    Abstract Aim: Anxiety is a psychological and physiological clinical state characterized by somatic, emotional, cognitive, and behavioral components. This disturbance may causes clinically significant distress or impairment in social, occupational or other important areas of functioning. Recurrent anxiety is a serious clinical entity in patients with spontaneous pneumothorax, and some patients may have strong anxiety and avoid risky activities. In this study, we searched for answer to the question “could recurrent anxiety be a new surgical indication for patients with spontaneous pneumothorax?”. Material and Method: A total 36 patients with spontaneous pneumothorax were evaluated, the data of frequency of admission without any recurrence due to the recurrent anxiety were collected, and the patients were evaluated with chest radiography in each admission. Results: In preoperative period 19 (52.8%) patients, in postoperative period 2 (5.6%) patients admitted to the hospital due to their recurrent anxiety in follow-up period. There were approximately 9.5-fold difference between the frequency of admission to the hospital in preoperative and postoperative period. Discussion: Surgical approach may be preferred in post episode whose have declining quality of life and serious recurrent anxiety in patients with spontaneous pneumothorax. Especially thoracoscopic surgery must be the preferred in the first episode of spontaneous pneumothorax in young and healthy patients.

    Abstract Objective: Traumatic first rib fracture is associated with multisystem injuries,morbidity and mortality. We wish to report thoracic and extrathoracic injuries related with traumatic first rib fracture in this study. Material and Methods: Due to rib fractures, 162 cases were hospitalized between June 2008-April 2011, 11 (6.8%) of these cases were accompanied by first rib fracture. The first rib fracture patients were evaluated according to gender, age, etiologies of trauma, thoracic and extrathoracic injuries, diagnostic and treatment modalities, morbidity and mortality. All patients were evaluated by a multidisciplinary trauma team in the emergency department.Results: Eight cases (72.7%) were male and 3 (27.3%) were female, ages ranged from 27 to 67 age, the average was 36.7. All of the cases were blunt trauma. 10 (90.9%) cases included thoracic injuries, 3 (27.3%) cases had no extrathoracic injuries. There was no any vascular, neurological injuries and mortality. Conclusion: The first rib fracture is associated with serious thoracic and extrathoracic injuries, but not always with vascular, neurological injuries and mortality. The trauma team must know the potential severity of the first rib fracture. A multidisciplinary approach, early diagnosis, appropriate treatment and observation in the intensive care unit may be prevent morbidity and/or mortality.

    Abstract Neurofibromatosis (NF) is an autosomal dominant inheritance or spontaneous mutation disease. Skin, eyes, central and peripheral nervous system lesions are characteristic. It is evaluated NF type 1 and NF type 2. NF-1 is the most common type of the disease, and described by von Recklinghausen in 1882. Cutaneous and subcutaneous neurofibromas, kyphoscoliosis, rib deformities, thoracic neoplasms, bullous, emphysematous and fibrotic changes could be seen on chest wall and lung parenchyma. We aimed to report this case with a rare cause of lung emph-ysema and the effects of the involvement of the NF-1 to the lung parenchyma.

    Abstract Aim: Both lungs and liver hydatid cysts is still an important health problem in our country. In this study we aimed to present the superiority of transthoracic approach to 24 adults right lung and liver dome hydatid cyst Material and Methods: Between September 2008 - June 2010 in our clinic was performed by transthoracic approach to 24 adult patients with simultaneous right lung and liver dome hydatid cyst in a single stage. Cases diagnosed by clinical evaluation, chest radiograph, chest computed tomography, abdominal ultrasonography and abdominal computed tomography. 19 cases (79.17%) were male and 5 cases (20.83%) were women, age range was 17-35 and average was 22±2,8. Results: The most common findings with 19 patients (79.17%) were chest pain. 6 () patients have bilateral lung hydatid cysts. 17 (70.83%) patients in the right lower lobe, 7 (29.17%) patients in the right middle lobe, 5 (20.83%) patients in the left lower lobe in 1 (4.16%) patient in the left upper lobe in lung, the hepatic lesions were all of the dome located. Conclusions :Single stage transthoracic approach is prevent the second surgical procedures on simultaneous right lung and liver dome hydatid cyst and it is safe and effective method.
