    Abstract Introduction: Total duct excision (TDE) is performed for the diagnosis and management of nipple discharge. The Association of Breast Surgery's recent guidelines recommend considering diagnostic surgery for single-duct, blood-stained or clear nipple discharge, and for symptomatic management.
    Methods: We retrospectively reviewed the diagnostic and surgical outcomes of all cases of TDE between January 2013 and November 2019.
    Results: In total, 259 TDEs were carried out: 219 for nipple discharge, 29 for recurrent mastitis, 3 for screening abnormalities and 8 for breast lumps. Of the nipple discharge group, 121 had blood-stained discharge. Mean patient age was 52 years (range 19-81). Median follow-up time was 45 months (interquartile range 24-63). The following cases were identified on histopathology: 236 benign breast changes, 10 atypical ductal hyperplasia, 4 lobular carcinoma in situ, 2 low-grade ductal carcinoma in situ (DCIS), 3 intermediate-grade DCIS, 2 high-grade DCIS and 2 invasive ductal carcinomas. In total, 3.5% of patients who underwent TDE had a diagnosis of DCIS or invasive carcinoma. Blood-stained discharge was associated with a significant increase in risk of DCIS or carcinoma compared with other nipple discharge colours (
    Conclusions: TDE can be considered for the diagnostics and management of nipple discharge. Blood-stained nipple discharge increases the risk of DCIS or malignancy, but the majority of the time TDE reveals benign breast pathology.

    Abstract Background: Investigating survival in endometrial cancer (EC) is crucial to determine the effectiveness of overall management as it will reflect on the level of care provided among this population.
    Objective: The study was conducted to analyze the overall survival (OS) and progression-free survival (PFS) in treated endometrial carcinoma and to determine the associated predictors.
    Design: Retrospective SETTING: Department of obstetrics and gynecology in university tertiary hospital PATIENTS AND METHODS: Baseline demographic and clinical data, tumor characteristics and perioperative and outcome data were collected from consecutive patients treated for EC between 2000 and 2018. Kaplan-Meier method and multivariate Cox regression were used to analyze factors and predictors of OS and PFS.
    Main outcome measures: OS, PFS and prognostic factors SAMPLE SIZE: 200 RESULT: Endometrioid type was the most common type accounting for 78.5% of the cases, followed by papillary serous carcinoma (18.5%). At diagnosis, 21.5% were stage III, and 12.0% were stage IV. Invasiveness features showed involvement of the myometrium (96.5%), lymph vessels (36.5%), cervix stroma (18.5%), lower segment (22.0%), and parametrium (7.0%). The majority of patients had open surgery (80.0%), while 11.5% and 7.0% had laparoscopy and robotic surgery, respectively. Staging and debulking were performed in 89.0% of patients, and 12.5% of patients had residual disease of more than 2 cm. The mean OS and PFS were 104.4 (95% CI=91.8-117.0) months and 96.8 (95% CI=83.9-109.7) months, respectively. The 5-year OS and PFS were 62.5% and 46.9%, respectively. The majority of the factors we assessed were significantly associated with OS or PFS. However, reduced OS was independently associated age ≥60 years (hazard ratio [HR]=1.99,
    Conclusion: There is a need for a national strategy to tackle the growing burden of EC, by identifying the locally-specific incidence, delayed diagnosis and survival outcome.
    Limitations: This was a single-center study conducted at a tertiary center, which may question the generalizability of the findings, as the sample may be biased by overrepresentation with patients who were diagnosed at an advanced stage.

    Abstract Purpose: The aim of this study is to assess the knowledge, attitude, and perception pattern of contraception and family planning among males in Saudi Arabia.
    Methods: A cross-sectional study was conducted using a self-administered questionnaire. Study sample were Saudi males who presented to the urology clinics in one tertiary center. Beside demographic data, we evaluate the responders' knowledge about types of contraceptive methods, usage of one or more methods, reasons for using contraceptives, knowledge of contraception complications, awareness of religious opinion on contraception, the ideal number of children, and birth interval between them. Statistical analysis was performed using the Chi-square and Fisher's exact tests. A value of
    Results: Two hundred and forty-three subjects filled the questioner. The participants' mean age was 42.7 years (range, 19-81); 227 (93.4%) were married. The majority of the participants were aware of the concept of contraception (79%). However, only 54% of the cohort reported using at least one type of contraception. A high percentage of the participants wanted a limited number of children with longer birth intervals. Many factors are responsible for increasing awareness and practice of contraception, additionally; there is limited knowledge and practice regarding male contraception, particularly vasectomy. Withdrawal technique and oral contraceptive pills for females were the most commonly used contraceptive methods for Saudi family planning. The most common reason for using birth control methods was having a lot of children. More than two-thirds of males believed that birth control methods are not prohibited by Islamic law.
    Conclusions: Younger age, shorter duration of marriage, governmental employee, less number of children, higher education degree, and higher monthly income had higher impact on contraception awareness and utilization. Couples still prefer noninvasive methods for contraception. Despite the relatively low use of contraceptive methods, particularly the male methods, the majority of the participants know about contraception. Efforts to advocate and promote the effective use of reproductive and sexual health services among newly married couples are warranted.

    Abstract The major global pressures of rapid urbanization and urban growth are being compounded by climate impacts, resulting in increased vulnerability for urban dwellers, with these vulnerabilities exacerbated during the COVID-19 pandemic. Much of this is concentrated in urban and peri-urban areas where urban development spreads into hazard-prone areas. Often, this development is dominated by poor-quality homes in informal settlements or slums with poor tenure security. Lessons from a resilience-building project in the Pacific shows that a fit-for-purpose (FFP) approach to land administration can provide solutions by increasing the number of households with security of tenure, and consequently, improving resilience outcomes as informal settlements grow. This paper specifically discusses the influence of FFP land administration on reducing vulnerabilities to external shocks, such as climate change and COVID-19. It proposes ways to be better manage urban growth through the responsible governance of land tenure rights and more effective land-use planning to improve resilience to multiple shocks and stresses, hence, delivering improved access to safe land and shelter. Land administration systems can contribute to enhanced resilience to the shocks of climate extremes and pandemics by improving tenure security and enhancing land-use planning controls. It is argued that climate change adaptation and disaster risk reduction need to be better mainstreamed into two major elements of land governance: (i) securing and safeguarding of land rights, and (ii) planning and control of land use.

    Abstract Overapplication of fertilizers is common and may lead to plant toxicity and pollution of water resources in agriculture. A practical method is needed to estimate nutrient stress quickly, easily, and economically for a sustainable management. This study dealt with the prediction of leaf moisture content (MC) and macronutrient contents including nitrogen (N), phosphorus (P), and potassium (K) of soybean leaves using three different optical instruments of chromameter, chlorophyll meter and fluorometer. Forty-five leaf samples from a commercial soybean field were obtained. Color parameters (L*, a*, b*), SPAD (soil plant analysis development) values and quantum yield (QY) values were quantified for each leaf sample. Leaf and soil samples were analyzed using standard leaf and soil chemical analysis procedures. Correlation analysis, principal component analysis (PCA) and partial least square regression (PLSR) were used for data analysis. High correlation was found between two color parameters (L* and b*) with MC and macronutrient contents (r ≥ 0.65) and also, among SPAD, QY, MC, and macronutrient contents (r ≥ 0.70). The prediction models were evaluated based on the RMSEP (root mean square error of prediction) and R ² (coefficient of determination) values. The results showed that soybean leaf N content can be estimated using all three optical instruments with chromameter giving slightly better results (RMSEP = 0.23%, R ² = 0.80). Concerning the MC, chlorophyll meter gave slightly better prediction performance (RMSEP = 2.69%, R ² = 0.73) than the other two instruments. Hence, results suggest that these three optical instruments can be used to assess the macronutrient contents of soybean leaves quickly, easily, and economically.

    Abstract The growing need for ocean monitoring and exploration has boosted underwater wireless optical communication (UWOC) technology. To solve the challenges of pointing, acquisition, and tracking (PAT) in UWOC technology, herein, we propose a 450-nm-laser/scintillating-fiber-based full-duplex (FD)-UWOC system for omnidirectional signal detection in real scenarios. The FD-UWOC system has a -3 dB bandwidth of 67 MHz with a low self-interference level of -44.59 dB. It can achieve a 250-Mbit/s data rate with on-off keying modulation scheme. The system's robustness was validated by operating over 1.5-m underwater channel with air-bubble-, temperature-, salinity-, turbidity-, and mobility-induced turbulence with a low outage probability. Under air-bubble-induced turbulence, the highest outage probability was 28%. With temperature-, salinity-, and turbidity-induced turbulence, the system performed adequately, showing a highest outage probability of 0%, 3%, and 4%, respectively. In mobile cases, the highest outage probability of the FD-UWOC system was 14%, compared to an outage probability of 100% without utilizing the fluorescent optical antenna. To further validate its robustness, a deployment test was conducted in an outdoor diving pool. The system achieved a 250-Mbit/s data rate over a 7.5-m working distance in the stationary case and a 1-m working range in the mobile case with a 0% outage probability. The scintillating-fiber-based detector can be employed in UWOC systems and would help relieve PAT issues.

    Abstract Objective: To report a single-center experience in non-epithelial malignant ovarian tumours (NEMOT), by presenting different clinical and pathological characteristics, management and outcomes.
    Methods: We retrospectively reviewed electronic files of all female patients who underwent surgery for NEMOT at the Gynecology Department of King Abdulaziz University Hospital, Jeddah, Saudi Arabia, from July 2003 to July 2019. We collected baseline demographic, anthropomorphic and clinical data; pathological characteristics; management and follow-up data; and outcomes including residual disease, recurrence and last follow-up status (deceased or alive).
    Results: Thirty-three women were included; mean (standard deviation) age = 33.24 (17.72) years, range = 4, 86 years. Granulosa cell tumor was the most frequent subtype diagnosed in 17 (51.5%) patients, followed by germ cell tumours 13 (39.4%). The majority of patients were diagnosed at FIGO Stage I (22, 66.7%) and with tumor Grade 1 (23, 69.7%), while 8 (24.2%) were diagnosed with Grade 3 tumors. Granulosa cell and Sertoli-Leydig cell tumours were diagnosed at an older age (mean age = 39.30 vs. 23.92 years) compared to germ cell tumours, respectively (P = 0.012). Two-third of the patients benefited from conservative surgery including oophorectomy + staging, and 16 (48.5%) benefited from chemotherapy with bleomycin, etoposide and platinum being the most common protocol (13, 39.4%) for germ cell tumours. Postoperatively, only 2 (6.1%) patients had residual disease. Recurrence and mortality were reported in one and four patients, respectively, resulting in recurrence rate = 3.0% (95% confidence interval [CI] = 0.01%, 15.8%) and mortality rate = 12.1% (95% CI = 3.4%, 28.2%).
    Conclusions: The present series of NEMOT was predominated by sex cord-stromal cell tumors, which were diagnosed in patients with older age, while germ cell tumours were underrepresented. Although survival rates were comparable to those reported internationally, more consideration should be given to following up patients regarding fertility outcomes to provide a more comprehensive evaluation of treatment success and quality of care.
