    Abstract Introduction: The goal of palliative care is to preserve the quality of life or patient "comfort" in patients with serious diseases. Palliative care providers serve a wide range of patients: from those who seek curative treatment to those who are actively dying. Given this range, palliative care must mirror the dynamic goals of the patient at different stages of life and treatment. Throughout these stages, a goal of the palliative care provider would be to avoid hastening death; however, this often leads to clinical decisions that directly pit the patient's comfort against the patient's life span. This is most salient with clinical decisions of withdrawing treatments that prolong life even at the expense of comfort. An example of this dichotomy can be seen when providers use enteral nutrition to treat head and neck cancer patients.
    Case presentation: We describe a patient with stage IV pancreatic cancer with metastases to her head and neck. The patient was experiencing increased morbidity related to her percutaneous endoscopic gastrostomy (PEG) tube feeding. These complications included tube-related concerns such as infection, leakage, and diarrhea but also decreased intended benefits as she lost weight and functionality while maintaining enteral feeding. Despite the patient experiencing a common and expected disease course, she remained unsure and was fearful about considering discontinuation of her enteral feeding. However, the care team who understood the risks, benefits, and harms related to withdrawal provided a foundation of discussion and mitigated patient fears, allowing for the successful removal of her PEG tube and increased quality of life at the end of life.
    Conclusion: To care for a patient in their entirety is also to care for them at all stages of disease. Care is not limited to those who might be cured of disease, but should also consider those who continue to live with disease and the downstream effects of medical interventions used to support them. Discontinuing treatments whose harms outweigh the benefits to patients is a moral imperative to providers; yet, how providers approach discontinuing life-prolonging treatment is seen as morally distressing. Our patient did not see the discussion as morally distressing and continued to benefit from active discussions even at the end of her life.

    Abstract Cytosine DNA methylation is a highly conserved epigenetic mark in eukaryotes. Although the role of DNA methylation at gene promoters and repetitive elements has been extensively studied, the function of DNA methylation in other genomic contexts remains less clear. In the nucleus of mammalian cells, the genome is spatially organized at different levels, and strongly influences myriad genomic processes. There are a number of factors that regulate the three-dimensional (3D) organization of the genome, with the CTCF insulator protein being among the most well-characterized. Pertinently, CTCF binding has been reported as being DNA methylation-sensitive in certain contexts, perhaps most notably in the process of genomic imprinting. Therefore, it stands to reason that DNA methylation may play a broader role in the regulation of chromatin architecture. Here we summarize the current understanding that is relevant to both the mammalian DNA methylation and chromatin architecture fields and attempt to assess the extent to which DNA methylation impacts the folding of the genome. The focus is in early embryonic development and cellular transitions when the epigenome is in flux, but we also describe insights from pathological contexts, such as cancer, in which the epigenome and 3D genome organization are misregulated.

    Abstract Starvation of Schizosaccharomyces pombe for inorganic phosphate elicits adaptive transcriptome changes in which mRNAs driving ribosome biogenesis, tRNA biogenesis, and translation are globally downregulated, while those for autophagy and phosphate mobilization are upregulated. Here, we interrogated three components of the starvation response: upregulated autophagy; the role of transcription factor Pho7 (an activator of the PHO regulon); and upregulated expression of ecl3, one of three paralogous genes (ecl1, ecl2, and ecl3) collectively implicated in cell survival during other nutrient stresses. Ablation of autophagy factor Atg1 resulted in early demise of phosphate-starved fission yeast, as did ablation of Pho7. Transcriptome profiling of phosphate-starved pho7Δ cells highlighted Pho7 as an activator of genes involved in phosphate acquisition and mobilization, not limited to the original three-gene PHO regulon, and additional starvation-induced genes (including ecl3) not connected to phosphate dynamics. Pho7-dependent gene induction during phosphate starvation tracked with the presence of Pho7 DNA-binding elements in the gene promoter regions. Fewer ribosome protein genes were downregulated in phosphate-starved pho7Δ cells versus WT, which might contribute to their shortened lifespan. An ecl3Δ mutant elicited no gene expression changes in phosphate-replete cells and had no impact on survival during phosphate starvation. By contrast, pan-ecl deletion (ecl123Δ) curtailed lifespan during chronic phosphate starvation. Phosphate-starved ecl123Δ cells experienced a more widespread downregulation of mRNAs encoding aminoacyl tRNA synthetases vis-à-vis WT or pho7Δ cells. Collectively, these results enhance our understanding of fission yeast phosphate homeostasis and survival during nutrient deprivation.

    Abstract Gastric metastases from primary renal cell carcinoma (RCC) are rare and poorly documented in the existing literature. This case report presents the clinical course of a 65-year-old male with multi-metastatic clear cell RCC (ccRCC) who was incidentally found to have stomach metastases during follow-up magnetic resonance imaging (MRI). Gastric metastases from ccRCC are typically associated with other metastatic sites. They often emerge at an advanced stage of the disease, indicating a poor prognosis. It is therefore important to consider gastric metastases as a potential site of involvement in RCC patients. MRI revealed three gastric mucosal lesions exhibiting hypervascularity, a characteristic feature of ccRCC. Histological analysis confirmed the presence of malignant cells compatible with RCC.

    Abstract Introduction: Educational inequality in multiple health behaviors is rarely monitored using data from the same individuals as they age. The aim of this study is to research changes in relative educational inequality in multiple variables related to health behavior (smoking, physical activity, alcohol intake, and body mass index), separately and collectively (healthy lifestyle), among middle-aged adults living in Northern Norway.
    Methods: Data from adult respondents aged 32-87 in 2008 with repeated measurements in 2016 (
    Results: Educational inequality was observed in all the variables related to health behavior at baseline and follow-up, in both men and women. Higher levels of educational attainment were associated with healthier categories (non-daily smoking, physical activity, normal body mass index, and a healthy lifestyle), but also with high alcohol intake. The prevalence of daily smoking and physical inactivity decreased during the surveyed period, while high alcohol intake, having a body mass index outside of the normal range and adhering to multiple health recommendations simultaneously increased. The magnitude of relative educational inequality measured at baseline increased at the follow-up in all the variables related to health behavior. Differences were larger among women when compared to men, except in physical inactivity.
    Conclusion: Persistent and increasing relative disparities in health behavior between the highest education level and lower education levels are found in countries with well-established and comprehensive welfare systems like Norway. Addressing these inequalities is essential for reducing both the chronic disease burden and educational disparities in health.

    Abstract Low levels of phosphate in blood secondary to a lack of phosphorous reabsorption in the renal tubule cause hypophosphataemic rickets. To increase blood calcium levels, bone calcium is released into the bloodstream by increasing the activity of osteoclasts in the bones. These alterations in bone metabolism lead to delayed growth and small height, diffuse bone pain, bone fragility with microfractures, and frequent fractures and bone deformities. Although management of some of these conditions is mainly conservative, in cases of bone deformities or severe osteomalacia surgery should be considered: from minimally invasive surgical techniques of guided growth in patients with less deformity and open physeal cartilage to correction osteotomies in three-dimensional deformities. This article briefly reviews the indications for surgery procedures to be performed in these patients, highlighting when to use aggressive and non-aggressive approaches.

    Abstract Rheumatoid arthritis (RA) is a chronic joint autoimmune disease with a higher prevalence of depressive symptoms due to the burden of the disease. There are several patient-self-depression scales used for the assessment and a wide range of variations in the prevalence of depression could be related to this. No evidence was found that reports the most accurate, sensitive, and specific depression instrument through an extensive review in the literature. To determine the most precise depression instrument to evaluate RA patients. A systematic review search was directed, considering type of study, prevalence of depressive symptoms, usage of validated depression scales, and reported scale performance measurements. The data extraction followed the PRISMA guidelines, and the risk of bias was assessed using RoB 2, ROBINS-I, and QUADAS-2. From a total of 1958, only 28 articles included in the analysis. The total number of patients analyzed was 6405, with a mean age of 56.53 years, 4474 women (75.22%), and a mean prevalence of depressive symptoms of 27.4%. The most frequent and the best scale used was the CES-D (n = 12), considering all characteristics. The CES-D reported the best psychometric properties, and it was the most frequently used tool.

    Abstract El continente africano cuenta con 54 países, territorios dependientes y distintos territorios integrados en Estados no africanos. Su población es mayor a los 1.000 millones de personas y en ese contexto, existe consenso acerca de la necesidad de mejora de la estructura de salud en el continente, que ha enfrentado a lo largo de los años, distintos desafíos (VIH-SIDA, enfermedades respiratorias de vías bajas, diarreas, malaria, tuberculosis, meningitis, sarampión, dengue, hepatitis B, etc.). Por otro lado, en diciembre de 2019, se inició en China, un brote de coronavirus (COVID-19), que sería declarado como pandemia, en marzo de 2020, cuyo alcance se extiende a más de 180 países, superando los 370.000 casos en todo el mundo, que ha llegado también al continente africano, reportándose casos en Angola, Argelia, Benín, Burkina Faso, Cabo Verde, Camerún, Congo, Costa de Marfil, Chad, Eritrea, el Reino de Eswatini, Egipto, Etiopía, Gabón, Gambia, Ghana, Guinea, Guinea Ecuatorial, Kenia, república Democrática de Congo, Madagascar, Mauricio, Mauritania, Mozanbique, Namibia, Níger, Nigeria, República Centroafricana, Tanzania, Ruanda, Senegal, Seychelles, Sudáfrica, Sudán, Togo, Túnez, Uganda, Zambia y Zimbabwe. Atendiendo a lo señalado, este trabajo revisa de manera somera la situación del continente africano en lo que hace a la pandemia y las dinámicas políticas generales frente a la situación y en el ámbito internacional, a través de la consulta de documentos, textos académicos y noticias periódicas.
