Abstract Introduction: In the field of prosthodontics, there lies a delicate balance between the preservation of supporting structures and the restoration of physiological function while providing complete denture prostheses to completely edentulous patients. A good prosthesis requires appropriate recording of the proper maxillo-mandibular relationship, including the vertical dimension of occlusion, which is a crucial step in complete denture fabrication. The reliability of anthropometric methods to determine the vertical dimension of occlusion has been widely discussed in the literature. Aim: To evaluate the correlation of various facial measurements with the vertical dimension of occlusion in dentate subjects. Materials and Methods: This cross-sectional study was conducted in the Department of Prosthodontics at a Dental College in Ambala district of Haryana, India, from February 2021 to July 2023. A total of 100 subjects (50 females and 50 males) within the age range of 20-35 years were selected. Five facial parameters were selected to correlate with the vertical dimension of occlusion: the distance from glabella to subnasion, the distance from the outer canthus of one eye to the inner canthus of the other eye, the distance from the outer canthus to the rima oris, the distance from the outer canthus to the External Auditory Meatus (EAM) on the left side of the face, and the Interpupillary Distance (IPD). Facial measurements were recorded using a digital vernier calliper, while IPD was recorded using a PD ruler. Each measurement was made three times and recorded by a single operator. Once recorded, all the measurements were tabulated and subjected to statistical analysis using the Pearson’s correlation coefficient test. Regression analysis was also conducted to formulate a regression equation for determining the vertical dimension of occlusion. Results: The mean Vertical Dimension of Occlusion (VDO) for males was 59.29±6.48 mm, and for females, it was 52.34±5.92 mm. The results showed a positive and significant ...

Abstract Cystoisospora belli (C. belli) is an opportunistic protozoal parasite. It is prevalent in tropical and subtropical areas of the world. It is frequently encountered in immunocompromised patients with Acquired Immunodeficiency Syndrome (AIDS) and other immunodeficiency illnesses. Infection usually occurs due to faecal contamination of food and water. Chronic severe watery diarrhoea occurs in C. belli infected patients. C. belli is diagnosed by demonstration of characteristic oocyst in the stool sample. The common methods used for stool examination are wet saline mount, iodine mount, and modified acid fast staining. Although C. belli is distributed worldwide and more prevalent in developing countries, it is hardly reported from India. Most of the cases reported from India are in Human Immunodeficiency Virus (HIV)-positive patients and not many are reported in renal transplant patients. Here, a case of a 27-year-old male post renal transplant patient having severe diarrhoea has been presented. The patient complained of watery diarrhoea off and on for last one month for which he had taken some medicine from his local level but was not relieved. The patient was negative for HIV and was on immunosuppressive drugs. The stool sample of the patient was examined by direct wet mount, iodine mount and acid fast staining. On microscopy, characteristic oocysts of C. belli were detected. The patient was given combination treatment to which he responded well.

Abstract Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection causes a disease (COVID-19) with multisystem involvement. The world is now entering a phase of post-COVID-19 manifestations in this pandemic. Secondary hemophagocytic lymphohistiocytosis (sHLH) is a life-threatening hyperinflammatory event triggered by viral infections, including SARS-CoV-2. Both Multisystem Inflammatory Syndrome-Adults (MIS-A) and Cytokine Storm Syndrome (CSS) are considered close differentials of sHLH and add to the spectrum of Post-acute COVID-19 syndrome (PACS). In this report, we presented the case of a middle-aged Asian man who was initially discharged upon recovery from severe COVID-19 infection after 17 days of hospitalization to a private institute and later came to our hospital 13 days post-discharge. Here, he was diagnosed with sHLH, occurring as an extension of CSS, with delayed presentation falling within the spectrum of PACS. The diagnosis of sHLH was made holistically with the HLH-2004 criteria. Our patient initially responded to intravenous immunoglobulin (IVIG) and dexamethasone, later complicated by disseminated Candida auris infection and had a fatal outcome. Though many cases of HLH during active COVID-19 and a few cases post COVID-19 recovery have been reported, based on H-score, which has limitations as a diagnostic tool. We report the first case report of post-COVID-19 sHLH using the HLH-2004 criteria, complicated by disseminated Candidemia, emphasizing that the care of patients with COVID-19 does not conclude at the time of hospital discharge. We highlight the importance of surveillance in the post-COVID phase for early detection of sHLH which may predispose to fatal opportunistic infections (OIs).

Abstract Abstract Oxygen therapy by appropriate oxygen delivery device (ODD) at different stages is an essential part of hypoxemic respiratory failure management. The choice of ODDs depends on the degree of hypoxemia, precision of delivery, patient comfort, risk of transmission of infection to Health Care Worker (HCW), and the cost. Management of hypoxemic respiratory failure depends on underlying etiology and pathophysiology of the disease process. Study from the current global pandemic of novel coronavirus disease 2019 (COVID-19) showed that the severity of hypoxemia is independently associated with in-hospital mortality and can be an important predictor risk of admission to intensive care unit. In this review, we will discuss the different ODDs, their indication for use along with the advantages and disadvantages.

Abstract Ulcerative Colitis (UC) is a chronic recurring inflammatory illness of the gut, whereas Guillain-Barré Syndrome (GBS) is a sudden onset of muscular weakness produced by the immune system attacking the peripheral nerve system. Both UC and GBS can be caused by immune system dysfunction and can co-exist. There are just a few case reports in the literature of GBS occurring in UC patients. However, present case (52-year-old Indian female) is an unusual instance of UC that developed during GBS on Intravenous Immunoglobulin (IVIG) treatment.

Abstract Introduction: A steep rise in the cost of noble metals leads to use of base metal alloys, due to their low cost, good mechanical properties and low density. It would be economically advisable to use fresh alloy in combination with reuse alloy, to reduce the cost of fixed partial dentures. Aim: To evaluate the effect of recasting of Nickel-Chromium (Ni-Cr) and Cobalt-Chromium (Co-Cr) base metal alloys on the metal ceramic flexural bond strength. Materials and Methods: This in-vitro study was conducted in the Department of Prosthodontics, Crown and Bridge of Maharishi Markandeshwar College of Dental Sciences and Research, Mullana, Ambala, Haryana, India in October 2020. A total of 60 specimens were fabricated using Ni-Cr and Co-Cr metal alloys (n=30). Different proportions of new and old Nickel-Chromium (Group A) and Cobalt-Chromium (Group B) base metal alloys were used to fabricate the specimens i.e., Control subgroup A0/ B0 (100% new alloy), subgroup A1/B1 (50% new alloy and 50% remnants from subgroup A0/B0), subgroup A2/B2 (50% new alloy and 50% remnants from subgroup A1/B1). Ceramic was applied in the centre of each prepared metal specimen in the dimension of 8×3×1 mm. All specimens were subjected to three point bending test in Universal Testing Machine (UTM) till failure occurred and values were recorded. Statistical analysis was carried out using Statistical Package for the Social Sciences (SPSS) software version 17.5. Data obtained was compiled and analysed using one-way Analysis of variance (ANOVA), Post-hoc tukey’s and student t-tests. The p-value <0.05 was considered as statistically significant. Results: In Group A (Nickel-Chromium) and Group B (CobaltChromium), maximum flexural bond strength was observed in subgroup A0, B0 (control subgroup) followed by subgroup A1, B1 whereas minimum was observed in subgroup A2, B2. A highly significant difference (p-value <0.001) in flexural strength was observed among all the subgroups. Metal ceramic bond strength observed in Group A (Ni-Cr) was higher than ...

Abstract Introduction: Iron and vitamin A deficiency are two very prevalent and easily preventable nutrient deficiencies. This study was conducted to assess vitamin A status in patients with iron deficiency anemia and to further study the correlation of vitamin A status with biochemical markers of iron deficiency. Materials and Method: Eighty patients with iron deficiency anemia were enrolled and investigated for a complete blood count, an iron profile, liver, and kidney function tests and plasma retinol binding protein levels. The mean age of patients was 31.14 ± 11.33 years, with a range of 16 to 62 years. Results: Mean hemoglobin was 7.19 ± 2.1 g/dL. Serum iron, ferritin, and transferrin saturation were low in all patients, while total iron binding capacity (TIBC) was elevated in only 74 patients (94.81%). Nineteen patients (23.8%) had vitamin A deficiency, with a mean retinol binding protein (RBP) 0.53 ± 0.13 µmol/L. Vitamin A deficient patients had a mean hemoglobin of 6.8±2.14 gm/dL, mean corpuscular volume (MCV) 71.35 ± 8.86 fL, a mean corpuscular hemoglobin (MCH) 19.43±4.36 pg, mean corpuscular hemoglobin concentration (MCHC) 25.44±4.92 gm/dL, serum iron of 28.21 ± 9.73 mcg/dL, serum ferritin 13.04 ± 12.41 ng/mL, transferrin saturation 6.81 ± 3.07%, and TIBC 427.85 ± 78.57 mcg/dL. Among vitamin A deficient patients, RBP had positive correlation with serum iron and transferrin saturation; while, simultaneously showing negative correlation with serum ferritin and TIBC. Conclusion: Vitamin A deficiency affects iron metabolism, causing abnormal iron trapping and systemic iron deficiency, thus worsening the clinical profile of iron deficiency anemia. This study guides us to screen iron deficiency anemia patients for the concomitant vitamin A deficiency for efficient treatment of such patients.

Abstract The SARS-CoV-2 pandemic has led researchers worldwide to study the patterns of association of SARS-CoV-2 with different diseases, which have been a prime focus of medical literature. Osler–Weber–Rendu syndrome, also known as hereditary hemorrhagic telangiectasia (HHT), is a rare genetic disorder in which patients present with recurrent epistaxis, nostril manipulations, and multiple arteriovenous malformations (AVMs) along with telangiectasias involving internal organs and mucocutaneous areas. In addition, these AVMs are prone to bleeding or act as a nidus for thrombus formation, apart from other serious complications, such as chronic hypoxemia, anemia, pulmonary artery hypertension, heart failure, and cerebrovascular accidents. Here, we present a case report of a patient who presented with acute onset respiratory complaints, had multiple episodes of epistaxis in the past, and was later diagnosed with HHT as per Curaçao criteria in our hospital. Doppler ultrasound over the left calf region showed an AVM. Contrast-enhanced computed tomography (CECT) angiography of the chest and abdomen revealed multiple pulmonary and hepatic AVMs along with splenic and uterine telangiectasias and malformations, who on acquiring severe COVID-19 infection developed complications such as anemia, pulmonary artery hypertension, sepsis, acute kidney injury, and post-COVID-19 persistence of Type 1 respiratory failure. Furthermore, the risk-benefit ratio of anticoagulation therapy in such patients with COVID-19 infection is tricky and challenging. However, our patient was prophylactically anti-coagulated with enoxaparin for 12 days with an uneventful outcome.
