    Abstract Aim: Volvulus is a disease characterized by the abnormal twisting of a gaseous distension of the loop of the intestine around itself. Colonic volvulus is the third common cause of colonic obstruction. The study aimed to determine whether the percentage of immature granulocytes is a marker of early necrosis in the volvulus.
    Methods: Demographic characteristics of the cases were collected in the study. In addition, age, gender, white blood cell count, immature granulocyte count, immature granulocyte percentage, platelet lymphocyte ratio, and neutrophil-lymphocyte ratio were compared between groups with and without necrosis in terms of treatment.
    Results: The effectiveness of immature granulocyte percentage (IG%) and IG markers were statistically significant in predicting necrosis. The AUC was the highest for IG (0.820, 95% CI: 0.586-1), followed by IG% (0.820, 95% CI: 0.617-1). The optimal cut-off value for the IG was 0.65, with a sensitivity of 76.2% and specificity of 85.7% (P = 0.013). The optimal cut-off value for the IG% was 0.065, with a sensitivity of 85.7% and specificity of 71.4% (0.018).
    Conclusion: IG% and IG count were found to be useful for predicting necrosis in patients with volvulus.

    Abstract Background/aim: His study aimed to evaluate the availability of common diagnostic tests and biochemical markers in predicting poor prognosis in patients with pancreatic adenocarcinoma (PAC). The primary outcome measure was to identify predictive prognostic factors. The secondary outcome measure was to compare predictive measures in patients who survived or did not survive in the follow-up period.
    Materials and methods: Medical data of 51 patients were obtained who underwent resection surgery for PAC between January 2016 and May 2022. There were two groups according to the mortality in the follow-up period group general mortality positive (GMP; n = 29) and group general mortality negative (GMN; n = 22). Stage IIb was the most common stage in subgroups.
    Results: Preoperative diagnostic tests revealed that aspartate aminotransferase (AST) level, De Ritis ratio (DRR), carcinoembryonic antigen (CEA), cancer antigen 19-9 (CA 19-9), immature granulocyte (IG) count, and IG ratio (IG%) are higher, and hemoglobin (Hgb) levels are lower in the GMP group (P < 0.05). In univariate analysis, seven variables, including AST ≥20.5 (P = 0.001), DRR ≥1.05 (P ≤ 0.001), CEA ≥3.32 (P = 0.02), IG count ≥0.06 (P < 0.01), Hgb ≤11.75 (P = 0.01), poor differentiation (P < 0.001) and existence of life-threatening complication (P < 0.01) were identified. In multivariate analysis, only DRR (≥1.05;100% specificity and 72% sensitivity) and poor differentiation (P = 0.019) were found to be independent prognostic factors for overall survival. The median overall survival of patients with the DRR ≥1.05 and poor tumor differentiation was lower, and the mortality rate was higher than the patients with DRR and without poor tumor differentiation (10.65 ± 3.11 months vs. 61.86 ± 5.36 months and 100% vs. 26.7%, P < 0.001).
    Conclusion: Pretreatment high DRR, high IG counts and IG%, and poor differentiation of the tumor might be used as independent predictors of poor prognosis and mortality in patients with PAC.

    Abstract The proposed economy is green due to the fact that, according to the authors, it is based on resource and energy conservation and energy efficiency, rational use of natural resources, as well as the disposal of man-made and household waste. Thus, with the energy saving potential in Russia of 40%, respectively, less land will be used for the extraction of fossil hydrocarbons, the area of pollution of adjacent areas during transportation will be reduced, and territories will also be freed up due to the processing of industrial waste and solid waste. All of these measures will help conserve the land and its overlying green spaces. As a result of the unprecedented economic growth of recent decades due to the anthropogenic factor - deforestation, the construction of mines, mines and oil wells, the unprecedented growth of cities, the almost uncontrolled burning of fuel, the expansion of areas occupied by industrial and domestic waste, the useful green surface of the planet (forests, farmland, parks, gardens, swamps) has been significantly reduced. That is, there is a growing imbalance of natural and physical capital. And the green color is a kind of indicator of this disharmony (balance) on the planet, which occurs due to human activity. Therefore, the new economic model should be energy efficient, low carbon, innovative, and necessarily green.

    Abstract Direct marketing is one of the most significant and effective strategies in terms of attracting a target audience interested in specific services or products. The essence of this type of marketing is that there is no need to involve specialists in intermediary services, and it is aimed only at its target group of customers. In the implementation of direct marketing, special attention is paid to the marketing information system and marketing research. It is appropriate to note that marketing, as an economic and social category, develops on the basis of continuous changes in technology, supply and demand, which predetermines the adaptability, and not at all the sustainability of the new marketing concept. A marketing information system involves the systematic collection, analysis, interpretation, and communication of market information within an organization. It collects data from both internal and external sources of the organization. All important marketing activities such as price, new product development, packaging, distribution, promotion, media, etc. depends on the information provided by that system. The availability of quality information helps organizations make more informed market decisions. Wrong decisions regarding market operations will have a negative impact on the business and hinder its survival.

    Abstract Objective: The aim of the investigation was to determine the influence of such parametric characteristics of the stone such as size and volume on the duration of tulium laser disintegration of the urinary stone and to determine which of these parameters is more effective to use like prognostic criterion for the duration of the planned surgical intervention in the percutaneous nephrolithotripsy.
    Materials and methods: Overall 52 patients (27 females and 25 males) with an average age of 56,9 (25-79) years participated in the present study. All patients underwent percutaneous nephrolithotripsy with disintegration of the kidney stone by thulium energy. Inclusion criteria were: stone size more or equal 2 cm, stone density >1100 HU. Exclusion criteria were: patients with a single kidney, urinary tract divertions, coagulopathy. The average operation time was 30 (15-100) minutes, with an average puncture time of 3.15 (1-10) minutes and lithotripsy time of 28 (14-98) minutes. To determine the volume we used the method of automatic lithometry according to CT data using the software: Vitrea ver. 4.1.52. The size of the stone was determined by the longest length in one of the plane. During the study it was found that the average size of the stone was 28.25 (20-58) mm and the average volume was 2579.4 (250-9990) mm3. To confirm our assumption, we decided to determine the dependence of the time of disintegration of the stone on the size and volume of the stone. For this purpose, we graphically presented the correlation of these parameters.
    Results: We have drawn 2 graphs reflecting the dependence of lithotripsy time parameters on the size and volume characteristics of the stone. As a result of comparing these parameters we found that size is a prognostically less reliable predictor of lithotripsy time, and is not characterized by a linear distribution, in contrast to the stone volume.
    Conclusions: Thereby, the main stereoscopic characteristic of a stone is a volume, which should be the primary guide in selecting the preferred method of treatment as well as in predicting the operative time and associated risks.

    Abstract The measured charged particle [Formula: see text] spectra in proton-proton collisions obtained by the CMS experiment at CERN is compared with the simulation results of EPOS-LHC and Pythia8.24 models at 7 TeV center-of-mass energy. The Pythia8.24 model describes the experimental data very well, particularly in the high [Formula: see text] region. The model also predicts the [Formula: see text] spectra for [Formula: see text] [Formula: see text] [Formula: see text] < 2.4 at 0 [Formula: see text] [Formula: see text] [Formula: see text] 6 [Formula: see text]. The EPOS-LHC model underpredicts the [Formula: see text] spectra from 0.1 to 2 [Formula: see text] in all [Formula: see text] bins for about 20% and the [Formula: see text] spectrum from 0.1 to 4.2 [Formula: see text] for [Formula: see text] [Formula: see text] [Formula: see text] < 2.4 by about 15% while reasonably predicts well for [Formula: see text] > 4.2 [Formula: see text] within the experimental errors. Furthermore, to get information about collective properties of the hadronic matter, modified Hagedorn function with embedded transverse flow velocity and thermodynamically consistent Tsallis distribution functions are used to fit the experimental data and simulated results. The values of [Formula: see text] show that the functions fit the data and simulation results well. The parameter extracted by the functions: [Formula: see text], [Formula: see text], and [Formula: see text] decreases with increasing [Formula: see text]. The decrease in [Formula: see text] with increasing [Formula: see text] is due to the large energy deposition in lower rapidity bins producing rapid expansion due to large pressure gradient resulting quick expansion of the fireball. Similarly, large energy transfer in the lower pseudo-rapidity bin results in higher degree of excitation of the system which results larger values of [Formula: see text] and [Formula: see text]. The values of the fit constant [Formula: see text] increase with [Formula: see text] where the values of [Formula: see text] extracted from Pythia8.24 are closer to the data than the EPOS-LHC model. The Pythia8.24 model has better prediction than the EPOS-LHC model which might be connected to its flow-like features and color re-connections resulting from different Parton interactions in the initial and final state.

    Abstract This lecture is dedicated to increasing stone free rate (SFR) in patients with staghorn renal stones by using a multimodal approach. Percutaneous nephrolithotomy (PCNL) is the "gold standard" for the treatment of this group of patients. Depending on a stone size and complexity, SFR for PCNL varies from 55 to 98%. Due to low SFR, which is a main criterion for the effective procedure, various approaches currently are being suggested. In this lecture, we highlighted the following approaches: multi-access PCNL, a use of a ureteroscope in case of antegrade stone migration, a use of flexible instruments, ureterorenoscopy in combination with PCNL. Various lithotripsy techniques are briefly described. The advantage of ultrasound-guided puncture of the kidney is also discussed.

    Abstract Introduction: The aim of the study is to compare the results and complications of muscle- invasive bladder cancer treatment using endovideo - surgical radical cycstectomy with orthotopic reservoir formation.
    Materials and methods: Between 2013 and 2016 years, 49 patients with muscle -invasive bladder cancer underwent endovideo-surgical treatment with orthotopic reservoir formation. 38 patients underwent laparoscopic radical cystectomy with extracorporeal ileal conduit urinary diversioon [ men - 28 (74%), women - 10 (26%)], intracorporeal reservoir were perfomed in 11 cases [men - 8 (73%) and women - 3 (27%)]. The Modified Studer Ileal Neobladder (1st group) was performed in 32 ( 84%) cases, The Hautmann neobladder in 6 (16%) cases. During intracorporeal neobladder formation (2nd group) in all 11 cases Ileal Neobladder was created according to the methods of Karolinska Institutet, ( Sweden).
    Results: In the first group 7 (18%) patients had early postoperative complications: in 1 case - Anastomosis failure, 3 patients - Defect of ureteroileal anastomosis and in other 3 cases - Adhesive intestinal obstruction. There was an incidence of late postoperative complications in 6 (15%) cases: 1 patient had exacerbation of chronic pyelonephritis, other 5 patients had stenosis of uretero-ileal anastomosis. Stenosis treatment methods were: in 3 cases - Laser endoureterotomy, and 2 patients underwent Intestinal plastic surgery of the ureter. In the second group 5 (45%) patients had early postoperative complications: in 2 cases - Defect of ureteroileal anastomosis, 2 patients had acute postoperative intestinal obstruction and in 1 case neobladder defect (multiple defects). In this group 4 (36%) patients had late postoperative complications: in 2 cases Stenosis of uretero-ileal anastomosis and other 2 - Active phase of chronic pyelonephritis.
    Conclusion: Due to medical technologies development and endovideo-surgical equipment improving, it became possible to perform high-technological operations, such as a laparoscopic radical cystectomy with neobladder formation using different parts of intestine. To imptove the results technical difficulties, postoperative complications and durations of operations were appraised, and we reccomend to perform laparoscopic radical cystectomy with extracorporeal neobladder formation.

    Abstract Background: Despite low HIV prevalence in the South Caucasus region, transmission is volatile. Little data are available from this region about addiction and infectious diseases among prisoners who transition back to communities.
    Methods: A nation-wide randomly sampled biobehavioral health survey was conducted in 13 non-specialty Azerbaijani prisons among soon-to-be-released prisoners. After informed consent, participants underwent standardized health assessment surveys and testing for HIV, hepatitis B and C, and syphilis.
    Results: Of the 510 participants (mean age = 38.2 years), 11.4% were female, and 31.9% reported pre-incarceration drug injection, primarily of heroin. Prevalence of HCV (38.2%), HIV (3.7%), syphilis (3.7%), and HBV (2.7%) was high. Among the 19 HIV-infected inmates, 14 (73.7%) were aware of their HIV status, 12 (63.2%) were receiving antiretroviral therapy (ART), and 5 (26.3%) had CD4 < 350 cells/mL (4 of these were on ART). While drug injection was the most significant independent correlate of HCV (AOR = 12.9; p = 0.001) and a significant correlate of HIV (AOR = 8.2; p = 0.001), both unprotected sex (AOR = 3.31; p = 0.049) and working in Russia/Ukraine (AOR = 4.58; p = 0.008) were also correlated with HIV.
    Conclusion: HIV and HCV epidemics are concentrated among people who inject drugs (PWIDs) in Azerbaijan, and magnified among prisoners. A transitioning HIV epidemic is emerging from migration from high endemic countries and heterosexual risk. The high diagnostic rate and ART coverage among Azerbaijani prisoners provides new evidence that HIV treatment as prevention in former Soviet Union (FSU) countries is attainable, and provides new insights for HCV diagnosis and treatment as new medications become available. Within prison evidence-based addiction treatments with linkage to community care are urgently needed.
