    Abstract General methods for combining interactions between particles characterised by non-identical intermolecular potentials are investigated. The combination methods are tested by performing molecular dynamics simulations to determine the pressure, energy, isochoric and isobaric heat capacities, thermal expansion coefficient, isothermal compressibility, Joule-Thomson coefficient, and speed of sound of 10-5 + 12-6 Mie potential binary mixtures. In addition to the two non-identical Mie potentials, mixtures are also studied with non-identical intermolecular parameters. The combination methods are compared with results obtained by simply averaging the Mie exponents. When either the energy or size parameters are non-identical, very significant differences emerge in the thermodynamic properties predicted by the alternative combination methods. The isobaric heat capacity is the thermodynamic property that is most affected by the relative magnitude of the intermolecular potential parameters and the method for combining non-identical potentials. Either the arithmetic or geometric combination of potentials provides a simple and effective way of performing simulations involving mixtures of components characterised by non-identical intermolecular potentials, which is independent of their functional form.

    Abstract Among the uremia-associated risk factor, which can be influenced today, anemia is considered most relevant because it induces functional and organic alterations of cardiac/circulatory function. Research concerning influence of the anemia on the pulmonary hemodynamic and cardiac output (CO) in pre-uremic patients are not available up to now. Cardiac and circulatory function of 52 patients were examined before initiation of dialysis therapy using a Swan-Ganz thermodilution catheter. After excluding patients with impaired cardiac pump function the results of 31 of the 52 patients could be analyzed. They were divided into two groups (Hb > resp. < 7.0 g/dl): in patients with severe anemia (Hb 5.7 +/- 0.6 g/dl; n = 7) cardiac index was higher (4.8 +/- 0.4 1/min/m2 < 0.01) compared with the other group (Hb 9.8 +/- 1.7 g/dl; n = 24; CI 3.9 +/- 1.1 1min/m2). The increase of cardiac index caused by anemia correlated with increased stroke volume and heart rate and lowered pulmonary and peripheral resistance. Patients with severe anemia showed a tendency to an impaired cardiac index below Hb < 5-6 g/dl. The hypercirculation did not cause an increase of the pulmonary arterial and pulmonary wedge pressure. Particularly in the case of already existing myocardial damage and coronary arteriosclerosis the presence of anemia and renal insufficiency leads to a highly increased morbidity and mortality. This "cardio-renal anemia-syndrom" is responsible for frequent refractory heart failure. Disturbances of cardiac/circulatory function are observed in pre-uremic patients three times more frequently than in patients after myocardial infarction. Early correction of anemia seems to reduce the risk of fatal cardial complications and to improve the quality of life and the prognosis of pre-uremic patients.

    Abstract History and admission findings: A 26-year-old Pakistani woman was admitted to hospital with fever, nausea and vomiting, abdominal pain and general weakness for the previous two weeks, but no diarrhea or constipation. Antibiotic therapy as an out-patient had not provided relief. She also had enlarged cervical lymph nodes. She reported that she had always been in good health before onset of the described symptoms.
    Investigations: Laboratory tests revealed pancytopenia and a decrease in C3 and C4. The ANA titer was positive. The serum was positive for preceding infections with EBV, CMV, Chlamydia trachomatis, HHV type 6, and toxoplasmosis. There was no evidence of current or previous infections with HIV, Bartonella henselae, Treponema pallidum, Listeria, Hantavirus, Leishmania or dengue fever. The patient's symptoms were found to be associated with Hashimoto's thyroiditis in conjunction with latent hypothyroidism. Abdominal, thoracic and cervical CT scan revealed enlarged lymph nodes, up to 2 cm in size, and enlarged spleen and liver.
    Therapy and clinical course: Histopathology of a cervical lymph node showed features of Kikuchi (Kikuchi-Fujimoto) disease (histiocytic necrotizing lymphadenitis). Prednisone administration (2 mg/kg body weight) led to the patient's rapid recovery.
    Conclusion: Kikuchi disease should be considered in patients with cervical but also general lymphadenopathy, after exclusion of other causes. The specific diagnosis is based on biopsy of affected lymph nodes.

    Abstract Background: Up to every fourth woman with multiple sclerosis (MS) or neuromyelitis optica spectrum disorder (NMOSD) suffers a clinically relevant relapse during pregnancy. High doses of steroids bear some serious risks, especially within the first trimester of pregnancy. Immunoadsorption (IA) is an effective and more selective treatment option in disabling MS relapse than plasma exchange. Data on the use of IA during pregnancy and breastfeeding are scarce.
    Methods: In this retrospective multicenter study, we analyzed the safety and efficacy of IA treatment in acute relapses during pregnancy or breastfeeding. The primary outcome parameter - change of acute relapse-related disability after IA - was assessed using Expanded Disability Status Scale (EDSS) and visual acuity (VA) measurements for patients with optic neuritis (ON).
    Results: A total of 24 patients were analyzed, 23 with relapsing-remitting MS, and 1 with NMOSD. Twenty patients were treated with IA during pregnancy. Four patients received IA postnatally during the breastfeeding period. Treatment was started at a mean 22.5 [standard deviation (SD) 13.9] days after onset of relapse. Patients were treated with a series of 5.8 (mean, SD 0.7) IA treatments within 7-10 days. Sixteen patients received IA because of steroid-refractory relapse, eight were treated without preceding steroid pulse therapy. EDSS improved clinically relevant from 3.5 [median, interquartile range (IQR) 2] before IA to 2.5 (median, IQR 1.1) after IA,
    Conclusions: Tryptophan-IA was found to be effective and well tolerated in MS/NMOSD relapses, both as an escalation option after insufficient response to steroid pulse therapy and as first-line relapse treatment during pregnancy and breastfeeding.

    Abstract ANAMNESE UND KLINISCHER BEFUND: Eine 26-jährige pakistanische Patientin wurde wegen seit 2 Wochen bestehendem Fieber bis 39 ˚C, trockenem Reizhusten, abdominellen Schmerzen und Erbrechen bei gemindertem Allgemeinzustand stationär aufgenommen, nachdem die vorherige ambulante antibiotische Behandlung keine Besserung der Beschwerden ergeben hatte. Am Hals waren vergrößerte Lymphknoten tastbar. Die Eigen-anamnese erbrachte keine Hinweise auf relevante Vorerkrankungen. UNTERSUCHUNGEN: Es bestand eine Panzytopenie, sowie erniedrigtes C3 und C4 bei positivem ANA-Titer. Im Serum fanden sich Hinweise auf vorausgegangene Infektionen mit EBV, CMV, Chlamydia trachomatis, HHV Typ 6 und Toxoplasmen. Demgegenüber war die Serologie unauffällig bezüglich HIV, Bartonella henselae, Syphilis, Listerien, Hantaviren, Leishmanien und Denguefieber. Es zeigte sich der Befund einer aktiven Hashimoto-Thyreoiditis mit derzeit latenter Hypothyreose. In einer Computertomographie von Abdomen, Thorax und Hals stellten sich ubiquitär pathologisch vergrößerte Lymphknoten bis zu einem Durchmesser von maximal 2 cm sowie eine Hepatosplenomegalie dar. THERAPIE UND VERLAUF: In der histologischen Untersuchung eines operativ entfernten zervikalen Lymphknotens zeigte sich eine Kikuchi-Lymphadenitis. Unter Therapie mit Prednisolon 2 mg/kg KG) besserten sich die Symptome rasch. FOLGERUNG: Nicht nur bei zervikaler, sondern auch bei generalisierter Lymphadenopathie sollte nach Ausschluss anderer Ursachen immer auch an eine Kikuchi-Lymphadenitis gedacht werden. Die Diagnosestellung ist nur über die histologische Begutachtung eines betroffenen Lymphknotens möglich.

    Abstract Double electron capture by proton-rich nuclei is a second-order nuclear process analogous to double beta decay. Despite their similarities, the decay signature is quite different, potentially providing a new channel to measure the hypothesized neutrinoless mode of these decays. The Standard-Model-allowed two-neutrino double electron capture ($2\nu ECEC$) has been predicted for a number of isotopes, but only observed in $^{78}$Kr, $^{130}$Ba and, recently, $^{124}$Xe. The sensitivity to this decay establishes a benchmark for the ultimate experimental goal, namely the potential to discover also the lepton-number-violating neutrinoless version of this process, $0\nu ECEC$. Here we report on the current sensitivity of the NEXT-White detector to $^{124}$Xe $2\nu ECEC$ and on the extrapolation to NEXT-100. Using simulated data for the $2\nu ECEC$ signal and real data from NEXT-White operated with $^{124}$Xe-depleted gas as background, we define an optimal event selection that maximizes the NEXT-White sensitivity. We estimate that, for NEXT-100 operated with xenon gas isotopically enriched with 1 kg of $^{124}$Xe and for a 5-year run, a sensitivity to the $2\nu ECEC$ half-life of $6 \times 10^{22}$ y (at 90% confidence level) or better can be reached.
