    Abstract Urban mobility and sustainable transportation are fundamental for the European Union’s goal of achieving climate neutrality by 2050. The EU encourages national governments to prioritize zero-emission urban transport systems that emphasize safety, accessibility, and inclusiveness. Promoting walking plays a fundamental role in sustainable urban mobility, offering advantages such as emission reduction, better air quality, and enhanced public health. Recent research underscores the importance of creating appealing and safe pedestrian environments to encourage walking. These efforts align with the United Nations’ Agenda 2030 sustainability goals, particularly Objective 11, which aims to build inclusive, safe, and sustainable cities and communities. This paper explores the factors influencing pedestrians’ willingness to walk and categorizes them into basic factors, performance factors, and excitement factors, on the basis of the three factors theory. Significantly, the study reveals that the importance of these factors varies based on demographics, mainly the age of the users. Understanding these factors and their relative significance for pedestrian satisfaction is crucial for shaping effective policies and urban planning strategies aimed at promoting sustainable mobility. By prioritizing pedestrian satisfaction and addressing the specific needs and preferences of diverse groups, cities can create more walkable and environmentally friendly urban environments. These findings offer valuable insights for policymakers and urban planners working toward EU climate-neutral objectives and enhancing the well-being of citizens.

    Abstract This research investigates the utilization of human driving models in autonomous vehicles, particularly in scenarios with minimal or no interactions with other vehicles. Human driving models provide valuable insights into driver behavior and play a crucial role in shaping the behavior of autonomous vehicles, enhancing their performance and user experience. The primary focus of this study is the creation of a planning model for autonomous vehicles when navigating roundabouts in the absence of traffic. This model seeks to emulate human driving behavior, ensuring predictability, safety, the optimization of traffic flow, and adaptation to various roundabout geometries. To achieve this, the research introduces a trajectory model that takes into account geometric attributes and speed variations within roundabouts. The model is calibrated using empirical data and generalizes parameters through statistical regression methodologies. In particular, speed profile modeling is evaluated for its consistency in creating plans that faithfully replicate human driving behavior in roundabouts. While the study presents a promising approach, it acknowledges limitations related to the model’s reliance on geometric attributes and its inability to account for external factors like weather conditions. This research underscores the importance of bridging the gap between theoretical research and practical application, with the aim of enhancing safety and the overall user experience in real-world driving scenarios.

    Abstract The speed value of 30 km/h should not be exceeded in urban areas, both to ensure safety requirements for all categories of users and to improve the overall quality of life in urban areas. Moreover, it is necessary not only to comply with the prescribed maximum speed, but also to ensure a uniform speed by limiting the variations in relation to the average value within an acceptable range of variation. An original analysis methodology is therefore proposed, useful for both technicians and administrators to verify the effectiveness of traffic calming measures, especially in areas where these measures are widely used, such as Zones 30. This methodology, called SPEIR (acronym for Speed Profile, Effectiveness Indicators and Results, which are the keywords of the three steps into which the proposed methodology is divided), is divided into three operational steps necessary to both verify the effectiveness of existing traffic calming measures in a given context and to plan new traffic calming measures to be implemented in specific urban sectors to be requalified and revitalized. Finally, three case studies are presented where the application of the SPEIR methodology is useful not only for understanding the operational steps in the application of the methodology itself, but also for understanding the differences in terms of the safety performance that the various traffic calming measures provide to the users of the urban streets where such measures are present.

    Abstract In the context of urban sustainability, promoting pedestrian travel is fundamental. Residents’ needs are critical to creating truly pedestrian-friendly streets. The objective of this study was to answer the following questions: What aspects most increase the willingness of citizens to walk? Is the extent to which these aspects are prioritized related to the context in which citizens move on a daily basis? Two linked surveys, conducted through the institutional website of the Department of Civil Engineering and Architectural of the University of Catania, allowed the inclusion of more than 1000 respondents residing in the metropolitan areas of eastern Sicily. The large database was first reduced using Principal Component Analysis (PCA) and then statistically processed using Path Analysis. Thus, it was found that in the residential areas of the respondents that were not very “pedestrian friendly”, the macro factors of “safety”, “comfort”, and “quality” of pedestrian infrastructures were equally desirable for citizens to adapt to the existing pedestrian routes and thus promote walking. On the other hand, the “attractiveness” of the urban environment as a whole was a non-statistically significant variable for residents’ decision to walk. These results are not valid for every urban context, but the method used is generalizable and applicable in any urban setting. If the quality, safety, and comfort of pedestrian infrastructure results are important, urban planning should prioritize the provision of safe and accessible sidewalks, crosswalks, and pedestrian streets. Furthermore, if the attractiveness of a neighborhood result is significant, urban design and planning should go beyond functional requirements and also focus on creating a vibrant and livable urban environment.

    Abstract High-flow nasal cannula (HFNC) oxygen therapy has rapidly become a popular modality of respiratory support in pediatric care. This is undoubtedly due to its ease of use and safety, which allows it to be used in a wide variety of settings, ranging from pediatric intensive care to patients' homes. HFNC devices make it possible to regulate gas flow and temperature, as well as allowing some nebulized drugs to be administered, features very useful in children, in which the balance between therapeutic effectiveness and adherence to treatment is pivotal. Although the physiological effects of HFNC are still under investigation, their mechanisms of action include delivery of fixed concentration of oxygen, generation of positive end-expiratory pressure, reduction of the work of breathing and clearance of the nasopharyngeal dead space, while providing optimal gas conditioning. Nevertheless, current evidence supports the use of HFNC mainly in moderate-to-severe bronchiolitis, whereas for asthma exacerbations and breath sleeping disorders there is a lack of randomized controlled trials comparing HFNC to continuous positive airway pressure (CPAP) and non-invasive ventilation (NIV), which are essentials for the identification of response and non-response predictors. In this regard, the development of clinical guidelines for HFNC, including flow settings, indications, and contraindications is urgently needed.

    Abstract This study investigated the nature and causes of unsafe driving behavior at roundabouts through an on-road study. Four urban double-lane roundabouts with different layouts were selected for an on-road study. Sixty-six drivers (41 males and 25 females) aged 18–65 years took part in the study. Unsafe behaviors observed during the in situ survey were divided into three different categories: entry unsafe behaviors, circulation unsafe behaviors, and exit unsafe behaviors. Three chi-square automatic interaction detection (CHAID) analyses were developed in order to analyze the influence of roundabout characteristics and maneuvers on unsafe behaviors at double-lane roundabouts. The results confirmed the awareness that double-lane roundabouts are unsafe and inadvisable. More than half of unsafe driving behaviors were found to be entry unsafe behaviors. Moreover, the entry radius was found to be the geometric variable most influencing unsafe driving behaviors.

    Abstract Abstract Background The cystic fibrosis transmembrane conductance regulator (CFTR) gene has been traditionally linked to cystic fibrosis (CF) inheritance in an autosomal recessive manner. Advances in molecular biology and genetics have expanded our understanding of the CFTR gene and its encoding products expressed in different tissues. Aim The study’s aim consists of reviewing the different pathological CF phenotypes using the existing literature. We know that alterations of the CFTR protein’s structure may result in different pathological phenotypes. Methods Open sources such as PubMed and Science Direct databases have been used for this review. We focused our selection on articles published within the last 15 years. Critical terms related to the CFTR protein have been used: “CFTR AND cancer,” “CFTR AND celiac disease,” “CFTR AND pancreatitis,” “children,” “adults,” “genotype,” “phenotype,” “correlation,” “mutation,” “CFTR,” “diseases,” “disorders,” and “no cystic fibrosis.” Results We analyzed 1,115 abstracts in total. Moreover, only 189 were suitable for the topic. We focused on the role of CFTR in cancer, gastrointestinal disorders, respiratory diseases, reproductive system, and systemic hypertension. Conclusions Mutations in CFTR gene are often associated with CF. In this review, we highlighted the broad spectrum of alterations reported for this gene, which may be involved in the pathogenesis of other diseases. The importance of these new insights in the role of CFTR relies on the possibility of considering this protein/gene as a novel therapeutic target for CF- and CFTR-related diseases.

    Abstract Background: H1-antihistamines (H1AH) represent the current mainstay of treatment for chronic spontaneous urticaria (CSU). However, the response to H1AH is often unsatisfactory, even with increased doses. Therefore, guidelines recommend the use of omalizumab as an add-on treatment in refractory CSU. This paved the way for the investigation of targeted therapies, such as monoclonal antibodies (mAbs), in CSU. Methods: A literature review was conducted including papers published between 2009 and 2022 and ongoing trials about the efficacy and safety of mAbs as treatment for CSU. Results: Twenty-nine articles, a trial with preliminary results, and seventeen ongoing or completed clinical trials on the use of mAbs in CSU were included. Randomized controlled trials (RCTs), meta-analysis, and real-life studies have proven the effectiveness and safety of omalizumab as a third-line treatment in refractory CSU. However, a percentage of patients remain unresponsive to omalizumab. Therefore, other mAbs, targeting different pathways, have been used off-label in case series and others are under investigation in RCTs. Most of them have showed promising results. Conclusions: Omalizumab remains the best choice to treat refractory CSU. Although results from other mAbs seem to be encouraging to achieve symptom control in refractory CSU, thus improving patients’ QoL, RCTs are needed to confirm their effectiveness and safety.

    Abstract Multiple properties of lactoferrin have been reported in the literature so far. Decades of in vitro and in vivo studies have demonstrated the important antimicrobial, anti-inflammatory, anti-oxidant, and immunomodulating properties. It suggests the use of lactoferrin as an effective and safe option for the treatment of several common disorders. Herein, we show the applications of lactoferrin in clinical practice, highlighting its evidence-based capacities for the treatment of heterogeneous disorders, such as allergic, gastrointestinal, and respiratory diseases, and hematologic, oncologic, gynecologic, dermatologic, and dental disorders. Moreover, the widespread use of lactoferrin in neonatology is summarized here. As a result of its antiviral properties, lactoferrin has also been proposed as a valid option for the treatment for COVID-19 patients. Here, the uses of lactoferrin in clinical practice as a new, safe, and evidence-based treatment for many types of disorders are summarized.
