    Abstract Breast cancer (BC) is the most common cancer in women worldwide and affects one in eight women in the UK at some point in their lifetime. Advances in treatment have led to greatly improved survival rates. Management of axillary lymph node (LN) metastases has been a controversial but evolving area of BC therapy. Positive LN status is of vital importance in determining long-term outcomes. Recent studies have shown that it is not only the absolute number of involved LNs, but also the ratio of metastatic to examined LNs (or LN ratio [LNR]) that confers prognostic information. In the evaluated article, a population-wide study was performed to elucidate the influence of LNR on mortality risk in Dutch BC patients. LNR was an independent prognostic factor, but was not better than current tumor, node, metastasis pathological nodal staging. It was, however, able to discriminate subpopulations within pathological nodal categories. In this article we examine the use of LNR in BC.

    Abstract Background: Sarcomas form a heterogenous group of relatively uncommon malignant tumours which are derived from connective tissue components. In total they comprise approximately 1% of all new cancers diagnosed per year in the United Kingdom (UK). As subset of this, the 'Unclassified' Sarcoma forms approximately 4% of the total 1. They often present with as relatively slow growing, asymptomatic masses and as such may often be misdiagnosed as in this case.
    Case presentation: A 52 year old man presented to his general practitioner (GP) with left sided chest pain. A strong family history of ischaemic heart disease prompted hospital referral and further investigations which all proved negative for coronary artery disease. Following weight loss and ongoing chest pain, he represented to his GP with a hard mass arising from the left pectoralis major muscle at the site of the previous pain. Surgical excision followed by later compartectomy revealed an unclassified low grade Sarcoma with lymphoma like features.
    Conclusion: In this case, chest pain masquerading as ischaemia, may have been caused by peri-neural infiltration or compression of adjacent muscle bulk by tumour, with eventual surgical resection providing a good long term prognosis.

    Abstract A 78-year-old woman heard a crack in her left mandible while eating a biscuit and reported to her dentist, who urgently referred her to the oral and maxillofacial surgery department. On examination she had a lesion in the body of her left mandible, which had eroded through the lower border and caused a pathological fracture. Her past medical history included a left mastectomy and level II axillary lymph node dissection for a 27 mm grade III invasive ductal carcinoma of the left breast 9 months prior to her mandibular fracture. A transoral incisional biopsy was performed which confirmed the mandibular lesion to be an osteolytic metastasis from the breast. The metastasis was subsequently surgically removed and the remaining mandible repaired with a reconstruction plate followed by postoperative radiotherapy. The patient regained full function of her mandible and is now eating normally. She is being closely followed-up in the oncology outpatient department.

    Abstract Background: Gallstone ileus remains a rare but important cause of intestinal obstruction.
    Method: We present a unique case of two gallstones causing intestinal obstruction at the same time.
    Results: A 90-year-old lady presented with signs and symptoms of small bowel obstruction. At operation, two gallstones stuck at different points within the bowel were causing the obstruction and were removed.
    Conclusion: When operating on patients with small bowel obstruction from gallstone ileus, examination of the entire small bowel should be considered mandatory.

    Abstract Background: Pagets disease of the nipple presents as an eczematous lesion, occurs in 1 - 4% of all female breast carcinoma cases and is invariably associated with underlying malignancy either overt or occult. The majority of these cases are invasive disease although 40-45% are associated with DCIS.
    Case presentation: A 39 year old lady presented to our unit with a palpable lump in the right breast. Radiological and histological investigation proved this to be an extensive area of Ductal Carcinoma in Situ (DCIS) for which she underwent a simple mastectomy and immediate latissimus dorsi flap reconstruction. Histology revealed high grade DCIS with 2 small foci of invasive carcinoma. At 1 year the patient represented with a nodule adjacent to the reconstruction scar which was proved on biopsy to be consistent with Paget's disease. This was proved on formal excision.
    Conclusion: In the absence of underlying breast or apocrine tissue this case details a case of Paget's disease of uncertain origin.
