    Abstract Background Teeth undergoing endodontic therapy are prone to structural weakening and increased risk of fractures. The absence of marginal ridges and pericervical dentin further compromises the fracture resistance. The choice of the post-endodontic coronal seal is crucial for the effectiveness of endodontic therapy. Aim and Objectives This study compared the fracture resistance and fracture modes of endodontically treated teeth (ETT) with mesial-occlusal-distal (MOD) cavities restored with two different posterior composite resins: e-glass fiber reinforced composite (FRC) (everX Posterior) and flowable bulk fill composite (SDR Flow+).  Materials and Methods Sixty human maxillary first bicuspids were divided into four groups: Group PC- positive control (intact teeth), Group NC- negative control (unrestored endodontically treated teeth), Group EXP- samples restored with everX Posterior and nano-hybrid composite, and Group SDR- samples restored with SDR Flow+ and nano-hybrid composite. The NC, EXP, and Smart Dentin Replacement (SDR) samples underwent endodontic procedures and MOD cavity preparation. The samples from EXP and SDR groups were restored with composite resins as post-endodontic coronal seals. The fracture resistance was evaluated using a Universal Testing Machine (UTM), and fracture modes were examined under a dental operating microscope (DOM) at 6x magnification. Statistical tests were performed using One-way ANOVA and Tukeys' post hoc tests. Results  The mean fracture resistance of the experimental groups was as follows: PC- 880.1 ± 209.3 N; NC- 238.1 ± 15.4 N; EXP- 766.1 ± 50.2 N; SDR- 540.8 ± 49.4 N. The highest fracture resistance values were observed in the PC group, whereas the NC group showed the least. The EXP group exhibited significantly higher fracture resistance than the SDR group. Adhesive failure was observed in most samples in the EXP group, whereas samples in the SDR group showed more cohesive failures. Favorable fractures were more prevalent in samples restored with EverX posterior. Conclusion The study findings suggest that everX Posterior can enhance the fracture resistance of structurally compromised ETT. Samples restored with everX Posterior showed a favorable mode of fracture, which can be restored. Applying FRCs can contribute to the longevity and success of endodontic treatment by reinforcing the weakened tooth structure and preventing fractures.

    Abstract Aim The aim of the study was to assess the instrument deformation following the usage of the TruNatomy (Dentsply Sirona, USA) file system in extracted premolars using Scanning Electron Microscopy (SEM). Materials and Methods For the present study, 84 extracted mandibular bicuspids were selected. The teeth were divided into two groups: Group 1, TruNatomy (n=42), and Group 2, Hero Shaper (Micro Mega, France) (n=42). The samples were shaped with 0.03 taper to size 36 with TruNatomy instruments and 0.04 taper to size 30 with Hero Shaper instruments. 5 ml of 5.25% Sodium Hypochlorite (NaOCl) and 2 ml of 17% ethylenediaminetetraacetic acid (EDTA) were used for the final irrigation of root canals. The master apical file was used to instrument seven teeth samples for assessing the safety of the instruments after multiple uses and autoclaving. So, a total of six files per group was used for the analysis of any distortions, cracks or micro-fractures after instrumentation of 42 teeth, at the tip (D0) and 5 mm from the tip (D5) of the rotary file under SEM at 500x magnification. Statistical analysis To determine the significance between the groups, the Mann-Whitney U-test was applied. Results The mean surface wear of the instruments, at the tip (D0) in Group 1 was 1.2857 and in Group 2 was 1.4762. The mean spiral distortion of the instruments, at the tip (D0) in Group 1 was 1.1905 and in Group 2 was 1.4286. A statistically significant difference (P<0.05) was observed between the groups for surface wear and spiral distortion at the tip of the file (D0). There was no significant difference between groups for surface wear and spiral distortion values of the instruments at 5 mm from the tip (D5) (P>0.05). Conclusion The instrument distortion of the rotary file systems analysed was minimal following the biomechanical preparation of seven mandibular bicuspids without root curvature, using a single file. Therefore, both rotary file systems can be considered safe.

    Abstract Snakebite in India is a severe problem as it causes a mortality rate of 58,000 and a disability rate of 140,000 every year which is the highest among any other country. Antivenom is the primary therapy for snakebite, and its manufacturing techniques have essentially stayed unaltered for over a century. Indian polyvalent antivenom, a scientifically validated medicine for treating the toxic effects of snakebites, is available against the venom of the so-called Big Four snakes namely Spectacled cobra (Naja naja), Saw-scaled viper (Echis carinatus), Russell's viper (Daboia russelli) and the Common krait (Bungarus caeruleus), responsible for majority of the deaths in India. India hosts many other species of snakes, including cobras, kraits, saw-scaled vipers, sea snakes, and pit vipers, responsible for clinically severe envenomation. Neutralization strategy has been applied to access the efficacy of antivenoms, crucial for reducing snake bite deaths and disabilities. This review aims to conduct a systematic review and meta-analysis on the neutralization efficiency of the Polyvalent Antivenom (PAV) and focus on the factors that may contribute to the poor recognition of the antivenom towards the venom toxins. Reports focusing on the investigation of antivenom efficacy were searched and collected from several databases. Preclinical studies that reported the neutralization efficacy of the commercial antivenom against the medically important snakes of India were included. The articles were screened based on the inclusion criteria and 8 studies were shortlisted for meta-analysis. Pooled proportion was calculated for the antivenom efficacy reported by the studies and was found to be statistically significant with a 95% confidence interval. The heterogenicity in the venom toxicity and neutralization potency of the antivenom was evident in the overall estimate (proportion) and individual data. We provide comprehensive evidence on antivenom efficacy against medically important snakes from various parts of India which may aid in identifying the gaps in snake envenomation therapy and the need for novel potentially improved treatment of snakebites.

    Abstract Background: Large cavity designs and access cavities impair endodontically treated tooth fracture resistance. As the tooth's strength is known to reduce significantly after the root canal treatment, occlusal loading as a result of functions such as chewing, biting and certain parafunctional tendencies makes the endodontically treated tooth vulnerable to fracture. Hence, after endodontic treatment, it is vital to give adequate and appropriate restorative material to avoid tooth fractures. Accordingly, the choice of such restorative material should be dictated by the property of fracture resistance.
    Objective: The goal of this study was to conduct a systematic review and critical analysis of available data from in vitro studies examining the fracture resistance of endodontically treated posterior teeth restored with fiber-reinforced composites.
    Methodology: The Preferred Reporting Items for Systematic Review and Meta-Analysis (PRIS-MA) Statement was used to guide the reporting of this systematic review A comprehensive literature search was performed using MEDLINE (via PubMed), Scopus, ScienceDirect, Google Scholar, and LILACS. A manual search of the reference lists of the articles was also performed. The databases provided a total of 796 studies from the electronic systematic search. The databases provided a total of 796 studies from the electronic systematic search. Two reviewers scrutinized the papers for eligibility based on inclusion/exclusion criteria and extracted data. The studies were assessed for their potential risk of bias. Based on modified JBI & CRIS (checklist for reporting in vitro studies) guidelines, along with the methodology and treatment objective, we have formulated 13 parameters specifically to assess the risk of bias. A total of 18 studies met the inclusion criteria and were included for qualitative analysis. Considering the high heterogeneity of the studies included, a meta-analysis could not be performed.
    Results: The majority of the included studies had a moderate or high risk of bias. When compared to traditional hybrid composites, fiber-reinforced composites showed increased fracture resistance of endodontically treated teeth in the majority of investigations. On the other hand, limited evidence was found for the bulk fill composites. Moreover, moderate evidence was found for the fracture resistance of inlays and fiber posts with fiber-reinforced composites for core build-up in endodontically treated teeth. No evidence could be found comparing the fracture resistance of endo crowns and fiber-reinforced composites in endodontically treated teeth.
    Conclusion: According to the research, using fiber-reinforced composites instead of conventional hybrid composites improves the fracture resistance of endodontically treated teeth. However, there was a high risk of bias in the research considered. No judgments could be reached about the superiority of one material over another based-on comparisons between other core restorations.
