    Abstract This article provides an overview of the European Society of Cardiology Guidelines for the management of patients with supraventricular tachycardia, developed in collaboration with the Association for European Paediatric and Congenital Cardiology and presented during the European Congress of Cardiology in Paris on August 31, 2019. The new Guidelines address the issues of diagnosis, acute and chronic treatment of all supraventricular tachycardias, with the exception of atrial fbrillation; they include chapters on supraventricular rhythm disorders in patients with congenital heart defects, tachycardia-induced cardiomyopathy, and pregnant women. Particular attention is paid to the use of radiofrequency ablation as the most modern, effective and dynamically developing technique for eradicating arrhythmias.

    Abstract The COVID-19 pandemic caused by SARS-CoV-2 is the greatest medical problem of our time. The presence of concomitant cardiovascular pathology (CVD) in a patient predisposes to a severe course and the formation of adverse outcomes of COVID-19. It is currently known that SARS-CoV-2 infection leads, on the one hand, to exacerbation and decompensation of the patient's CVD, and, on the other hand, it is the origin of acute CVD. Understanding the impact of COVID-19 on the cardiovascular system is essential to provide comprehensive care for critically ill patients. In this review, we summarize the rapidly changing data on cardiovascular damage associated with COVID-19.

    Abstract The purpose of the work is to analyze the literature and our own data on the supplementation of cholecalciferol in the treatment and prevention of hypovitaminosis D in arterial hypertension (AH), taking into account risk factors and combined antihypertensive therapy. The analysis of literature data and the results of a prospective, controlled, randomized, comparative, single-center clinical study carried out by us on 173 patients with AH grade II, who took cholecalciferol at a dose of 2000 IU / day for 3, 6 or more months, allowed us to offer the algorithms for the supplementation of cholecalciferol in the treatment and prevention of hypovitaminosis D in AH.

    Abstract Interpretation of each electrocardiogram should include measurement and evaluation of the QT interval, because both an increase and a decrease in its duration are associated with a risk of polymorphic ventricular tachycardia and sudden cardiac death. This article reflects the methodological approaches to determining, measuring and evaluating the duration of the QT interval, describes the nature of its dependence on gender, age and heart rate of the patient, and also describes a number of indicators calculated on its basis. Recommendations for measuring and evaluating the QT interval of a standard electrocardiogram have been formulated on the basis of the literature analysis.

    Abstract Since the frst cases of coronavirus disease (COVID-19) have been identifed, the search for effective drugs for its treatment has not stopped. The most promising in the treatment of COVID-19 are antimalarial, antiretroviral drugs and macrolide antibiotics. A common side effect for these groups of drugs is their ability to prolong the QT interval and induce paroxysms of polymorphic ventricular tachycardia torsades de pointes, especially in patients with genetic defects of ventricular myocardial repolarization. This article provides management algorithms of COVID-19 in patients with QT interval prolongation and risk of life-threatening ventricular arrhythmias, developed with the support of the EurAsian Arrhythmology Association (EURA-3).

    Abstract Aim of the study. To study the prevalence of polymorphic variants of the melatonin receptor gene type 2 (MTNR1B) in patients with gastroesophageal reflux disease (GERD) and analyze their association with the features of the clinical presentation of the disease. Material and methods. A total of 46 patients with GERD and 40 patients of the comparison group were examined. The distribution of polymorphic variants of the MTNR1B gene was determined. The association of the genotypes of the studied gene with the severity of the main symptoms of GERD, the level of melatonin and the intensity of expression of the melatonin receptors type 2 in the esophageal mucosa was evaluated. Results. Patients with GERD more often have C/G genotype of MTNR1B (χ²=6.70; р=0.0096). C/G and G/G genotype carriers are characterized by more marked heartburn (p = 0.034) and lower level of 6-СОМТ in 24-hour daily urine (99.97 (70.25; 145.16) vs 58.56 (10.83; 76.71), p = 0.0019) and its daytime portion (129.37 (67.84; 172.19) vs 72.51 (21.10; 105.14), р =0.028) as compared to C/C genotype carriers. Multidirectional links between C/G and G/G genotypes and the severity of heartburn (t = 0.33; p = 0.00091), the level of 6-СОМТ in 24-hour daily urine (τ = -0.38, p = 0.00025) and daytime urine (τ = -0.28, p = 0.0082), as well as the intensity of expression of MTNR1B receptors in the esophageal mucosa (r = -0.43, p = 0.0032) have been established. Conclusions. The received results indicate the association of C/G and G/G genotypes with the development of GERD and its clinical presentation.

    Abstract A review of current publications suggests that patients with arterial hypertension (AH) are at increased risk for severe course and fatal outcome of COVID-19. The main pathogenetic mechanisms explaining the association of AH with COVID-19 include the effect of coronavirus on the functioning of the renin-angiotensin-aldosterone system (RAAS), which is involved in the development and progression of AH. The review presents the results of a number of studies evaluating the effect of the use of ACE inhibitors / ARBs on the development and course of COVID-19 in patients with cardiovascular diseases. The results of international data analysis showed that the use of ACE inhibitors / ARBs in patients with AH and COVID-19 is associated with lower rates of total mortality, and the use of ACE inhibitors, statins and the female sex are associated with a greater likelihood of patient survival. The data on the promising development of new drugs affecting the functioning of RAAS are presented.

    Abstract Heart rate variability (HRV) is understood as the severity of fluctuations in heart rate or RR-intervals in relation to their average values. HRV is defned as the actual variability of the duration of the RR-intervals between consistent heart contraction cycles for a certain period of time. Traditional methods for analyzing heart rate variability are often insuffcient to characterize the complex heartbeat dynamics, since the mechanisms involved in the regulation of the cardiovascular system are likely to interact with each other in a non-linear manner. The article represents data on the association between non-linear parameters of heart rate variability in patients with cardiovascular diseases. It can be concluded, that the non-linear analysis of HRV provides useful information for characterizing the autonomic balance and is a reliable marker of complications and mortality in patients with cardiovascular diseases.

    Abstract This review focuses on major causes and risk factors for death of patients with atrial fibrillation (AF). The authors analyzed current therapeutic strategies for managing patients with AF with respect of their effects on prediction and mortality. Special attention is paid to the strategy of rhythm control and the clinical significance of catheter ablation in the treatment of patients with AF and heart failure.
