaches for the development of T cell-based immunotherapies.     Abstract The main objective of this study was to compare the effect of season on serum and intrafollicular concentrations of melatonin (MT). The relationship between follicular size, MT and estradiol (E2) concentrations was also explored. Twelve mares were used. Aspiration of fluid from all follicles larger than 5 mm was carried out in each mare during the breeding season and during the anovulatory season. All aspirations were carried out between 7AM and 8AM, and a serum sample was obtained at the same time. Follicles were classified into small (≤ 15 mm); medium (16-30 mm); and large (> 30 mm). MT and E2 concentrations were measured by enzymatic immunoassay (Melatonin ELISA Mybiosource, MBS1603596; Estradiol ELISA DRG, EIA-2693). Analysis of Variance was used to evaluate the effects of season, fluid type (follicular vs serum) and follicle size. Intrafollicular concentrations of MT were higher than serum concentrations both in the reproductive season (127±1.41 vs 69.59±1.73 ng/L) and in the anovulatory season (118.5±2.93vs 115.45±3.06 ng/L). MT concentrations in serum were higher during the anovulatory season than during the reproductive season (p<0.05), but the opposite was the case for intrafollicular MT. During the ovulatory season MT was higher (p<0.05) in large follicles (141.39±2.13ng/L) than in medium (130.25±2.55 ng/L) and small follicles (109.36±2.57 ng/L). The difference between medium and small follicles was also significant(p<0.05). In contrast during the anovulatory season MT concentration was not different between small and medium follicles (123.43±3.79 vs 109.92±5.55 ng/mL). Intrafollicular levels of E2 were higher during the ovulatory season than during the anovulatory season in medium (211.89±20.4 vs 126.18±0.07) and small follicles (67.42±2.4 vs 14.78±1.06 ng/mL). E2 concentrations in large follicles during the ovulatory season was 4,312.83±65.8 ng/mL. The results of this study indicate that while serum MT concentrations were different between seasons due to the known negative correlation between pineal MT secretion and day length, this seasonal effect was not found in the case of intrafollicular MT concentrations. Also, in this study there was no correlation between serum and intrafollicular MT concentrations. Together, these findings suggest photoperiod-independent intrinsic follicular MT synthesis. E2 concentrations were correlated with follicle size in both ovulatory and anovulatory seasons but were always higher during the ovulatory season for a given size. During this season there was a positive correlation between intrafollicular MT and estradiol concentrations, thus suggesting that intrafollicular estradiol may stimulate intrafollicular melatonin synthesis. It is concluded that intrafollicular MT concentrations in the mare are at least in part of intrinsic origin and may be stimulated by estradiol.

    Abstract Investigating emotions relies on pre-validated stimuli to evaluate induced responses through subjective self-ratings and physiological changes. The creation of precise affect models necessitates extensive datasets. While datasets related to pictures, words, and sounds are abundant, those associated with videos are comparatively scarce. To overcome this challenge, we present the first virtual reality (VR) database with continuous self-ratings and physiological measures, including facial EMG. Videos were rated online using a head-mounted VR device (HMD) with attached emteqPRO mask and a cinema VR environment in remote home and laboratory settings with minimal setup requirements. This led to an affective video database with continuous valence and arousal self-rating measures and physiological responses (PPG, facial-EMG (7x), IMU). The AVDOS-VR database includes data from 37 participants who watched 30 randomly ordered videos (10 positive, neutral, and negative). Each 30-second video was assessed with two-minute relaxation between categories. Validation results suggest that remote data collection is ecologically valid, providing an effective strategy for future affective study designs.     Abstract Objective: We report a case of unilateral adrenal hemorrhage resulting in adrenal insufficiency in a patient on rivaroxaban for thromboprophylaxis after total knee arthroplasty.
    Methods: The clinical presentation, laboratory findings, and outcome of the case are described. An overview of the etiology, diagnosis, and treatment of adrenal hemorrhage is reviewed.
    Results: A 61-year-old male underwent bilateral total knee replacements and postoperatively was started on rivaroxaban at 10 mg daily for thromboprophylaxis. About 2 weeks after the procedure, the patient presented with abdominal pain, nausea, vomiting, and generalized weakness. Abdominal computed tomography scan identified an ill-defined heterogeneous density in the left supra-renal fossa, and an abdominal magnetic resonance imaging scan identified diffuse thickening of the left adrenal gland with periadrenal fat stranding suggestive of adrenal hemorrhage. Adrenal insufficiency was suspected and confirmed with a cosyntropin stimulation test. Pre-stimulus cortisol level was 2.7 μg/dL and 60 minutes after cosyntropin administration it was 2.5 μg/dL. The patient was started on intravenous hydrocortisone at 100 mg every 6 hours with improvement of symptoms and discharged on oral hydrocortisone at 20 mg twice daily. Follow up abdominal magnetic resonance imaging 6 weeks after discharge demonstrated resolution of the left adrenal hemorrhage.
    Conclusion: A high clinical suspicion and early detection of adrenal hemorrhage in a patient on anticoagulation can allow for earlier intervention and improved outcomes.

    Abstract Rapid eye movement (REM) behavior disorder (RBD) is characterized by loss of skeletal muscle atonia that can lead to dream enactment. This condition can cause harm to patients and their bed partners if appropriate safety measures are not ensured. This condition is often the initial presenting symptom in a group of complex neurodegenerative processes. Definitive diagnosis requires a thorough history and an in-laboratory polysomnogram to look for evidence of REM sleep without atonia. Treatment options are limited but consist of sleep safety measures and pharmacotherapy. Patients diagnosed with idiopathic RBD associated with alpha-synucleinopathy are likely to have progression of disease.

    Abstract Background: Although laparoscopic lavage for perforated diverticulitis with peritonitis has been grabbing the headlines, it is known that the clinical presentation of peritonitis can also be caused by an underlying perforated carcinoma. The aim of this study was to determine the incidence of patients undergoing inadvertent laparoscopic lavage of perforated colon cancer as well as the delay in cancer diagnosis.
    Methods: The PubMed database was systematically searched to include all studies meeting inclusion criteria. Studies were screened through titles and abstracts with potentially eligible studies undergoing full-text screening. The primary endpoints of this meta-analysis were the rates of perforated colon cancer patients having undergone inadvertent laparoscopic lavage as well as the delay in cancer diagnosis. This was expressed in pooled rate % and 95% confidence intervals.
    Results: Eleven studies (three randomized, two prospective, six retrospective) totaling 642 patients met inclusion criteria. Eight studies reported how patients were screened for cancer and the number of patients who completed follow-up. The pooled cancer rate was 3.4% (0.9%, 5.8%) with low heterogeneity (Isquare2 = 34.02%) in eight studies. Cancer rates were 8.2% (0%, 3%) (Isquare2 = 58.2%) and 1.7% (0%, 4.5%) (Isquare2 = 0%) in prospective and retrospective studies, respectively. Randomized trials reported a cancer rate of 7.2% (3.1%, 11.2%) with low among-study heterogeneity (Isquare2 = 0%) and a median delay to diagnosis of 2 (1.5-5) months.
    Conclusions: This systematic review found that 7% of patients undergoing laparoscopic lavage for peritonitis had perforated colon cancer with a delay to diagnosis of up to 5 months.
