    Abstract We consider the semiparametric accelerated failure time (AFT) model with multiple covariates measured with error. Existing methods for the AFT model are either inconsistent, computationally intensive, or require stringent assumptions. To overcome these limitations, we develop a correction approach for a general smooth function of error-contaminated variables. We apply this method to the smoothed rank-based score function for the AFT model. The estimator is consistent and asymptotically normal. The finite-sample performance of the method is assessed by simulation studies. The approach is illustrated by application to data from an HIV clinical trial.

    Abstract Background: In this study, we compared the clinical efficacy of endoscope-assisted anterior cervical discectomy and fusion (ACDF) with open ACDF in the treatment of single-segment cervical spondylotic myelopathy.
    Methods: A retrospective analysis was performed on 52 patients with single-segment cervical spondylotic myelopathy between June 2021 and February 2022, including 33 males and 19 females, with a mean age of 58.42 ± 9.26) years. Among them, 28 patients were treated with endoscope-assisted ACDF (Group A), including 2 cases of C4/5 segment, 16 cases of C5/6 segment, and 10 cases of C6/7 segment; 24 patients were treated with open ACDF (Group B), including 4 cases of C4/5 segment, 11 cases of C5/6 segment, and 9 cases of C6/7 segment. The operation time, intraoperative blood loss, hospital stay, and complications were recorded and compared between the two groups. The Visual Analogue Scale (VAS) and the Japanese Orthopaedic Association (JOA) score were used for clinical evaluation during the follow-up in the 1st month and 3rd month after surgery, and at the final follow-up.
    Results: The 52 patients were followed up on average for 13.04 months (12-17 months). The operation time in Group A and Group B was (105.18 + 8.66) minutes and (81.88 + 6.05) minutes, the intraoperative blood loss was (84.29 + 13.45) mL and (112.92 + 17.81) mL, and the hospital stay was (6.75 + 1.29) days and (7.63 + 1.41) days, respectively. The difference between the two groups was statistically significant (P < 0.05). The VAS and JOA scores in the 1st month and the 3rd month after surgery and the last follow-up significantly improved in both groups compared with those before surgery (P < 0.05). The VAS and JOA scores of Group A in the 1st month, 3rd month after surgery, and the last follow-up were better than those in Group B (P < 0.05). The complication rate in Group A was 7% (2/28), which was not significantly different from the 17% (4/24) in Group B (P > 0.05).
    Conclusion: Both endoscope-assisted ACDF and open ACDF can achieve satisfactory clinical efficacy in the treatment of single-segment cervical spondylotic myelopathy. Although the operation time of endoscope-assisted ACDF is prolonged, it has the advantages of clear vision, thorough decompression, less blood loss, and reduced risk of nerve damage, and is worthy of clinical promotion and application.

    Abstract The application field of drones is being broadened with the development of communication and computer technology, but the communication link and data security of drones are facing great threats due to the limited hardware and software resource environment. Attackers can access not only the intermediate data of drone nodes through various means, but even tamper or manipulate their communication environment, when the drone and the control terminal are not trusted and can be regarded as a white-box attack environment. To solve the security problem in drone communication, a lightweight block cipher CARX for drones is proposed and white-boxed for its implementation to cope with the increasingly severe white-box attacks. And over this cryptographic algorithm, a simulated lightweight white-box encryption communication system for drones (LWED) is designed, which can complete the encryption and decryption operations and secure transmission of data and images, etc. The average encryption rate of the 64-bit version of the algorithm is 7.29×10-2Mbps, and the average encryption rate of white-box CARX (WCARX) is 1.18×10-2Mbps, and the average decryption time of the flight status parameters data of the encrypted communication control system is 0.14s. Compared with similar algorithms, the CARX algorithm can guarantee the encryption and decryption efficiency while providing high security and resistance to a variety of attacks, and can fully secure the data during the communication process after being built into the security system of drone.

    Abstract The annual rainfall in tropical rain forests in Africa is concentrated, and the abundant rainfall can easily lead to roadbed landslides. Therefore, it is necessary to analyze the impact of rainfall on the stability of roadbeds. This paper first uses the pore fluid permeability/stress coupling analysis step provided by ABAQUS to calculate the impact of rainfall infiltration on the overall stability of the roadbed slope and then discusses the rainfall infiltration on the slope seepage field, stress field, and displacement combined with the strength reduction method and the influence of field and safety factors. In the end, it is concluded that the 72-hour rainfall with an intensity of 50 mm/d will reduce the safety factor of the roadbed by 4.9% compared with before the rainfall. At the same time, it will increase the internal pore water pressure of the roadbed, reduce the suction of the matrix, and reduce the effective stress, which is caused by various factors. The overall stability of the roadbed is reduced.

    Abstract Background: Early, rapid, and accurate pathogen diagnosis can help clinicians select targeted treatment options, thus improving prognosis and reducing mortality rates of severe pneumonia. Metagenomic next-generation sequencing (mNGS) has a higher sensitivity and broader pathogen spectrum than traditional microbiological tests. However, the effects of mNGS-based antimicrobial treatment procedures on clinical outcomes and cost-effectiveness in patients with severe pneumonia have not been evaluated.
    Methods: This is a regional, multi-center, open, prospective, randomized controlled trial to evaluate that whether the combination of mNGS and traditional testing methods could decrease 28-day call-cause mortality with moderate cost-effectiveness. A total of 192 patients with severe pneumonia will be recruited from four large tertiary hospitals in China. Bronchoalveolar lavage fluid will be obtained in all patients and randomly assigned to the study group (mNGS combined with traditional microbiological tests) or the control group (traditional microbiological tests only) in a 1:1 ratio. Individualized antimicrobial treatment and strategy will be selected according to the analysis results. The primary outcome is 28-day all-cause mortality. The secondary outcomes are ICU and hospital length of stay (LOS), ventilator-free days and ICU-free days, consistency between mNGS and traditional microbiological tests, detective rate of mNGS and traditional microbiological tests, turn-out time, time from group allocation to start of treatment, duration of vasopressor support, types and duration of anti-infective regimens, source of drug-resistant bacteria or fungi, and ICU cost.
    Discussion: The clinical benefits of mNGS are potentially significant, but its limitations should also be considered.
    Trial registration: ChineseClinicalTrialRegistry.org, ChiCTR2300076853. Registered on 22 October 2023.

    Abstract Background: Colonoscopy is widely used for examination, diagnosis, and treatment because of its low incidence of associated complications. Post-colonoscopy appendicitis (PCA) is very rare and is easily misdiagnosed as electrocoagulation syndrome or colon perforation. Therefore, clinicians should pay close attention to this complication.
    Case summary: A 47-year-old female patient underwent a colonoscopy for a systematic physical examination, and the procedure was uneventful with normal endoscopic and histologic findings. However, the bowel preparation was suboptimal (Boston 2-3-2). After the examination, the patient experienced pain in the lower abdomen, which progressively worsened. Computed tomography of the lower abdomen and pelvis revealed appendiceal calcular obstruction and appendicitis. As the patient refused surgery, she was managed with antibiotics and recovered well.
    Conclusion: In the current literature, the definition of PCA remains unclear. However, abdominal pain after colonoscopy should be differentiated from acute appendicitis.
