    Abstract Introduction and purpose: Hemodialysis, as the most common renal replacement therapy in renal patients, leads to a variety of physical and psychological problems. Therefore, the aim of this study was to determine structural equation modeling of adherence to treatment based on personality traits mediated by locus of control in hemodialysis patients. Methods: The research method was descriptive and correlation research design was structural equation model. The statistical population of this study was all chronic kidney patients undergoing hemodialysis in Tehran in 2018, among whom 200 patients were selected by available sampling method. Data were collected using adherence to treatment scale, neo-personality questionnaire, and health locus of control questionnaire. To analyze the research data, correlation coefficient-Pearson and path analysis and SPSS.22 and AMOS.22 softwares were used. Results: The results showed that neuroticism (p=0.029), openness (p=0.031), conscientiousness (p=0.003), significant other locus of control (p=0.003) and internal locus of control (P=0.039) had a direct effect on adherence to treatment. The results also showed that the source of health control plays a mediating role in the relationship between personality traits and adherence to treatment. Conclusion: It can be concluded that the locus of control among patients undergoing hemodialysis mediates the relationship between neuroticism, conscientiousness and extraversion and adherence to treatment.

    Abstract Background and Objectives: As a health emergency of the 21st century, Diabetes is associated with an increased risk of cardiovascular diseases, stroke, diabetic nephropathy, neuropathy, retinopathy, pregnancy complications, psychological problems, lower quality of life experience, as well as a high economic burden and high risk of premature death. This study aimed to develop psychological interventions headings for patients referring to healthcare service centers based on local needs assessment through content analysis method of experts' opinion to provide services in the primary healthcare system. Material and Methods: This study was conducted on 19 mental health specialists, professors, and the health team providing services in healthcare service centers (family doctor and mental health expert) in 2018-19. The data obtained from the semi-structured interviews were analyzed using the directed content analysis technique. Results: The essential finding themes related to the closed headings of psychological interventions in diabetes for improving disease management included "collaborative care," "psychological assessment," the importance of "diabetes coexistence with psychiatric disorders," "creating health behaviors," "self-management training," and "intervention" In crisis" for the management of type 2 diabetes. Conclusion: This qualitative study was conducted with the participation of mental health experts. Therefore, the mentioned themes and categories can be used in developing the headings of psychological intervention programs for type 2 diabetic patients, focusing on the needs of Iranian patients and the structure of the health system.

    Abstract Background Mindfulness-based intervention has been found efficacious in reducing primary headaches and negative cognitive-related pain. However, little is known about the potential moderators in this regard. One of the most important moderators affecting the headaches is Alexithymia. Objective The present study investigated the moderating role of Alexithymia on the effect of Mindfulness- Based Cognitive Therapy (MBCT) on pain intensity in patients with primary headache. Methods A clinical trial was conducted in 2017 and 2018 at Imam Hossein Hospital of Tehran City, Iran. Using judgment sampling method, of 94 inpatients with chronic headache, 85 were selected and randomly assigned into the two groups of MBCT (43 patients) and control (42 patients). The Numerical Rating Scale (NRS) of Pain Self-Efficacy Questionnaire (PSEQ) and Pain Catastrophizing Scale (PCS) were applied to collect data. The obtained data were analyzed by Independent Samples t-test and Chi-squared test; longitudinal data were analyzed using linear mixed model analysis. Findings Statistically significant time×group interactions were found for pain intensity (P<0.001), selfefficacy (P<0.001), and catastrophizing (P<0.001). The obtained results indicated a significant reduction in pain, pain catastrophizing and increased self-efficacy. Conclusion MBCT is a potentially efficacious approach for individuals with headache. Alexithymia may have clinically relevant factors for identifying the patients who may benefit most from MBCT as a pain intervention.

    Abstract Background and Objective: the purpose of this study is to investigate the mediating role of self-efficacy as a cognitive factor in the relationship between pain sensation and long-term suffering. Methods: This is a descriptive correlational study. The statistical population of the study included all patients with chronic pain with Rheumatoid Arthritis referring to 11clinics and private and governmental centers specialized in chronic pain in Tehran and Karaj in a specified time interval (Between Nov2015 and May 2016). 399 patients(257 women and 142 men) were selected via accessible sampling. The sensory pain index of the McGill pain scale, the Pain Discomfort Scale (PDS) and the Pain Self-efficacy Questionnaire (PSEQ) were used to collect the data. Data were analyzed by structural equation modeling (SEM). Results: The results showed that pain sensation and Self-efficacy are considered as good predictors of long-term suffering. Based on the findings of the structural equation modeling, there are significant and a positive causal relationship between pain sensation, self-efficacy and long-term suffering and the mediating role of self-efficacy in the relationship between pain sensation and long-term suffering has been confirmed (P<0.05). Conclusion: The result of this research, In addition to conceiving long-term suffering, It can be helpful in identifying one of the cognitive-emotional factors of long-term suffering, providing more adaptive strategies for people with in prevention and treatment and health promotion.

    Abstract Background: Intraoperative hemorrhage is one of the problems during surgery and, if it happens in a high volume without an immediate action to control, it can be fatal. Nowadays, various injectable solutions are used. The aim of this study was to compare the acid–base and hemodynamic status of the patient using two solutions, Ringer lactate and 1.3% sodium bicarbonate, in half saline solution. Materials and Methods: This clinical trial was performed at the Al-Zahra Hospital in 2013 on 66 patients who were randomly selected and put in two studied groups at the onset of hemorrhage. For the first group, crystalloid Ringer lactate solution and for the second group, 1.3% sodium bicarbonate in half-normal saline solution was used. Electrocardiogram, heart rate, O2 saturation non-invasive blood pressure and end-tidal CO2 were monitored. The arterial blood gas, blood electrolytes, glucose and blood urea nitrogen were measured before serum and blood injection. After the infusion of solutions and before blood transfusions, another sample was sent for measurement of blood parameters. Data were analyzed using SPSS software. Results: The mean arterial pressure was significantly higher in the second group than in the first group at some times after the infusion of solutions. pHh levels, base excess, bicarbonate, sodium, strong ion differences and osmolarity were significantly greater and potassium and chloride were significantly lower in the second group than in the first group after the infusion of solutions. Conclusion: 1.3% sodium bicarbonate in half-normal saline solution can lead to a proper correction of hemodynamic instability. By maintaining hemodynamic status, osmolarity and electrolytes as well as better balance of acid–base, 1.3% sodium bicarbonate solution in half-normal saline solution can be more effective than Ringer lactate solution during intraoperative bleeding.
