    Abstract A entrevista traz à discussão a multiplicidade dos fatores sociais e ambientais associados à obesidade na infância e adolescência, que dificilmente são investigados e que passam ao largo das ações educativas. No Brasil, este problema de saúde pública atinge um terço das crianças menores de 10 anos e se mostra preocupante na medida em que se apresenta de forma cada vez mais precoce. A entrevistada aponta para a necessidade de uma abordagem que tome a obesidade como um problema social e ambiental, que ultrapassa em muito as questões individuais. Trata-se de abordar de forma ampla os sistemas de produção e de consumo alimentar, que impactam a saúde pública e a infância, de modo a tornar mais efetivas as ações de prevenção e controle da obesidade.

    Abstract ObjectiveTo describe the evolution of beverage portion sizes consumed in Brazil between 2008 and 2018.MethodsData from the dietary surveys of 2008–2009 and 2017–2018 Brazilian Household Budget Surveys, conducted with 34,003 and 46,164 individuals, respectively, were used to analyze the portion size of beverages. Food consumption data were used to group beverages according to nutritional characteristics, type and size of portions into eight groups: high-calorie soft drinks, fruit refreshments, alcoholic beverages, coffee/tea, fruit juices, milk and milk substitutes and ultra-processed milk-based beverages. The two-day food record and recall were considered to analyze the consumed portions. Comparisons between the surveys were done using Chi-Square tests and linear regression models.ResultsBetween 2008 and 2018, the average portion consumed showed a significant increase for the group of alcoholic beverages (+29%), flavored juices (+11%), caloric soft drinks (+8%), milk and milk substitutes (+6%) and fruit juices (+5%); and reduction for the coffee/tea group (−11%). Analyzes by age group showed that among individuals between 20 and 40 years of age, the soft drinks and alcoholic beverage groups showed the greatest increase in portion size, +12 and +44%, respectively.ConclusionThe beverage portion sizes consumed in Brazil between 2008 and 2018 increased for the group of alcoholic beverages, flavored juices, caloric soft drinks, milk and milk substitutes, and fruit juices.

    Abstract Objetivo: Realizar adaptação transcultural e avaliar a reprodutibilidade de questionário sobre aspectos psicossociais associados ao consumo de frutas e hortaliças de adolescentes. Métodos: Trata-se de estudo teste-reteste, realizado em 2015, com 92 adolescentes estudantes de escola pública de Niterói, Rio de Janeiro (Brasil). Realizou-se a adaptação transcultural das questões relativas ao consumo de frutas e hortaliças do questionário costarriquenho “Aspectos Psicosociales Asociados a la Nutrición y Actividad Física de Adolescentes”, o qual possuía cinco dimensões, distribuídas em 46 questões, que avaliavam a influência da autoeficácia, família, pares, imagem corporal e mídia/meio sobre o consumo. Avaliou-se a reprodutibilidade por meio da comparação de dados obtidos em duas aplicações do instrumento. Utilizaram-se a estatística kappa e o coeficiente de correlação intraclasse (ICC). Resultados: As análises mostraram que para 60% das questões, o valor do kappa foi estatisticamente significativo (p<0,05) e variou entre 0,21 e 0,52, indicando concordância razoável ou moderada. Os aspectos psicossociais que apresentaram maior concordância foram a autoeficácia (ICC 0,75), a família (ICC 0,59), a imagem corporal (ICC 0,47) e os pares (ICC 0,30). Conclusão: O questionário apresenta boa reprodutibilidade e permite avaliar aspectos psicossociais específicos relacionados ao consumo de frutas e hortaliças em adolescentes. O questionário testado oferece subsídios importantes para o desenvolvimento e aplicação de programas e ações voltados para promoção da alimentação saudável entre adolescentes.

    Abstract OBJECTIVE:To identify food choices allowing the fulfillment of nutritionally adequate diets resembling actual food patterns at the lowest cost achievable for the Brazilian population, stratified by income level. METHODS:Food consumption and prices were obtained from the Household Budget Survey (n = 55,970 households) and National Dietary Survey (n = 32,749 individuals). The sample was stratified into capitals of the states and further by income levels according to the official minimum wage (totaling 108 geographic-economic strata, or GES). Linear programming models were performed for each GES in order to find the lowest cost of diets that meet a set of nutritional constraints. In order to find realistic diets, constraints referring to preferences were introduced in the models allowing optimized food quantities to depart progressively from the current intake for each food and food group. The impact of meeting each target nutrient was assessed by performing models removing each nutrient at the time. RESULTS:The observed and optimized diet costs were US$2.16 and US$2.58 per capita/day. The highest cost increment and the greatest food shifts were observed in the lowest income level. The nutrient adequacy was reached by mainly increasing fruits and vegetables, beans, fish and seafood, dairy, nuts, and eggs; and reducing red and processed meat, chicken, margarine and butter, cookies, cakes, sugar-sweetened beverages, and sauces. As the departure from the current intakes increase, the optimized healthy diet cost reduced. In the lowest income, the lowest cost increment was about US$ 0.10; in the higher income levels, it tended to be cheaper than the observed cost. Calcium was the most expensive nutrient to meet adequacy. CONCLUSION:Nutritionally adequate diets are possible but costlier than the observed.

    Abstract ABSTRACT: Objective: The aim of this study was to compare the proportion of deaths among hospitalized cases of COVID-19 in São Paulo and Rio de Janeiro, stratified by private and public services. Methods: Hospitalization data for severe acute respiratory syndrome (SARS) were obtained using the SIVEP-Gripe Database. All hospitalized adults who were diagnosed as COVID-19 or unspecified SARS, between January and December 2020, were included in the analysis. Logistic regression models were fitted to evaluate the risk of death between Rio de Janeiro and São Paulo, adjusted for age, sex, and comorbidities. Results: A total of 388,657 hospital registers for Rio de Janeiro and São Paulo (91,532 and 297,125, respectively) were analyzed. Missing data are frequent in the database and it was greater in Rio de Janeiro, at the state and capital levels. Adjusting for confounders, the odds ratio of dying by COVID-19, comparing the state of Rio de Janeiro with São Paulo, was 2.51 in the private hospitals and 2.29 in the public ones. For the capitals, the scenario is worse. The lethality among hospitalized patients with COVID-19 is at least twice in Rio de Janeiro than São Paulo, both at the states and capitals. The public or private services showed important differences, with odds ratios of 2.74 and 3.46, respectively. Conclusion: It appears that the worst governance in the health sector in Rio de Janeiro, more than lack of resources, explains the excess mortality of hospitalized COVID-19 patients in Rio de Janeiro.

    Abstract Objective This study evaluated the influence of family meal frequency on the occurrence of common mental disorders (CMD) in children and adolescents over eight months of follow-up. Design Data from this longitudinal study were derived from the PAAPAS-Parents, Students, Community Health Agents, and Teachers for Healthy Eating-community trial. CMD were evaluated using the General Health Questionnaire. Frequency of family meals (breakfast and dinner) was categorized as "breakfast and dinner regularly with the family", "at least breakfast or dinner regularly with the family", and "does not have any meal regularly with the family." The effect of family meal frequency on CMD was analyzed using generalized estimation equations with log-binomial models for repeated measures. Setting This study was conducted in public schools (N = 18) of Duque de Caxias, Rio de Janeiro, Brazil, in 2016. Participants Children (aged 9-11 years) and adolescents (aged 12-17 years) from the fifth and sixth grades (N = 2,743). Results These findings suggested that regular family meals were a protective factor for mental health. The adjusted relative risk of CMD was 0.75 (95% confidence interval = 0.69-0.83) for those who had two family meals regularly and 0.87 (95% confidence interval = 0.77-0.97) for those who had only one regular family meal, compared to students who had no regular family meals. Conclusions Potential strategies that educate and encourage families about the mental health benefits of eating regular meals together must be explored and implemented.

    Abstract Abstract Background Common mental disorders (CMD) in children and adolescents can be initial and non-specific manifestations of more serious mental diseases and often persist into adulthood. Therefore, early detection is important, as is the identification of the factors that impact development. Insufficient sleep represents one of the most common and potentially remediable risks to mental health in children and adolescents for whom chronic sleep loss has become normal. This study aims to investigate the influence of sleep duration on CMD in schoolchildren and adolescents. Methods The data for this longitudinal study were derived from the community trial Parents, Students, Community Health Agents and Teachers for Healthy Eating (PAAPAS) in 2016 (N = 2743), with fifth and sixth graders from 18 public schools of Duque de Caxias, Rio de Janeiro, Brazil. CMD were assessed by the General Health Questionnaire (GHQ-12) at baseline and at a 9-month follow-up. Sleep duration was evaluated at baseline and was categorized as short, adequate or long according to age group. The effect of sleep duration on common mental disorders was analysed by sex using linear mixed-effects models. Results At baseline, the frequency of CMD was 33.2% and was higher in boys than in girls and higher among students with short sleep duration. There was an increase in the CMD score over time among girls with short sleep duration (p < 0.01). Among boys, a similar trajectory of the CMD score was observed in the subgroups with short and long sleep duration, but there was a significant reduction in the subgroup with long sleep duration (p = 0.05). Conclusion Changes in common mental disorder patterns differ according to sex, and short sleep duration seems to be problematic for students’ mental health. The promotion of health strategies that involve the family and school environment, such as later school hours, could help change this scenario.

    Abstract ABSTRACT OBJECTIVE: To describe the evolution of food consumption by the Brazilian population in 2008–2009 to 2017–2018. METHODS: Data from the National Dietary Surveys of 2008–2009 and 2017–2018 were used. Both surveys estimated food consumption of two non-consecutive days of individuals aged 10 years or older. The first survey collected consumption data from 34,003 individuals through food records; the second, obtained data from 46,164 individuals, through 24-hour recalls. The twenty most frequently reported food groups in the two surveys were identified. The probability of consumption of each food group in the two surveys was estimated according to sex, age and income. This study presents the foods that had a change in the frequency of consumption of 5% or higher between the two surveys. The probability of consumption was corrected for intra-individual variability using the method developed by the National Cancer Institute. RESULTS: Rice, beans, coffee, bread, vegetables and beef remained the staple Brazilian diet, ranking as the six most consumed items in both surveys. Ultra-processed foods such as sweet/stuffed cookies, savory cookies, processed meats and carbonated drinks also remained among the 20 most consumed foods. Trend analyses showed, regardless of gender, age and income range, a decrease in the consumption of rice, beans, beef, bread, fruit, milk and dairy, processed meats and carbonated drinks, and an increase in the consumption of sandwiches. CONCLUSION: The Brazilian diet is still characterized by consumption of traditional foods, such as rice and beans, and by high frequency of consumption of ultra-processed foods, such as cookies and carbonated drinks. However, between the years of 2008–2009 and 20172018, there was a decrease in the consumption of rice, beans, beef, bread, fruit, milk and dairy, processed meats and carbonated drinks, but an increase in the consumption of sandwiches. The results show a decrease in quality in the Brazilian diet.
