    Abstract Abstract Subvolcanic hydrothermal systems can lead to hydrothermal eruptions as well as unrest phenomena without an eruptive event. Historical eruptions and recent unrest events, including ground inflation, demagnetization, and a gradual decrease in the plume height, at Mt. Tokachidake, central Hokkaido, Japan, are related to such a subvolcanic hydrothermal system. This study investigates the three-dimensional (3-D) resistivity structure of Mt. Tokachidake to image its subvolcanic hydrothermal system. A 3-D inversion of the magnetotelluric data, acquired at 22 sites around the crater area, was performed while accounting for the topography. Our resistivity model was characterized by a high-resistivity layer at a shallow depth (50–100 m) and two conductors near the active crater and dormant crater. The high-resistivity layer was interpreted to be composed of dense lava, which acts as a caprock surrounding the conductor. The high conductivity beneath the active crater can be explained by the presence of hydrothermal fluid in fractured or leached zones within the low-permeability lava layer, as the sources of ground inflation and demagnetization were identified within the conductive zone immediately beneath the resistive layer. The resistivity structure was used to estimate the volume of hydrothermal fluid within the pore space. The minimum volume of hydrothermal fluid beneath the active crater that can explain the resistivity structure was estimated to be 3 × 106 m3. This estimate is comparable to the water volume that was associated with the long runout and highly fluidized lahar in 1926. The resistivity structure and volume of hydrothermal fluid presented in this study can be used as a reference for further numerical simulations, which aim to reveal the mechanisms of recent unrest events and assess the risk of hazards, such as lahar.

    Abstract Photoplethysmographic (PPG) pulses contain information about cardiovascular parameters. In particular, blood pressure can be estimated using PPG pulse decomposition analysis, which assumes that a PPG pulse is composed of the original heart ejection blood wave and its reflections in arterial branchings. Among pulse decomposition wave functions that have been studied in the literature, Gaussian waves are the most successful ones. However, a more adequate pulse decomposition function could be found to improve blood pressure estimates. In this paper, we propose pulse decomposition analysis using hyperbolic secant (sech) waves and compare results with corresponding Gaussian wave decomposition. We analyze how the parameters of each of the two types of decomposition waves correlate with blood pressure. For this analysis, continuous blood pressure data and PPG data were acquired from ten healthy volunteers. The blood pressure of volunteers was varied by asking them to hold their breath for up to 60 s. The results suggested sech wave decomposition had higher accuracy in estimating blood pressure than the Gaussian function. Thus, sech wave decomposition should be considered as a more robust alternative to Gaussian wave pulse decomposition for blood pressure estimation models.

    Abstract Asthma is a chronic disease, characterized by airway inflammation, airflow limitation, hyper-reactivity and airway remodeling. It is believed that asthma is caused by the interaction between genetic and environmental factors. The prevalence of allergic diseases, including asthma, has increased worldwide during the past two decades. Although the precise reasons that have caused this increase remain unknown, dietary change is thought to be one of the environmental factors. Flavonoids, which are polyphenolic plant secondary metabolites ubiquitously present in vegetables, fruits and beverages, possess antioxidant and anti-allergic traits, as well as immune-modulating activities. Flavonoids are powerful antioxidants and anti-allergic nutrients that inhibit the release of chemical mediators, synthesis of Th2 type cytokines, such as interleukin (IL)-4 and IL-13, and CD40 ligand expression by high-affinity immunoglobulin E (IgE) receptor-expressing cells, such as mast cells and basophils. They also inhibit IL-4-induced signal transduction and affect the differentiation of naïve CD4+ T cells into effector T-cells through their inhibitory effect on the activation of the aryl hydrocarbon receptor. Various studies of flavonoids in asthmatic animal models have demonstrated their beneficial effects. The results of several epidemiological studies suggest that an increase in flavonoid intake is beneficial for asthma. Moreover, clinical trials of flavonoids have shown their ameliorative effects on symptoms related to asthma. However, these human studies are currently limited; further validation is required to clarify whether an appropriate intake of flavonoids may constitute dietary treatment and for part of a preventive strategy for asthma.

    Abstract In Japan, the nationwide earthquake early warning (EEW) system has been being operated by the Japan Meteorological Agency (JMA) since 2007, disseminating information on imminent strong ground motion to the general public and advanced technical users. In the beginning of the operation, the system ran based mainly on standard source-based algorithms with a point-source location estimate and ground motion prediction equation. The point-source algorithms successfully provided ground motion predictions with high accuracy during the initial operation; however, the 2011 Mw9.0 Tohoku-Oki earthquake and the subsequent intense aftershock and triggered earthquake activities underscored the weaknesses of the source-based approach. In this paper, we summarize major system developments after the Tohoku-Oki event to overcome the limits of the standard point-source algorithms and to enhance the EEW performance further. In addition, we evaluate how the system performance was influenced by the updates. One of significant improvements in the JMA EEW system was the implementation of two new ground motion prediction methods: the integrated particle filter (IPF) and propagation of local undamped motion (PLUM) algorithms. IPF is a robust point-source algorithm based on the Bayesian inference, and PLUM is a wavefield-based algorithm that predicts ground motions directly from observed shakings. Another notable update was the incorporation of new observation facilities including S-net, a large-scale ocean bottom seismometer network deployed along the Japan and Kuril trenches. The prediction accuracy and warning issuance performance analysis for the updated JMA EEW system showed that IPF improved the source-based ground motion prediction accuracy and reduced the risk of issuing overpredicted warnings. PLUM made the system less likely to underpredict strong ground motions and improved the warning issuance timeliness. The detection time analysis for the S-net incorporation suggested that S-net enabled the system to issue the first EEW report earlier than before the S-net incorporation for earthquakes around the Japan and Kuril trenches. Those findings indicate that the JMA EEW system has made substantial progress both on software and hardware aspects over the 10 years after the Tohoku-Oki earthquake.

    Abstract Introduction: Contamination of human cell-processed therapeutic products (hCTPs) with tumorigenic/immortalized cellular impurities is a major concern in the manufacturing and quality control of hCTPs. The cellular immortality test based on cell growth analysis is a method for detecting tumorigenic/immortalized cellular impurities in hCTPs. However, the performance of the cellular immortality test has not yet been well characterized. In this study, we examined the reproducibility of the cellular immortality test in detecting HeLa cells as a model of tumorigenic cellular impurities, as well as the applicability of other models of cellular impurities with different tumorigenicity to the cellular immortality test. Methods: Using HeLa cells as a model for cellular impurities, we measured the growth rate of human mesenchymal stem cells (hMSCs) supplemented with HeLa cells at concentrations ranging from 0.01 to 0.0001% at each passage in three laboratories and evaluated the reproducibility of the detection of immortalized cellular impurities. In addition, HEK293 cells (another immortalized cell line) and MRC-5 cells (a non-immortalized cell line) were employed as cellular impurity models that exhibit different growth characteristics from HeLa cells, and the ability of the cellular immortality test to detect these different impurities when mixed with hMSCs was examined. Results: In the multisite study, the growth rate of hMSCs supplemented with 1 and 10 HeLa cells (0.0001% and 0.001%) significantly increased and reached a plateau in all three laboratories, whereas those of hMSCs alone eventually decreased. Moreover, when hMSCs were supplemented with 10 and 100 HEK293 and MRC-5 cells (0.001% and 0.01%), the growth rate significantly increased. The growth rate of hMSCs supplemented with HEK293 cells increased with passage and remained high, whereas that of hMSCs supplemented with MRC-5 cells eventually decreased, as in the case of hMSCs alone.     Abstract OBJECTIVE:Cardiovascular disease is an important contributor to the mortality rate of patients with systemic lupus erythematosus (SLE), which is related to SLE disease activity. Fragmented QRS (fQRS) complexes, defined by additional spikes in the QRS complex, are useful for identifying myocardial scars on electrocardiography and can be an independent predictor of cardiac events. We aimed to assess the relationship between disease activity in patients with SLE and fQRS at the time of diagnosis. METHODS:Forty-four patients with SLE were included. Patients with cardiac diseases, other rheumatic diseases, and prior treatment at the time of electrocardiography measurement were excluded. The appearance of fQRS represented exposure. The primary outcome was SLE Disease Activity Index 2000 (SLEDAI-2K). Multiple regression analysis was conducted to assess the association between fQRS and SLEDAI-2K adjusted for age, sex, and time from the estimated onset date to the date of diagnosis. RESULTS:Among patients with SLE at diagnosis, 26 (59.1%) had fQRS. The median SLEDAI-2K was 18 (interquartile range [IQR], 12-22) and 9 (IQR, 8-15) in the fQRS(+) and fQRS(-) groups, respectively. SLEDAI-2K was significantly higher in the fQRS(+) group than in the fQRS(-) group (regression coefficient, 2.69; 95% confidence interval, 0.76-4.61; p = 0.008). CONCLUSION:Our results suggested that fQRS(+) patients with SLE had high disease activity. fQRS could likely detect subclinical myocardial involvement in patients with SLE and predict long-term occurrence of cardiac events.

    Abstract Abstract Six wells were continuously monitored in Hokkaido, northern Japan, to relate groundwater level changes to regional seismic activity. Groundwater level changes following the 2018 Hokkaido Eastern Iburi earthquake were detected in three of the six wells, even though they are located hundreds of kilometers from the epicenter. The groundwater level changes are qualitatively consistent with the volumetric strain induced by the earthquake. We analyzed groundwater level responses to the M2 tidal constituent before and after the earthquake, but related changes in amplitude and phase shifts remained within the usual variation. Observed coseismic change was explained by the response to the M2 tidal constituent component and the calculated volumetric strain for one of the wells, where groundwater level decreased. The observed change in the other two was found to be much greater than the corresponding estimates of the volumetric strain.

    Abstract Abdominal spacers are useful for maintaining the distance between the target tumors and surrounding tissues, such as the gastrointestinal tract, in patients treated with carbon ion radiotherapy. Surgical intervention to remove the spacers is sometimes necessary because of abdominal infections triggered by long-term spacer placement or intestinal perforation. Therefore, spacers that do not require surgical removal and provide effective drainage against abdominal infections are urgently needed. This study aimed to develop a spacer that could be removed non-surgically and one that provides the therapeutic effect of drainage in patients who receive carbon ion radiotherapy for abdominal tumors. A novel fan-shaped spacer was constructed from a film drain that was folded along the trigger line. Simple withdrawal of the trigger line caused the film drain to fold and the holding lines to become free. We performed laparoscopy-assisted insertion with pneumoperitoneum and blind removal of the spacer fourteen times using a porcine model. Saline in the abdominal cavity was effectively aspirated using the spacer. Our novel fan-shaped spacer could be removed safely without surgery and was able to drain fluid effectively from the abdominal cavity.

    Abstract Abstract Background Postoperative paralytic ileus can be a difficult complication for both surgeons and patients. Causes and treatments have been discussed for more than two centuries, but have not yet been fully resolved. Daikenchuto (TJ-100, DKT) is a traditional Japanese herbal medicine. Recently, some beneficial mechanisms of DKT to relieve paralytic ileus have been reported. DKT can suppress inflammation, increase intestinal blood flow, and accelerate bowel movements. Therefore, we have designed a randomized controlled trial to investigate the effects of DKT on postoperative gastrointestinal symptoms following laparoscopic colectomy in patients with left-sided colon cancer at a single institution. Methods/design As primary endpoints, the following outcomes will be evaluated: (i) grade of abdominal pain determined using the numeric rating scale (NRS), (ii) grade of abdominal distention determined using the NRS, and (iii) quality of life determined using the Gastrointestinal Quality Life Index (GIQLI). As secondary endpoints, the following will be evaluated: (i) postoperative nutritional status (Onodera’s Prognostic Nutritional Index (PNI) and the Controlling Nutritional Status score (CONUT score)), (ii) duration to initial flatus, (iii) duration to initial defecation, (iv) bowel gas volume, (v) character of stool (Bristol Stool Form Scale), (vi) defecation frequency per day, (vii) postoperative complications (Clavien-Dindo classification), (viii) length of postoperative hospital stay, and (ix) metabolites in the stool and blood. This trial is an open-label study, and needs to include 40 patients (20 patients per group) and is expected to span 2 years. Discussion To our knowledge, this is the first randomized controlled trial to investigate the effects of DKT on postoperative subjective outcomes (i.e., postoperative quality of life) following laparoscopic colectomy as primary endpoints.     Abstract Abstract Tsunami deposits were collected along the coast of southwestern Hokkaido and Okushiri Island, northern Japan. The distribution of these deposits suggested that large earthquakes and tsunamis have repeatedly occurred off southwestern Hokkaido. Along the southern coast of Okushiri Island, five tsunami sand/gravel layers have been deposited during the last 3000 years. The latest was deposited by the 1741 Oshima–Oshima landslide tsunami and the second by the 12th century tsunami. The later tsunami was probably generated by a large earthquake because submarine seismo-turbidites with similar age exist in the region and a large inland landslide had occurred in Okushiri Island in approximately the 12th century. The ages of paleo-tsunami events prior to the 12th century are 1.5–1.6, 2.4–2.6, 2.8–3.1 ka. In this study, a fault model of the 12th century earthquake was estimated by comparing tsunami deposit distributions and calculated tsunami inundation areas at five sites in Okushiri Island and Hiyama region. Fault model F17, a submarine active fault in the Japan Sea near Oshima–Oshima, is a probable source for this tsunami. Numerical simulation of the tsunami was performed based on fault model F17; we modified the fault parameters (length and slip amount) from the original model to explain tsunami deposit distributions. A shorter length of 104 km and a larger slip amount of 18 m were appropriate for the fault model on the basis of parametric studies. The seismic moment of the earthquake was calculated to be 9.95 × 1020 Nm (Mw 7.9) assuming a rigidity of 3.43 × 1010 N/m2. The estimated fault model is located between the focal regions of the 1993 Hokkaido Nansei-oki earthquake and the 1983 Japan Sea earthquake.
