    Abstract Idiopathic condylar resorption (ICR), though a rare event, is associated with severe detrimental sequelae for the patient. To date, the etiology remains unknown, and treatment strategies are highly controversial. Therefore, the aim of this study is to present an analysis of the consensus- and evidence-based approach to ICR by a German interdisciplinary guideline project of the AWMF (Association of the Scientific Medical Societies in Germany). Following a systematic literature search, including 56 (out of an initial 97) publications, with a predominantly low level of evidence (LoE), two independent working groups (oral and maxillofacial surgery and interdisciplinary, respectively) voted on a draft comprising 25 recommendations in a standardized anonymized and blinded Delphi procedure.     Abstract Introduction: Due to potentially severe sequelae (impaired growth, condylar resorption, and ankylosis) early diagnosis of chronic rheumatic arthritis of the temporomandibular joint (TMJ) and timely onset of therapy are essential. Aim: Owing to very limited evidence the aim of the study was to identify and discuss controversial topics in the guideline development to promote further focused research. Methods: Through a systematic literature search, 394 out of 3771 publications were included in a German interdisciplinary guideline draft. Two workgroups (1: oral and maxillofacial surgery, 2: interdisciplinary) voted on 77 recommendations/statements, in 2 independent anonymized and blinded consensus phases (Delphi process). Results: The voting results were relatively homogenous, except for a greater proportion of abstentions amongst the interdisciplinary group (p < 0.001). Eighty-four percent of recommendations/statements were approved in the first round, 89% with strong consensus. Fourteen recommendations/statements (18.2%) required a prolonged consensus phase and further discussion. Discussion: Contrast-enhanced MRI was confirmed as the method of choice for the diagnosis of TMJ arthritis. Intraarticular corticosteroid injection is to be limited to therapy-refractory cases and single injection only. In adults, alloplastic joint replacement is preferable to autologous replacement. In children/adolescents, autologous reconstruction may be performed lacking viable alternatives. Alloplastic options are currently still considered experimental.

    Title translation Revalidation of a questionnaire assessing patients' experiences in hospitals
    Abstract Es wird informiert über die erneute Validierung eines Erhebungsinstruments zur Erfassung von Erfahrungen von akutstationären Patienten während des Krankenhausaufenthaltes sowie über die Prüfung der Eignung des Fragebogens für den Einsatz im internen und externen Qualitätsmanagement von Kliniken. Es handelt sich dabei um die deutsche Übersetzung eines am Picker Institute Boston entwickelten Fragebogens. Daten wurden im Rahmen einer postalischen, postationären Befragung von 11.826 Patienten aus 31 Krankenhäusern erhoben. Die testtheoretische Überprüfung erfolgte über Faktoren-, Reliabilitäts- und Regressionsanalysen. Die Differenzierungsfähigkeit zwischen Subgruppen sowie zwischen verschiedenen Einrichtungen wurde untersucht. Die durchschnittliche Rücklaufquote lag bei 60 %. Es wurden 9 Faktoren ermittelt (Cronbach's alpha 0,6-0,9), die 57% der Gesamtvarianz erklären. Alter, Gesundheitszustand und Krankenhausgröße scheinen einen Einfluss auf die Weiterempfehlungsbereitschaft der Patienten zu haben. Dies gilt vorrangig für Aspekte, bei denen die Interaktion und hier insbesondere eine gelungene Kommunikation mit den betreuenden Ärzten und Pflegekräften im Vordergrund stehen. Schlussfolgerung: Psychometrische Kennwerte, Differenzierungsfähigkeit sowie Praktikabilität machen den Fragebogen zu einem für das interne und externe Qualitätsmanagement von Krankenhäusern geeigneten Instrument.

    Abstract Background: Active participation of medical students in patient care has been shown to be important for professional development of learners. Not much is known about the impact of active student participation (ASP) to the quality of patient care.
    Aims: We established a Clinical Education Ward (CEW) for the final year medical students caring for patients under structured clinical supervision. This study investigates the views of both patients and clinical staff on the impact of ASP on patient care.
    Methods: The Picker Inpatient Questionnaire (PIQ) was used to survey all the patients admitted to the CEW during the pilot phase. Results concerning the general quality of health care and the patient-physician relationship (PPR) were compared to two matched pair control groups: patients of the same department (CG1) and of internal wards in Germany (CG2). In addition, quantitative and qualitative data were collected from patients and clinical staff members to specify the impact of students on patient care.
    Results: Out of 111 patients, 64 responded. The PIQ results revealed very minor problems in the assessment of the overall general quality of care and in PPR at the CEW, while significant improvements existed when compared to CG2. Furthermore, 79% of the patients and 95% of the staff members recorded a positive impact of ASP. Qualitative data illustrated and complemented these results. Chances and challenges in programs with high participation of students in clinical care are discussed.
    Conclusion: ASP may not only be useful for learners but also offers chances and benefits for patient care.

    Abstract Although condylar dislocation is not uncommon, terminology, diagnostics, and treatment concepts vary considerably worldwide. This study aims to present a consensus recommendation based on systematically reviewed literature and approved by the European Society of TMJ Surgeons (ESTMJS). Based on the template of the evidence-based German guideline (register # 007-063) the ESTMJS members voted on 30 draft recommendations regarding terminology, diagnostics, and treatment initially via a blinded modified Delphi procedure. After unblinding, a discussion and voting followed, using a structured consensus process in 2019. An independent moderator documented and evaluated voting results and alterations from the original draft. Although the results of the preliminary voting were very heterogenous and differed significantly from the German S3 guideline (

    Abstract Hintergrund und Ziele: Für das Tertial Innere Medizin schreibt die in Deutschland geltende Ärztliche Approbationsordnung eine stationsgebundene Ausbildung von 16 Wochen vor. Die Ausbildung im Praktischen Jahr (PJ) sollte im Kontext des späteren beruflichen Umfeldes und kompetenzbasiert erfolgen. Die wesentlichen Ziele des hier beschriebenen Projektes liegen darin, erstens eine Ausbildungsstation für eine praxisnahe und selbstgesteuerte Ausbildung von PJ-Studierenden aufzubauen sowie zweitens unter Beteiligung der relevanten Personengruppen (Patienten, Mitarbeiter, Studierende) die Auswirkungen dieses Modells auf Patientenversorgung, Stationsorganisation und Lernprozess der Studierenden zu untersuchen. Dabei sollen die Voraussetzungen für eine „lernende Organisation“ geschaffen werden.
    Methoden: Im Herbst 2007 wurde im Rahmen eines Pilotprojektes eine Ausbildungsstation in einer internistischen Abteilung aufgebaut. Dabei übernehmen Studierende im PJ als „Ärzte unter Supervision“ die Betreuung der Patienten. Sie werden dabei fachärztlich angeleitet und supervidiert. Alle behandelten Patienten wurden nach Entlassung mittels eines Fragebogens zu einer Beurteilung der studentischen Integration in die Patientenversorgung aufgefordert. Ebenso wurden alle beteiligten Mitarbeiter (Ärzte, Pflegende, Therapeuten) bezüglich der Veränderungen in der Stationsorganisation und der interprofessionellen Zusammenarbeit befragt. Mit den Studierenden wurden vor dem Tertial Ausbildungsziele in definierten Kompetenzfeldern vereinbart. Die Studierenden schätzten sich am Beginn und am Ende des Tertials selbst zu diesen Kompetenzen ein. Das Projekt ist Teil des Begleitstudiums Anthroposophische Medizin an der Universität Witten/Herdecke, in dem u. a. selbstgesteuertes Lernen gefördert werden soll.
    Ergebnisse: Von 56 befragten Patienten beantworteten 34 den Fragebogen (Rücklaufquote 60,7%). Die Mehrzahl (71%) der Patienten beurteilten die Auswirkung der studentischen Einbindung auf die Patientenversorgung als positiv. Die Mitarbeiter (n=28, Rücklauf: 23 (82%)) befürworteten alle die Weiterentwicklung der Ausbildungsstation zu einer kontinuierlichen Einrichtung. Die PJ-Studierenden der ersten beiden Tertiale (n=9) haben nach Selbsteinschätzung in allen Kompetenzfeldern Fortschritte erzielt, am meisten in Organisations- und Entscheidungskompetenz.
    Schlussfolgerung: Die betroffenen Personengruppen (Patienten, Mitarbeiter, Studierende) beurteilen die der auf der Ausbildungsstation realisierte Verbindung von Patientenversorgung und Lehre weit überwiegend positiv. Weitere Studien werden durchgeführt um zu klären, ob die Ausbildungsstation sich langfristig bewährt und ob sie als Modell auch für andere klinische Abteilungen geeignet ist.

    Background and aims: The final year of the undergraduate medical curriculum in Germany consists of three 16-week rotations including one hospital-based rotation in internal medicine. The final-year program is supposed to be oriented toward real-world practice and competency-based learning. A Clinical Education Ward (CEW) was developed to promote contextual and self-directed learning among medical students during their final-year rotation in internal medicine. The goal of multisource evaluation is to analyze the implications of this model in terms of patient care, ward organization, and the learning progress of medical students. Prerequisites for a “learning organization” should be established.
    Methods: At the CEW, final-year medical students were acting as “doctors under supervision” and taking care of patients in a ward for internal medicine. Students were instructed and closely supervised by clinical tutors. All patients admitted to the CEW were surveyed using a questionnaire to assess the implications of student involvement in clinical care. Clinical staff members (physicians, nurses, therapists) were asked about changes in terms of ward organization and interprofessional teamwork.
    Students assessed themselves at the beginning and at the end of the rotation in terms of clinical competencies, which were developed in cooperation with the students in preparation for the CEW.
    The project is part of the Integrated Studies of Anthroposophic Medicine program at the University of Witten/Herdecke (Germany), which aims to foster self-directed learning.
    Results: Fifty-six patients on the CEW were asked to complete a survey; 34 (60.7%) responded. The majority (71%) saw positive implications of student involvement in clinical care. Staff members (n = 28, return: 23 or 82%) were in favor of the continued implementation of the CEW as a permanent institution. Medical students of the first two rotations (n = 9) self-assessed progress in all competency fields, with the highest progress having been made in organization and ward management.
    Conclusions: The main stakeholders (patients, staff members, students) see significant advantages in the specific combination of patient care and clinical education at the CEW. Further studies have to be performed to analyze the long-term effects of the CEW as well as its function as a model for other teaching hospitals.
