    Abstract Purpose of review: Despite available treatments, many bladder pain syndrome/interstitial cystitis (BPS/IC) patients continue to have poor quality of life. Thus, there is an urge for new therapies. Our manuscript aims to review papers about BPS/IC treatments published in the last 2 years.
    Recent findings: During this period, several treatments were tested, most of them new and others combining treatments already used. Pentosan polysulfate, interleukin 1 antagonist, low energy shock wave, physical therapy, hypnosis, acupuncture, clorpactin, dimethyl sulfoxide and hyaluronic acid plus botulinum toxin-A showed positive results. ASP3652 and lidocaine-releasing intravesical systems failed to prove their efficacy.
    Summary: Validation of these studies is arduous due to the broad spectre of BPS/IC phenotypes, small number of patients enrolled, distinct outcome measures and short-term follow-up. It is also important to highlight that some authors combined therapies, and others split central and peripheric phenotypes before treatment. Therefore, soon, phenotyping and combining therapies with a step-by-step approach will be needed in BPS/IC treatment.

    Abstract Abstract Background and Aims The use of ultrasound screening is primarily facilitated by point‐of‐care ultrasound (POCUS) and its integration into healthcare systems is a result of the versatility of this imaging technique. This study intends to compare the accuracy and pertinence of sonographic findings obtained by a sonographer in a Basic Emergency Service (BES) with that of radiologists at referral hospital (RH) in Portugal. Methods Twenty patients with right upper quadrant (RUQ) pain and suspected cholecystitis or biliary pathology underwent sonography screening using POCUS in the BES. They were then forwarded to the RH where a radiologist performed a conventional ultrasound exam on the same patients. The results of both exams were compared to determine if the findings obtained in the BES were confirmed by the radiologist in the RH. Results In our sample, 60% of cases were related to biliary pathology, 20% were liver‐related, 10% had hepatopancreatic biliary etiology, and 10% had unknown etiology. A strong association between the sonographic findings in the BES and the RH was found in the variables “Sonographic Murphy sign” (V = 0.859; p = 0.001), “Cholelithiasis/Gallbladder sludge” (V = 0.840; p = 0.001), and “Intrahepatic biliary tract dilatation” (V = 0.717; p = 0.006). Adequate measures of agreement between the findings of the radiographer and radiologist were obtained for the “Sonographic Murphy sign” (k = 0.664; p = 0.001) and the presence of “Cholelithiasis/Gallbladder sludge” (k = 0.712; p = 0.000). Conclusion Major biliary abnormalities were detected in patients with RUQ pain in BES using sonography. The correlation between the sonographic findings obtained by the sonographers at BES and those obtained by radiologists at the RH in Portugal was strong, showing that POCUS screening could be extended to other similar settings; however, more studies are needed.

    Abstract Abstract The costs of sending remittances to Pacific small island developing states (SIDS) are among the highest in the world. Tackling this issue is crucial not only for economic and social development, but also for improving financial inclusion. This article analyses fintech adoption in remittance services, namely the adoption of alternative payment methods in transferring money by using the internet or mobile phones, in the Pacific. It introduces an original framework to assess the current landscape of fintech in the remittance sector and draws tailored policy recommendations. The framework is conceptualised through a ladder with five rungs: availability, accessibility, awareness, literacy and trust. Based on the ladder analysis, the authors observe the lack of basic digital infrastructure and digital platforms in many Pacific SIDS. Where the technological landscape is better developed, fintech services have established strong footholds, but there is a need for greater awareness to broaden its appeal and customer base. The benefits of fintech platforms are high, especially in the context of lower remittance costs which constitute an unduly large share of GDP in Pacific SIDS. The basic infrastructure needed to develop fintech services are equally important for the overall sustainable development of Pacific SIDS. The article observes fintech services in the Pacific are a means for financial inclusion of the unbanked, that can accelerate the economic and social development of the SIDS, and countries in the Pacific region are at different stages in their readiness for fintech adoption.

    Abstract Introduction: Mucormycosis, a rare and life-threatening infection, is caused by microorganisms of the Mucorales order. It affects almost exclusively immunocompromised and diabetic patients, requiring extensive surgical debridement and prolonged antifungal therapy. Discussion/Results: We report the case of a 26-year-old immunocompetent woman, presenting with cutaneous mucormycosis after suffering blunt force trauma. This rare occurrence of mucormycosis in an immunocompetent patient reinforces the importance of elevated clinical suspicion and early initiation of adequate surgical and antifungal treatment. Conclusion: Mucormycosis is a challenging condition with potentially devastating consequences. Timely diagnosis and appropriate management are vital to mitigate the morbidity and mortality associated with this condition.

    Abstract Introduction: Cystic adventitial disease is a rare cause of intermittent claudication. Its presentation as acute limb ischemia is exceedingly rare. This disease is caused by the growth of multiple cysts within the adventitia. Although its etiopathogeny isn’t fully understood most evidence supports an embryonic defect during the development of non-axial arteries. Treatment includes several options: adventitial layer excision, percutaneous aspiration of the cysts and segmental arterectomy with bypass. Methods: We present a case of acute limb ischemia caused by popliteal cystic adventitial disease. Clinical Case: A 60 years old male presented with acute limb ischemia of the right leg. Duplex ultrasound showed a tight stenosis of the popliteal artery caused by adventitial cysts. The patient was intervened: from a posterior approach an excision of the cysts and adventitial layer was performed with total recovery of foot’s perfusion and pedal pulse. After 18 months of follow-up the patient is asymptomatic without imagiological evidence of recurrence. Conclusions: Due to the disease’s rarity there are no studies designed to compare the different treatment strategies for this pathology. However, the available data from case series shows that the surgical technique chosen for this patient presents very good results and, therefore, it seems to be a good therapeutic option assuming a patent popliteal artery without intimal lesion. Clinical presentation as acute limb ischemia isn’t described in the literature and isn’t today understandable because the factors affecting mucin secretion rate by cyst’s mesenchymal cells are unknown.

    Abstract Abstract Robot polishing is increasingly being used in the production of high-end glass workpieces such as astronomy mirrors, lithography lenses, laser gyroscopes or high-precision coordinate measuring machines. The quality of optical components such as lenses or mirrors can be described by shape errors and surface roughness. Whilst the trend towards sub nanometre level surfaces finishes and features progresses, matching both form and finish coherently in complex parts remains a major challenge. With increasing optic sizes, the stability of the polishing process becomes more and more important. If not empirically known, the optical surface must be measured after each polishing step. One approach is to mount sensors on the polishing head in order to measure process-relevant quantities. On the basis of these data, machine learning algorithms can be applied for surface value prediction. Due to the modification of the polishing head by the installation of sensors and the resulting process influences, the first machine learning model could only make removal predictions with insufficient accuracy. The aim of this work is to show a polishing head optimised for the sensors, which is coupled with a machine learning model in order to predict the material removal and failure of the polishing head during robot polishing. The artificial neural network is developed in the Python programming language using the Keras deep learning library. It starts with a simple network architecture and common training parameters. The model will then be optimised step-by-step using different methods and optimised in different steps. The data collected by a design of experiments with the sensor-integrated glass polishing head are used to train the machine learning model and to validate the results. The neural network achieves a prediction accuracy of the material removal of 99.22%.     Abstract Background: Bladder pain syndrome/interstitial cystitis (BPS/IC) is a chronic pain condition, often underdiagnosed, with an important impact on patient quality of life. More recently, an association between VEGF and its receptors has been suggested in BPS/IC pathophysiology, due to their role in promoting angiogenesis and inflammation, which can enhance bladder pain. Eventually, VEGF may be used as a biomarker for the diagnosis and prognostication of BPS/IC. To further clarify this issue, this review aims to critically summarize the available information, giving rise to a solid starting point for future studies. Methods: We systematically searched PubMed and Embase, using the queries “urinary VEGF”, “urinary VEGF” AND “pain”, “urinary VEGF” AND “lower urinary tract symptoms” and “urinary VEGF” AND “LUTS” from January 2016 to February 2022. Results: A total of 1026 papers were identified from which 7 articles were included in this study, which assessed 1036 participants. Regarding VEGF levels, overactive bladder (OAB) and healthy patients were used for comparison with BPS/IC patients. VEGF concentration seems to be higher when compared to healthy patients and overactive bladder (OAB) patients. Higher levels of VEGF were associated with pain severity, while a decrease in VEGF concentration was associated with pain and symptom improvement in women. However, these findings were not constant in all studies. Conclusions: There is a trend toward a relevant association between increased VEGF levels and pain or symptom severity in BPS/IC patients. Although there are some discrepancies among the studies and the number of patients included is small, VEGF and its receptors should be considered for future studies regarding its use in BPS/IC pathophysiology, diagnosis and prognostication.
