    Abstract Background: Primary chronic pain (PCP), a relatively new classification, characterizes pain that is not a secondary response to an underlying primary condition such as trauma or cancer. This study explored the lived experience of adolescents with a diagnosis of PCP.
    Method: A qualitative methodology, Interpretative Description (ID), was used to guide our study. ID uses a constructivist approach and allows for clinician experience to guide a theoretical scaffold of inquiry, which can be refined as the data collection progresses. We interviewed fifteen adolescents (n = 15) living with PCP.
    Results: All participants in this sample reported struggling with diagnostic uncertainty, depression, and anxiety. Adding to their distress was the fact that our participants perceived that health care professionals did not believe them when they described their pain and its intensity.
    Conclusions: While significant research is being conducted on PCP, participants believe there is a lack of knowledge about PCP as a diagnosis and thus there are limited resources and a lack of empathy and understanding for these adolescents.

    Abstract There is limited evidence that socially responsible investment (SRI) strategies can resolve persistent concerns brought up in scholarship on the industry, particularly as it relates to considerations of justice. It is critical that SRI initiatives be interrogated about their broader impacts on environmental inequality and justice in the context of global power relations. Drawing upon environmental justice (EJ) theory, we propose a framework for transformative investment to halt the exploitation of humans and environment in pursuit of profit. We posit that transformative investment initiatives, including those related to strategies of screening and shareholder resolutions, would reflect three general conditions of environmental justice. They would require (1) cumulative responsibility that focuses on holistic gains so that investment initiatives do not replace one harm with another; (2) embedded accountability that is rooted in the decision-making and concerns of directly impacted communities; and (3) counter-hegemonic practices that are integrated with broader strategies and movements that challenge structures of oppression and exploitation. Through these principles, we address problems with SRI that have already been identified in scholarship on the subject. Moreover, we draw upon Antonio Gramsci's concept of 'strategic power' to conceptualize the conditions and actions necessary for transformative investment to take root.

    Abstract A geologist, M. King Hubbert, came up with a logistics trend to explain his observation that oil and mineral exploration and production for a region followed a certain pattern of first increasing production, then a peak in production followed by a decline in production. Critics claimed that the postulated Hubbert trend did not statistically work. However, rather like how free markets work in principle but often fail in reality due to institutional effects (like a failed regulation), externalities (like pollution), and market power problems (like a monopoly), so likewise Hubbert’s original trend could be affected by outside influences that simply needed to be conceptualized. And since Hubbert was a geologist, rather than an economist, it took some time for the two disciplines, geology and economics, to actually talk to each other and indeed to interchange among themselves in order to postulate an overarching theory. This treatise, then, is the result of that interchange and explains the economic theory and antithetical influences of the Hubbert Curve. It is about U.S. shale-oil production trends in particular, and other Hubbert related theories in general, and was accepted to be given as an academic presentation at the 43rd International Association for Energy Economics, (IAEE)’s annual International Energy Conference that was to take place in Paris, France, at the Palais des Congrès, on 21 – 24 June 2020, which has been cancelled due to COVID-19 and Corona-Virus concerns. The presentation was to pertain to important potential petroleum supply side production changes in the U.S. and therefore for the World’s oil market.

    This treatise is an addendum to Reynolds’ (2011). It looks at the U.S. shale-oil production trend, and specifically at the Hubbert peak of that trend. Simmons (2005), Deffeyes (2001), Hubbert, (1962), Norgaard (1990) and Campbell (1997), among others show how there can be a peak in oil production. Reynolds (2002, 2009) explains the economic and cost theory for how and why the Hubbert Curve works, including how the information and depletion effects create such a curve. Nevertheless, Maugeri (2007), Adelman and Lynch (1997), and Lynch (2002) suggest that one should never curve fit an oil production trend, contrary to most economic disciplines where curve fitting using econometrics is the norm. Although, as of early 2020 the COVID-19 recession is greatly affecting petroleum markets. Nevertheless, the Hubbert supply trend is relevant. Also, Reynolds and Umekwe (2019) show that shale-gas and shale-oil can be compliments or substitutes in production. Based on that relationship, once the U.S. shale-oil peak occurs, it may be the world’s ultimate Hubbert peak with much smaller and lower Hubbert cycles thereafter. Worldwide petroleum institutions and strategies will also change. This treatise estimates a U.S. shale-oil Hubbert peak, scrutinizes the Hubbert related theories and explores oil price forecasts, taking into account medium run COVID-19 oil demand effects.

    Abstract Given the length, depth and breadth of this treatise, some readers may need to pick and choose what they read at any given time, such as the idea of cognitive dissonance when choosing oil tax credits that can inadvertently end up giving corporate subsidies to the oil industry in Section 4b. Another important topic is the summary, in Section 8a, of how to make monopoly electric utilities more efficient by using a CEO bonus mechanism. Also, a parallel history of the U.S. and Alaska will be interesting to many in Section 9. Certainly some will be interested in the idea of a state owned oil company summarized in section 10 with greater detail provided in Sections 6, 6a and 6b, especially since it’s incentivized setup is counterfactual to Socialism. The issue of how the state should deal with the Trans-Alaska Pipeline System (TAPS) is summarized in the Part IV Analysis introduction and explained in more detail in Section 2. While the explanation of tax credits in Section 3, 3a and 3b (the Gross Value Reduction Credits) and Section 4, 4a and 4b (General Tax Credits) are detailed, a quick analysis can be had in Section 1. Indeed, assorted readers will quite like the substitutes versus compliments in production analysis of shale resources explained in Sections 3a and 3b. Anyone interested in Alaska’s public education will want to look at Section 5. Plus, Section 7, 7a and 7b explain important aspects of the state’s economic development.

    This paper in the form of a treatise is about how to improve Alaska’s overall socio-economic welfare. It explains economic issues in Alaska starting with the Trans-Alaska (oil) Pipeline System (TAPS) and how TAPS interacts with Alaska’s oil industry and induces risk averse reactions by the state. It also explains how an alternative oil pipeline can replace TAPS in order to reduce Alaska’s expensive oil tax credits. The treatise also explains some of the issues surrounding how the oil tax credits work or don’t work including such interactions as how oil exploration is carried out, why shale-oil will not easily be developed in Alaska and how the credits subsidize Anchorage’s area energy costs to the detriment of the state as a whole. Ideas for economic development of the state are given including building natural gas infrastructure and how to set up electric utilities to maximize their value to the state. An alternative for Anchorage energy needs is a simple natural gas pipeline to Fairbanks with rail connection to Anchorage and eventually the use of TAPS for natural gas. An incentivized management system for monopoly electric power utilities is explained which can provide better cheaper electric power and an incentivized management system for a state owned oil company is explained which can help Alaska negotiate with OPEC to Alaska’s advantage. Aspects of the university and education funding are explained.

    Abstract This podcast, and the working paper series treatise that goes with it, is essentially an addendum to the book, “Energy Civilization,” that I wrote in 2011.

    This paper, in the form of a Pod Cast, estimates a U.S. shale-oil production trend forecast and explores potential consequences of that trend on U.S. and World macroeconomic conditions and growth prospects. It explains the economics of the Hubbert curve including a literature review both pro and con. It explains the relationship of shale-oil and shale-gas. It falsifies various U.S. shale-oil trend hypotheses using logic and econometrics. It then presents oil price expectations based on an analyses of entropy-economic relationships, physical energy characteristics, new-institutional economic theories of OPEC, and OPEC+ game-theoretic plays. Covid-19, OPEC+ and macro-economic principles are analyzed for their potential market changing effects using Schwartzian futurology methodology. A comparison of the current global civilization to past civilizations is also carried out.

    Abstract This narrative review explores the physiology and evidence-based management of patients with severe acute respiratory distress syndrome (ARDS) and refractory hypoxemia, with a focus on mechanical ventilation, adjunctive therapies, and veno-venous extracorporeal membrane oxygenation (V-V ECMO). Severe ARDS cases increased dramatically worldwide during the Covid-19 pandemic and carry a high mortality. The mainstay of treatment to improve survival and ventilator-free days is proning, conservative fluid management, and lung protective ventilation. Ventilator settings should be individualized when possible to improve patient-ventilator synchrony and reduce ventilator-induced lung injury (VILI). Positive end-expiratory pressure can be individualized by titrating to best respiratory system compliance, or by using advanced methods, such as electrical impedance tomography or esophageal manometry. Adjustments to mitigate high driving pressure and mechanical power, two possible drivers of VILI, may be further beneficial. In patients with refractory hypoxemia, salvage modes of ventilation such as high frequency oscillatory ventilation and airway pressure release ventilation are additional options that may be appropriate in select patients. Adjunctive therapies also may be applied judiciously, such as recruitment maneuvers, inhaled pulmonary vasodilators, neuromuscular blockers, or glucocorticoids, and may improve oxygenation, but do not clearly reduce mortality. In select, refractory cases, the addition of V-V ECMO improves gas exchange and modestly improves survival by allowing for lung rest. In addition to VILI, patients with severe ARDS are at risk for complications including acute cor pulmonale, physical debility, and neurocognitive deficits. Even among the most severe cases, ARDS is a heterogeneous disease, and future studies are needed to identify ARDS subgroups to individualize therapies and advance care.

    Abstract In spring, when the mountainous snow cover becomes patchy, the strong multi-scale surface heterogeneity influences atmospheric heat exchange processes. At valley scale, thermally driven winds advect warm air towards snow-covered regions at higher elevations. Additionally, the difference in albedo between bare ground and adjacent snow patches causes strong surface temperature differences on a (sub-)meter scale. Consequently, the structure of the near-surface boundary layer is spatio-temporally highly variable. During spring 2021, we recorded data in a comprehensive field campaign in the Dischma valley close to Davos (CH). The dataset includes multiple eddy-covariance measurements at different measurement heights. The topographic setting allows to group the measurements into up valley and down valley flow systems. Using a multi-resolution spectral decomposition of turbulent flux variables, we investigate the structure and dynamics of near-surface turbulence. During up valley flows, the advection of warm air induces stable internal boundary layers (SIBL) adjacent to snow patches with near-neutral static stability above. The strong stability withing the SIBL eventually leads to decoupling of the near-surface turbulence from the submeso-scale motions aloft. In contrast, during down valley flows, stability dampens turbulence similarly at all measurement heights. In concert with those findings, measurements utilizing the IR-screen setup during the same campaign yield high spatio-temporally resolved air temperature profiles during phases of different stability. Furthermore, the screen measurements visualize the near-surface boundary layer dynamics. In the next step, we will use the gained process understanding to test new physical parameterizations especially for warm air advection in hectometer scale coupled snow-atmosphere models.
