    Abstract Coracoid process is a hook-like structure connected with multiple ligaments. The complexity and its close relations often create confusion. The aim of this study is to identify the ligaments attached to the coracoid process as discrete structures on 3T magnetic resonance imaging (MRI) scans. This is a retrospective observational study. One-hundred twenty MRI shoulder studies performed during November and December 2019 at Dept of Radiology, Deenanath Mangeshkar Hospital, Pune, were evaluated. These were obtained on 3T MRI (Siemens, Pennsylvania, United States) machine. All scans were evaluated by two experienced musculoskeletal radiologists. The ligaments studied were coracohumeral (CHL), coracoclavicular (CCL), coracoacromial (CAL), and coracoglenoid (CGL) ligaments. Mean age of cohort was 56.3 years with 59.2% males and 56.2% right shoulder studies. Pain was the main presenting symptom (49%) followed by instability (35%). Reader 1 identified CHL in 106 out of 120 and reader 2 identified 108 out of 120 on all 3 planes. Substantial interobserver agreement (κ =0.65) was noted for sagittal T1 and proton density fat saturated (PDFS) sequences. Both readers could identify CCL and CAL ligaments in all three sequences with better confidence on nonfat saturated sagittal images. Reader 1 identified CGL in 9 on axial and 12 on sagittal images among 120 cases. Reader 2 identified CGL in 13 on axial and 16 on sagittal images among 120 cases. With a κ = 0.702 for the axial PDFS and κ = 0.72 for sagittal T1 and PDFS sequence, substantial interobserver agreement was noted. Sagittal images were better in the evaluation of CGL. Identifying ligaments attached to coracoid on MRI helps in managing associated pathological states.

    Abstract Objective: The goal of this study was to look at the incidence, risk factors, clinical characteristics, and radiological aspects of COVID-19 patients who developed pneumomediastinum and compare these features between those who died and those who survived. Materials and methods: This retrospective observational study included COVID-19 patients having pneumomediastinum on CT from May 2020 to May 2021 in a COVID-19 care hospital. 1st wave patients were considered between the period of May 2020 to January 2021 and those in the second wave between February 2021 to May 2021. The clinical details were analyzed by a consultant intensivist and CT scans were read by a team of 6 resident radiologists and 5 experienced radiologists. Demographic data, co-morbidities, clinical parameters, hemodynamic markers, radiological involvement and associated complications were analyzed. Results: During the study period, 10,605 COVID-19 patients were admitted to our hospital of which 5689 underwent CT scan. 66 patients were detected to have pneumomediastinum on CT; 26 of them in the first wave and 40 in the second wave. Out of 66, 28 patients were admitted to ICU, 9 during the first wave and 18 during the second wave. The overall incidence of developing pneumomediastinum was 1.16%. Incidence in the 1st wave was 1.0% and in the 2nd wave was 1.29%. The overall mortality rate in admitted COVID-19 patients was 12.83% while it was 43.9% in COVID-19 patients who developed pneumomediastinum. Incidence of pneumomediastinum and pneumothorax was high in patients with extensive parenchymal involvement. 59/66 (89%) cases of pneumomediastinum had severe CT score on imaging. Conclusion: We conclude that pneumomediastinum is a marker of poor prognosis. Timely diagnosis of interstitial emphysema or pneumomediastinum will aid in planning early protective ventilation strategies and timely intervention of complications.

    Abstract Purpose: Manual interpretation of chest radiographs is a challenging task and is prone to errors. An automated system capable of categorizing chest radiographs based on the pathologies identified could aid in the timely and efficient diagnosis of chest pathologies. Method: For this retrospective study, 4476 chest radiographs were collected between January and April 2021 from two tertiary care hospitals. Three expert radiologists established the ground truth, and all radiographs were analyzed using a deep-learning AI model to detect suspicious ROIs in the lungs, pleura, and cardiac regions. Three test readers (different from the radiologists who established the ground truth) independently reviewed all radiographs in two sessions (unaided and AI-aided mode) with a washout period of one month. Results: The model demonstrated an aggregate AUROC of 91.2% and a sensitivity of 88.4% in detecting suspicious ROIs in the lungs, pleura, and cardiac regions. These results outperform unaided human readers, who achieved an aggregate AUROC of 84.2% and sensitivity of 74.5% for the same task. When using AI, the aided readers obtained an aggregate AUROC of 87.9% and a sensitivity of 85.1%. The average time taken by the test readers to read a chest radiograph decreased by 21% ( p < 0.01) when using AI. Conclusion: The model outperformed all three human readers and demonstrated high AUROC and sensitivity across two independent datasets. When compared to unaided interpretations, AI-aided interpretations were associated with significant improvements in reader performance and chest radiograph interpretation time.

    Abstract We present a case of a 15-year-old male with history of back pain and bilateral lower limb radiculopathy due to fall. The magnetic resonance imaging scan showed disc bulge at L2-L3 level causing compression on contained nerve roots. In this case, computed tomography scan was indispensable for diagnosis and classification of the vertebral apophyseal fracture and to guide appropriate further management. Apophyseal ring fracture is an uncommon cause of back pain with radiculopathy in adolescents and athletes. High degree of suspicion is necessary to differentiate these injuries from disc herniation so as to further guide appropriate conservative or surgical management. The common cause of back pain in this population is related to musculoskeletal injuries. Lumbar disc herniation contributes to negligible number of cases in this age group, as against that seen in the adult population. An important and rare etiology to be considered for these patients includes vertebral ring apophyseal fracture.

    Abstract Context : Oral cancer is the most common cancer in India and tongue is one of the most commonly affected site. Aims and Objectives : The current study assesses the clinico-epidemiological trends of carcinoma tongue, its survival rates in different stages and relationship between different variables in central India. Materials and Methods : This was a retrospective study of 121 carcinoma tongue patients admitted between the period of 2½ years in a hospital of central India. The data collected were age and sex distribution, location and site of the tumor, tumor stage, histopathological type and grade, nodal status, modality of treatment, recurrences and survival. The disease free survival (DFS) was analyzed against stage, nodal status and recurrences using log rank test. Results : In this study, the incidence of cancer was more in males (male: female = 6.1:1) particularly in their fifth decade (mean 52.7 years) and the most common location was anterior 2/3 rd of the tongue mainly on right lateral side. Most of the patients presenting to out-patient department were having neck nodes positive status. The clinical stage at presentation was mainly advanced stages with well-differentiated squamous cell carcinoma. The most common treatment offered was surgery with radiotherapy (RT), followed by chemotherapy plus RT. The mean DFS time was of 27.8 ± 1.68 months, and it was directly related to tumor stage (P < 0.0001) in comparison to nodes positivity and recurrences. Conclusion : The study signifies better prognosis of carcinoma tongue in early stages and warrants more awareness campaigns and health education in the health facilities as well as in community for early diagnosis of the disease with proper staging and subsequently multimodal treatment for increasing survival rates.
