    Abstract Membre de lAcadémie royale de Belgique, Jean-Marie Cauchies a enseigné durant près de quarante années à lUniversité Saint-Louis - Bruxelles. En lui offrant ce Liber amicorum à loccasion de son soixante-cinquième anniversaire, ses collègues ont souhaité honorer limportance de son œuvre scientifique dans le domaine de lhistoire du droit, des institutions et de la vie politique. Ils ont aussi voulu témoigner leur reconnaissance à lanimateur dinnombrables sociétés savantes. Les contributeurs belges et européens réunis pour cet hommage livrent ici une vingtaine détudes touchant les mécanismes du gouvernement et de la législation, mais aussi la mise en œuvre de la justice, tant en France et en Italie quen Suisse, aux Pays-Bas ou en Belgique. Dans la droite ligne des centres dintérêt de Jean-Marie Cauchies qui, médiéviste, na jamais négligé dans ses travaux les périodes plus récentes, le présent ouvrage embrasse la longue durée, du Moyen Âge à nos jours.     Abstract Introduction: The effects of antipsychotic drugs are dose-dependent, which is particularly true for their efficacy, each antipsychotic having a specific dose-response curve. This may justify individualizing doses for these agents.
    Areas covered: We review the pharmacokinetic profiles of seven oral antipsychotics: haloperidol, risperidone, olanzapine, clozapine, quetiapine, ziprasidone, and aripiprazole. Their main indications are psychotic and affective disorders. They are prescribed in a very large population which may have comorbidities. Hence, we analyze the impact of the latter on the pharmacokinetic profiles of these antipsychotics, focusing on renal and hepatic impairment. Reviews and clinical trials were discussed based on a systematic literature search (PubMed) ranging from 1995 to 2022.
    Expert opinion: Factors liable to impact antipsychotic dosage are numerous and their subsequent effects often hard to predict, due to multilevel interactions and compensatory phenomena. In clinical practice, physicians must be aware of these potential effects, but base their decisions on monitoring antipsychotic plasma levels.

    Abstract Introduction: Antipsychotic polypharmacy (APP), defined as the use of more than one antipsychotic, is common in schizophrenia. However, current guidelines consider that the level of evidence to support APP is weak and mostly recommend monotherapy.
    Areas covered: Meta-analyses of randomized controlled trials (RCTs) on the efficacy and tolerability of APP, effectiveness studies on its use in clinical practice, as well as theoretical models liable to legitimate this use are reviewed and discussed on the basis of a systematic literature search (PubMed) ranging from 1995 to 2020.
    Expert opinion: There is now increasing evidence from both efficacy and effectiveness studies, that APP may be beneficial for some schizophrenia patients. The most evidence seems to be for the combination of clozapine and aripiprazole. The choice of this combination may fit in well with the dopamine supersensitivity hypothesis in schizophrenia. Guidelines should be revised, but further studies are needed to confirm the efficacy/effectiveness of this combination, especially in the case of first-episode schizophrenia.

    Abstract Angioedema (AE) is a reason for emergency care when it is severe. Care is difficult when the diagnostic is not known before the attack: mast cell (MC) or bradykinin (BK) mediated. One is very common but often benign, the other rare but potentially fatal. The French national reference center of angioedema (CREAK) provides emergency physicians with a hotline and a guideline to help them manage their patients. This study aimed to describe the clinical features of AE episodes prompting a call on the CREAK hotline and classify patients depending on the suspected cause of the AE. This is a retrospective study between March and August 2019. Each physician calling on the CREAK hotline was asked to fill a clinical description form for the AE emergency. Known patients of CREAK was excluded. Eighty four patients were included. Forty one (48.8%) in the angiotensin converting enzyme inhibitors induced acquired angioedema (ACEi-AAE), 39 (46.4%) in the mast cell induced angioedema, and 4 (4.8%) in the Bradykinin mediated angioedema. The mast cell induced angioedema patients have more history of hives (29.3%) than ACEi-AAE (2.4%, P = .0004). ACEi-AAE mainly affected the tongue (58.5% vs 25.6%, P = .003) and larynx (29.3% vs 13%, P = .001). In 65.5% of cases, the etiological diagnosis was not mentioned by the appellant, but made by the hotline. In 31% of cases, the hotline suggested the administration of a specific treatment not previously provided by the caller. All the doctors who called the hotline appreciate this tele-expertise especially in case of ACEi-AAE presumptions. In addition to providing rapid AE expertise, this service also allows to educate physicians in the management of AE irrespective of its origin.

    Abstract Objectives: This study aimed to monitor the contamination by antineoplastic drugs on work surfaces in a compounding unit 4 years after its implementation.
    Methods: This descriptive study was done in a unit performing on average 45 000 preparations per year. Surface sampling points (N=23) were monitored monthly in the frame of routine activity from the opening of an anticancer drug compounding unit. Contamination with nine antineoplastic drugs (cyclophosphamide, ifosfamide, dacarbazine, 5-fluorouracil, methotrexate, gemcitabine, cytarabine, irinotecan and doxorubicin) was assessed on wipes with a local liquid chromatography coupled with a tandem mass spectrometer analysis. The contamination rate (CR, %) was prospectively monitored every month during the entire study period. The occurrence of critical incidents was also registered. The effect of each safety measure implemented during this period was also analysed.
    Results: Based on the 1104 samples collected between March 2016 and March 2020, the CR was 18.5%. If three different critical incidents among a vial breakage that occurred were individually considered, this CR was slightly lower than that in the literature. Eight months after opening and taking different corrective actions, the overall CR dropped from 42.39% to 11.52% (p<0.001). Contamination was limited to the area that includes the compounding room and, more precisely, the welder and the QC-Prep
    Conclusions: From the beginning of the study and from month to month, surface contamination was limited to the nearest sampling points to the compounding unit. This 4-year monitoring study allowed us to determine the intravenous conventional antineoplastic drugs and sampling points to be focused on.
