    Abstract Mit der Einführung der ambulanten spezialfachärztlichen Versorgung, kurz ASV, für urologische Tumoren im April 2018 wurde diese neue Versorgungsform auch für die Diagnostik und Therapie von Patienten mit urologischen Tumoren etabliert. Ziel von ASV ist es, die „Lücke“ zwischen ambulanter und stationärer Versorgung zu schließen, interdisziplinär und sektorenübergreifend! Dies gilt insbesondere für komplexe und fortgeschrittene urologische Tumoren, bei denen moderne diagnostische Verfahren und innovative Therapieansätze zur praktischen Umsetzung gelangen sollen. Die Bedeutung urologischer Karzinomerkrankungen in der Onkologie ist hoch, etwa 20 % aller erstdiagnostizierten Karzinomerkrankungen in Deutschland sind urologisch. Dazu kommt, dass die Therapie dieser Erkrankungen in fortgeschrittenen Stadien in den letzten Jahren hochinnovative Veränderungen erfahren hat und häufig im ambulanten Sektor durchgeführt wird. Aber wird ASV von den uro-onkologisch tätigen Kolleginnen und Kollegen akzeptiert? Bringt ASV wirklich Verbesserungen in der medizinischen Versorgung von Patienten? Wirkt ASV als konkurrierende Versorgungsform gegenüber interdisziplinären Tumorzentren und der Onkologievereinbarung? Die Nuklearmedizin ist ein spezifischer und wichtiger Bestandteil bei der Diagnostik und Therapie urologischer Tumoren. Welche Rolle kann die Nuklearmedizin unter ASV-Bedingungen spielen? Der aktuelle Status von ASV bei urologischen Tumoren, die Probleme wie auch die Chancen werden in diesem Beitrag unter besonderer Berücksichtigung der Nuklearmedizin analysiert.

    Title translation Acceptance and status of digitalization in clinics and practices : Current assessment in German urology.
    Abstract Background: Digitalization of patient documentation and introduction of the electronic patient record (ePA) pose challenges to everyday clinical practice.
    Objectives: We investigated the acceptance and status of the digitalization of patient data and the introduction of the ePA among German urologists.
    Materials and methods: A questionnaire with 30 questions about the acceptance and status of digitalization of patient documentation and ePA was sent out via the newsletter of the German Society of Urology.
    Results: A total of 80 urologists participated in the survey (response rate 2%). Digital platforms such as Urotube or Researchgate are used by 63% of participants. The complete implementation of digital patient documentation was reported by 72% of respondents working in medical practice and by 54% of those working in the hospital (p = 0.042). While 76% see the digitalization process as reasonable, 34% expressed partial or strong concerns about the complete digitalization of patient documentation. Only 14% of the participants offer video consultations. Advantages for ePA include better networking of the healthcare system (73%), improved diagnosis, indication (41%) and treatment quality (48%), and avoidance of medication errors (70%).
    Conclusion: German urologists are open to the digitalization process and ePA. Especially younger urologists are using digital media. The advantages of digitalization are, in particular, an improvement in treatment processes. For a smooth introduction, a cross-departmental establishment and, if necessary, an adaptation of the treatment processes are necessary.

    Abstract We examined need-related and reciprocal provision of support in couples facing radical prostatectomy and its sequelae, including patients' urinary incontinence. Partners' reciprocal support provision to patients was assumed to drop from prior to until after patients' surgeries and increase again in the following months, while need-related indicators were assumed to remain unique correlates throughout. In this study of German prostatectomy patients and their partners, N = 141 couples provided data on 4 measurement occasions from presurgery to 1-year postsurgery. Need-based predictors of partners' support provision were patients' mobilized support, such as efforts to obtain advice or comfort, and degree of postsurgery incontinence. Strength of association between partner-received and provided supports served as an indicator of reciprocal support provision. Data suggested that partners' reciprocal support provision dropped significantly postsurgery and then increased again in the following months. This was true for emotional as well as instrumental reciprocal support provision. Findings also indicated that one need-based predictor of partners' support provision, patients' mobilization of support from their partners, remained a unique correlate of partners' support provision to patients. Reciprocal support provision in couples may vary during the adaptation to illness-related functional impairment and coexist with need-oriented support provision.
