Abstract Recently, the Microservices whole link whole life cycle optimization framework has been a research hotspot in the academic and engineering fields, especially how to encapsulate the component security mechanism and conduct high concurrency testing based on the existing active framework. The whole link delay, load, and cost have always been the key optimization goals of Microservices splitting and deployment logic. Based on this, this paper proposes a multi-objective optimization Microservices framework that takes into account the security mechanism, Define the fitness function, define the upper and lower limits, and perform multi-dimensional constraints to filter for global and local optima. At the same time, reconstruct the circuit breaker current limiting mechanism, dynamically detect protocol parameters, and perceive the logical relationship of heartbeat status in real-time. Experimental results have shown that this innovative framework can solve scenarios such as high concurrency, high reliability, and high availability Performance bottleneck, business degradation, especially in financial and securities scenarios.

Abstract Previous researches demonstrate that mean arterial pressure (MAP) is associated with major cardiovascular events, but there are few research on the correlation between MAP and brachial-ankle pulse wave velocity (BaPWV). Thus, the present study is designated to examine the association between MAP and BaPWV. Our study is a cross-sectional study in Japanese. They participated in a medical check-up program which included BaPWV and standardized questionnaires. The main measurements include systolic blood pressure, diastolic blood pressure, BaPWV, ankle-brachial index (ABI), fatty liver diagnosed by ultrasonography, etc. It needs to be emphasized that Fukuda et al. finished the study and their data are used for secondary analysis. MAP is positively related with BaPWV [effect size = 10.4; 95% confidence interval (CI), 9.2-11.6] after adjusting age, sex, BMI, smoking status, alcohol consumption, exercise, glutamyltranspeptidase, alanine aminotransferase, aspartate transaminase, total cholesterol, triglyceride, low-density lipoprotein, high-density lipoprotein cholesterol (HDL), fatty liver, fasting-blood glucose, uric acid, estimated glomerular filtration rate, and ABI. The nonlinear association is identified between MAP and BaPWV whose inflection point is 94 mmHg. The effect size and 95% CI on the bilateral sides of the inflection point are 8.0 (4.4-11.6) and 14.8 (12.5-17.1), respectively. Subgroup analysis indicates that MAP has a more significantly positive association with BaPWV in ex-smokers, old people (age ≥60), and participants with lower HDL (<40 mg/dL). Consequently, it is practicable for us to roughly predict BaPWV with simple MAP measurement instead of the BaPWV examination.

Abstract Purpose: Small-scale clinical trials have provided evidence suggesting the effectiveness of stem-cell therapy (SCT) for patients diagnosed with Crohn's disease (CD). The objective of the research was to systematically assess the effectiveness and safety of SCT for individuals diagnosed with CD through a comprehensive review and meta-analysis.
Methods: A search was conducted in Medline (PubMed), CENTER (Cochrane Library), and Embase (Ovid) to find randomized controlled trials (RCTs) that assessed the impact of SCT on the occurrence of clinical remission (CR) and severe adverse events (SAE) among patients diagnosed with CD. The Cochrane Q test and estimation of I
Results: Overall, 12 RCTs involving 632 adult patients with medically refractory CD or CD-related fistula were included. In comparison with placebo or no treatment, SCT showed a greater likelihood of CR (odds ratio [OR] 2.08, 95% CI 1.39-3.12, p < 0.001) without any notable heterogeneity (I
Conclusions: For patients with medically refractory CD or CD-related fistula, SCT may be an alternatively effective and safe treatment.

Abstract Maize kernels were treated using two varieties of drying methodologies, namely combined hot air- and vacuum-drying (HAVD) and natural drying (ND). We performed frequency sweep tests, modified Cole-Cole (MCC) analysis, and frequency-temperature superposition (FTS) on these kernels. The kernels' elastic and viscous properties for ND were higher than those for HAVD. The heterogeneous nature of maize kernel may account for the curvature in MCC plot for the kernel treated by HAVD 75 °C and the failure of FTS. MCC analysis was more sensitive than FTS. The kernel treated by HAVD 75 °C demonstrated thermorheologically simple behavior across the entire temperature range (30-45 °C) in both MCC analysis and FTS. The frequency scale for the kernel treated using HAVD 75 °C was broadened by up to 70,000 Hz. The relaxation processes in the kernel treated by HAVD 75 °C were determined to be mainly associated with subunits of molecules or molecular strands. The data herein could be utilized for maize storage and processing.

Abstract The current state-of-the-art anomaly detection methods based on knowledge distillation (KD) typically depend on smaller student networks or reverse distillation to address vanishing representations discrepancy on anomalies. These methods often struggle to achieve precise detection when dealing with complex texture backgrounds containing anomalies due to the similarity between anomalous and non-anomalous regions. Therefore, we propose a new paradigm-Cosine Similarity Knowledge Distillation (CSKD), for surface anomaly detection and localization. We focus on the superior performance of the same deeper teacher and student encoders by the distillation loss in traditional knowledge distillation-based methods. Essentially, we introduce the Attention One-Class Embedding (AOCE) in the student network to enhance learning capabilities and reduce the effect of the teacher-student (T-S) model on response similarity in anomalous regions. Furthermore, we find the optimal models by different classes' hard-coded epochs, and an adaptive optimal model selection method is designed. Extensive experiments on the MVTec dataset with 99.2% image-level AUROC and 98.2%/94.7% pixel-level AUROC/PRO demonstrate that our method outperforms existing unsupervised anomaly detection algorithms. Additional experiments on DAGM dataset, and one-class anomaly detection benchmarks further show the superiority of the proposed method.

Abstract Introduction: This study aimed to assess factors affecting the prognosis of early-stage hepatocellular carcinoma (HCC) patients undergoing ablation therapy and create a nomogram for predicting their 3-, 5-, and 8-year overall survival (OS).
Methods: The research included 881 early-stage HCC patients treated at Beijing You'an Hospital, affiliated with Capital Medical University, from 2014 to 2022. A nomogram was developed using independent prognostic factors identified by Lasso and multivariate Cox regression analyses. Its predictive performance was evaluated with concordance index (C-index), receiver operating characteristic curve (ROC), calibration curve, and decision curve analysis (DCA).
Results: The study identified age, tumor number, tumor size, gamma-glutamyl transpeptidase (GGT), international normalized ratio (INR), and prealbumin (Palb) as independent prognostic risk factors. The nomogram achieved C-indices of 0.683 (primary cohort) and 0.652 (validation cohort), with Area Under the Curve (AUC) values of 0.776, 0.779, and 0.822 (3-year, 5-year, and 8-year OS, primary cohort) and 0.658, 0.724, and 0.792 (validation cohort), indicating that the nomogram possessed strong discriminative ability. Calibration and DCA curves further confirmed the nomogram's predictive accuracy and clinical utility. The nomogram can effectively stratify patients into low-, intermediate-, and high-risk groups, particularly identifying high-risk patients.
Conclusions: The established nomogram in our study can provide precise prognostic information for HCC patients following ablation treatment and enable physicians to accurately identify high-risk individuals and facilitate timely intervention.

Abstract On July 23, 2022, the World Health Organization (WHO) declared the monkeypox (mpox) outbreak a "Public Health Emergency of International Concern." Since 2022, outbreaks of mpox in many countries around the world have primarily resulted in fatalities among immunocompromised individuals, such as untreated HIV/AIDS patients. Since the eradication of smallpox was declared by the WHO in 1980, the global vaccination against smallpox has been gradually discontinued. China also stopped routine smallpox vaccination in 1981. The protective effect of the smallpox vaccine has decreased over time due to aging and declining immunity in those who were vaccinated. For individuals, timely vaccination against smallpox is an effective means of protection against mpox. However, due to safety concerns with the smallpox vaccine and the limitations of current mpox vaccines, there is no vaccine that is safe, effective, and has low side effects applied in clinical settings. This article provides a comprehensive review of the development of mpox virus (MPXV) vaccines, their application in special populations, and the current state of vaccine research, considering the etiology, transmission, and prevention of the MPXV. Vaccination, as an effective method of epidemic prevention, can provide long-term immune protection and effectively reduce the severity of infection. However, as there is no licensed specific MPXV vaccine available globally, the vaccines currently used for mpox prevention are mostly smallpox vaccines. These smallpox vaccines can offer some degree of protection against mpox by activating cross-protection in the body.

Abstract Purpose: The aim of the study is to identify and evaluate multifaceted factors impacting the survival of elderly cirrhotic HCC patients following ablation therapy, with the goal of constructing a nomogram to predict their 3-, 5-, and 8-year overall survival (OS).
Patients and methods: A retrospective analysis was conducted on 736 elderly cirrhotic HCC patients who underwent ablation therapy between 2014 and 2022. LASSO regression, random survival forest (RSF), and multivariate Cox analyses were employed to identify independent prognostic factors for OS, followed by the development and validation of a predictive nomogram. Harrell's concordance index (C-index), calibration plot and decision curve analysis (DCA) were used to assess the performance of the nomogram. The nomogram was finally utilized to stratify patients into low-, intermediate-, and high-risk groups, aiming to assess its efficacy in precisely discerning individuals with diverse overall survival outcomes.
Results: Alcohol drinking, tumor number, globulin (Glob) and prealbumin (Palb) were identified and integrated to establish a novel prognostic nomogram. The nomogram exhibited strong discriminative ability with C-indices of 0.723 (training cohort) and 0.693 (validation cohort), along with significant Area Under the Curve (AUC) values for 3-year, 5-year, and 8-year OS in both cohorts (0.758, 0.770, and 0.811 for training cohort; 0.744, 0.699 and 0.737 for validation cohort). Calibration plots substantiated its consistency, while DCA curves corroborated its clinical utility. The nomogram further demonstrated exceptional effectiveness in discerning distinct risk populations, highlighting its robust applicability for prognostic stratification.
Conclusion: Our study successfully developed and validated a robust nomogram model based on four key clinical parameters for predicting 3-, 5- and 8-year OS among elderly cirrhotic HCC patients following ablation therapy. The nomogram exhibited a remarkable capability in identifying high-risk patients, furnishing clinicians with invaluable insights for postoperative surveillance and tailored therapeutic interventions.

Abstract Background: Several types of highly-crosslinked polyethylene with different manufacturing processes and mechanical properties are commercially available, including HIGHCROSS (Medacta). The aim of this registry study is to ascertain the long-term safety of the HIGHCROSS liners in total hip arthroplasty and compare the revision rates with its contemporaries using real-world data to establish their safety and generalizability when used by multiple surgeons.
Methods: The Australian Orthopaedic Association National Joint Replacement Registry (AOANJRR) and the Swiss National Joint Registry (SIRIS) were selected for the large number of users of Medacta implants and the availability of long-term results. Three reports from SIRIS and 4 reports from AOANJRR were examined to establish the overall and Kaplan-Meier (KM) cumulative revision rate.
Results: According to SIRIS, the HIGHCROSS liner was the most commonly used bearing surface with Medacta stems with overall revision rates for wear/osteolysis at 9.7 years of 0.04% and 0.03%, respectively, for AMIStem and Quadra. Based on AOANJRR, the KM cumulative revision rate for any reason of Medacta stems with HIGHCROSS liners was lower than that with ceramic liners at 3 years for MasterLoc and at 10 years for Quadra. The KM cumulative revision rate for any reasons of Medacta cementless cups with HIGHCROSS liners at 12 years was lower than the comparator made of all other implants and tribological couplings (1.6% vs 2.1%).
Conclusions: This real-world data proves that long-term HIGHCROSS survival rates are comparable to other modern bearing surfaces.
