    Abstract Objective: The study aimed to assess private general physicians'(GPs) healthcare practices, identifying perceived malpractices, the support they receive, and barriers they experience in providing healthcare services.
    Design: Qualitative exploratory study.
    Setting: Rural district, Thatta in Province of Sindh, Pakistan.
    Participants: 15 GPs.
    Results: Our results include increased motivation among GPs for continued professional development, the high influence of pharmaceutical companies on providers' prescribing practices, perceived malpractices by GPs, and the prevalence of quackery and ineffective regulatory mechanisms for private GPs in a rural district.
    Conclusion: Our findings have implications for the capacity building of GPs by academic institutions, enforcement of regulatory measures by the authorities, and the introduction of measures to curb practices by unqualified practitioners. Finally, more research will be needed to further understand the perceptions of GPs, their needs and the service delivery interventions that will enhance the quality of care they provide.

    Abstract A young man in his 40s was evaluated in the emergency department for abdominal and right flank pain. A CT scan of the abdomen and pelvis showed a solid, well-circumscribed lesion measuring 7.1×8.1×5.4 cm, which was arising from the retroperitoneum and extending from the third portion of the duodenum towards the right kidney. A percutaneous core biopsy was obtained, demonstrating an atypical smooth muscle neoplasm suggestive of a low-grade leiomyosarcoma. The patient underwent surgery for an en-block resection of the mass and the final pathology confirmed a perivascular epithelioid cell neoplasm without significant pleomorphism, mitosis or necrosis. Our case adds to the small number of perivascular epithelioid cell tumour cases reported in the literature and we present it in order to increase our understanding of this tumour and to assist in its appropriate diagnosis and management.

    Abstract Pakistan developed an essential package of health services at the primary health care level as a key component of health reforms aiming to achieve universal health coverage. This supplement describes the methods and processes adopted for evidence-informed prioritization of services, policy decisions adopted, and the lessons learned in package design as well as in the transition to effective rollout. The papers conclude that evidenceinformed deliberative processes can be effectively applied to design affordable packages of services that represent good value for money and address a major part of the disease burden. Transition to implementation requires a comprehensive assessment of health system gaps, strong engagement of the planning and financing sectors, serious involvement of key national stakeholders and the private health sector, capacity building, and institutionalization of technical and managerial skills. Pakistan's experience highlights the need for updating the evidence and model packages of the Disease Control Priorities 3 initiative and reinforcing international collaboration to support technical guidance to countries in priority setting and UHC reforms.

    Abstract Childhood trauma and adverse childhood experiences have a strong relationship with health disparities across the lifespan. Despite experiencing approximately doubled rates of trauma, Adverse Childhood Experiences (ACEs) are poorly characterized in deaf populations. We sought to characterize deaf-specific demographic factors and their association with multiple experiences of ACEs before the age of 18 years old. An analytical cross-sectional approach was used to ascertain associations of deaf-specific demographic factors and experiences with ACEs. The complete dataset included 520 participants for a total response rate of 56%. After adjusting for confounding effects, less severe hearing loss of 16-55 dB (2+ OR: 5.2, 4+ OR: 4.7), having a cochlear implant (2+ OR: 2.1, 4+ OR: 2.6), and not attending at least one school with signing access (2+ OR: 2.4, 4+ OR: 3.7) were significantly and independently associated with reported experiences of multiple ACEs. We conclude that factors associated with childhood hearing loss and language experiences increase risk of experiencing ACEs. Given the strong relationship between ACEs and poor social outcomes, early intervention clinical practice and health policies should consider interventions to support healthy home environments for deaf children.

    Abstract Objective: To determine the improvement in service volumes from baseline, if any, in the contracted out primary and secondary healthcare facilities against key performance indicators, and to explore the perceptions of health managers and experience of patients in this regard.
    Methods: The mixed-method study was conducted at Aga Khan University, Karachi, from November 2019 to April 2020, and comprised secondary data extracted from the district health information system related to Thatta and Sujawal districts of the Sindh province of Pakistan from July 2016 to June 2019. Apart from data analysis for baseline versus end-line comparison of key performance indicators, the study also comprised of a cross-sectional survey of health facilities, patient exit interviews and in-depth interviews with healthcare managers.
    Results: The key performance indicators showed improved service volumes compared to the baseline. All services, including general outpatient department (33%), consultancy services (91%) and emergency services (106%) increased in volumes. Facility-based deliveries increased by 37% and antenatal care visits increased by 100% but immunisation volumes declined. Specialist workforce increased by 47%. Healthcare managers perceived delayed/partial budget release as the key determinant of staff retention, availability of drugs, equipment, supplies, water and electricity at health facilities. Lack of control over government-appointed employees coupled with political interference created workforce shortage. Patients were satisfied with service delivery, but unavailability of medicine was the main concern for 64.3%.
    Conclusions: Contracting out showed improvement in service volumes, but lack of autonomy over budget allocation and utilisation, staff appointment and poor coordination among the stakeholders were key barriers.

Endoplasmic reticulum (ER)-to-Golgi transport of nascent VLDL is the rate-limiting step in its secretion and is mediated by the VLDL transport vesicle (VTV).     Abstract A functional referral system for addressing health emergencies requires coordinated efforts by health system's stakeholders and the community. Using the Rural Health Programme (RHP) Thatta as a platform provided by an academic institution, district health system stakeholders and the community of Mirpur Sakro, taluka of Thatta, were engaged to strengthen the referral system for health emergencies through active community engagement. Through consultations with stakeholders, a referral system plan was developed with community volunteers from each village as focal persons for transportation, referring patients to health facilities and maintaining referral documentation. The stakeholders' experience and perceptions about the referral system were assessed through qualitative in-depth interviews. Patients' experiences of referral system improved with enhanced transport availability, but they faced structural challenges, including out of pocket transport expenses, weak back-referral links, lack of trust between the community and healthcare providers and poor availability of medicines.
