    Abstract Industry 4.0 and digital transformation will likely come with an era of changes for most manufacturers and tech industries, and even healthcare delivery will likely be affected. A few trends are already foreseeable such as an increased number of patients, advanced technologies, different health-related business models, increased costs, revised ethics, and regulatory procedures. Moreover, cybersecurity, digital invoices, price transparency, improving patient experience, management of big data, and the need for a revised education are challenges in response to digital transformation. Indeed, forward-looking innovation about exponential technologies and their effect on healthcare is now gaining momentum. Thus, we developed a framework, followed by an online survey, to investigate key areas, analyze and visualize future-oriented developments concerning technologies and innovative business models while attempting to translate visions into a strategy toward healthcare democratization. When forecasting the future of health in a short and long-term perspective, results showed that digital healthcare, data management, electronics, and sensors were the most common predictions, followed by artificial intelligence in clinical diagnostic and in which hospitals and homes would be the places of primary care. Shifting from a reactive to a proactive digital ecosystem, the focus on prevention, quality, and faster care accessibility are the novel value propositions toward democratization and digitalization of patient-centered services. Longevity will translate into increased neurodegenerative, chronic diseases, and mental illnesses, becoming severe issues for a future healthcare setup. Besides, data privacy, big data management, and novel regulatory procedures were considered as potential problems resulting from digital transformation.     Abstract The typical curriculum of training and educating future clinicians, biomedical engineers, health IT, and artificial intelligence experts lacks needed twenty first-century skills like problem-solving, stakeholder empathy, curiosity stimulation, entrepreneurship, and health economics, which are essential generators and are pre-requirements for creating intentional disruptive innovations. Moreover, the translation from research to a valuable and affordable product/process innovation is not formalized by the current teachings that focus on short-term rather than long-term developments, leading to inaccurate and incremental forecasting on the future of healthcare and longevity. The Stanford Biodesign approach of unmet clinical need detection would be an excellent starting methodology for health-related innovation work, although unfortunately not widely taught yet. We have developed a novel lecture titled HealthTec Innovation Design (HTID) offered in an interdisciplinary setup to medical students and biomedical engineers. It teaches a future-oriented view and the application and effects of exponential trends. We implemented a novel approach using the Purpose Launchpad meta-methodology combined with other innovation generation tools to define, experiment, and validate existing project ideas. As part of the process of defining the novel curriculum, we used experimentation methods, like a global science fiction event to create a comic book with Future Health stories and an Innovation Think Tank Certification Program of a large medical technology company that is focused on identifying future health opportunities. We conducted before and after surveys and concluded that the proposed initiatives were impactful in developing an innovative design thinking approach. Participants' awareness and enthusiasm were raised, including their willingness to implement taught skills, values, and methods in their working projects.     Abstract Biochar (BC) is gaining attention day by day due to its potential benefits for the improvement in degraded soil health. During its production by pyrolysis, carbon sequestration is an important aspect that makes it environment-friendly amendment. However, 100% anaerobic combustion of waste at such a high temperature decreases its adaptability to produce BC at commercial scale. On the other hand, the alkaline nature of BC also causes adverse effects on soil health when used in alkaline soils. Keeping in mind the problem of BC production and its high pH, current experiment was conducted to introduce chemical production of acidified activated carbon (AAC) and its effects on soil nutrients status comparative to high temperature pyrolyzed BC. As compared to thermal pyrolysis, sulphuric acid produce acidified activated carbon in minimum time and large in quantity. Sulphuric acid produces acidified activated carbon, fix higher carbon as compared to thermal pyrolyzed BC. Results also showed that application of 2% AAC was far better for decreasing alkaline soil pHs (3.52 and 4.71%) and ECe (45.2 and 71.4%) as compared to control in clay and sandy clay loam. A significant maximum increase in available P (117.5 and 25.9%), extractable Zn (42.0 and 52.2%), B (111.4 and 46.2%) and Fe (59.5 and 34.4%) in clay and sandy clay texture soils also validated the efficacious functioning of AAC over BC and control. It is concluded that sulphuric acid use is an easier and adaptable method to produce activated carbon at commercial scale. As compared to thermal pyrolyzed BC, application of AAC could be more effective in the improvement of soil health and fertility status.

    Abstract This paper presents a deep learning-based analysis and classification of cold speech observed when a person is diagnosed with the common cold. The common cold is a viral infectious disease that affects the throat and the nose. Since speech is produced by the vocal tract after linear filtering of excitation source information, during a common cold, its attributes are impacted by the throat and the nose. The proposed study attempts to develop a deep learning-based classification model that can accurately predict whether a person has a cold or not based on their speech. The common cold-related information is captured using Mel-frequency cepstral coefficients (MFCC) and linear predictive coding (LPC) from the speech signal. The data imbalance is handled using the sampling strategy, SMOTE-Tomek links. Then, utilizing MFCC and LPC features, a deep learning-based model is trained and then used to categorize cold speech. The performance of a deep learning-based method is compared to logistic regression, random forest, and gradient boosted tree classifiers.     Abstract Healthcare providers as well as medical technologists lay a strong focus on clinical conditions for patient centric care delivery. Currently, the challenges are to (1) obtain a consolidated view of various stakeholders and pain points for the entire disease lifecycle, (2) identify interdependencies between different stages of the disease, and (3) prioritize solutions based on customer needs. A structured approach is required to address clinical needs across disease care plans tailored to different geographies and ethnicities. Innovation Think Tank (ITT) teams across multiple locations formed focus groups to elaborate the pathways of 22 global diseases, selected based on ranking of associated economic burden and threat to life. Ideation sessions were held to identify pain points and find innovative solutions. Additionally, inputs were taken from co-creation sessions at universities worldwide. The optimization and design of infographics and care plan was done based on the key information gathered—facts and figures, stakeholders, pain points and solutions. Finally, validation was obtained from clinical and technology experts globally. A disease pathway framework was created to develop pathways for 22 global diseases. Over 1,500 pain points were collected and about 1,900 ideas were proposed. The approach was applied to optimize its application to 30 product and portfolio definition projects over 2 years at Siemens Healthineers, as well as co-creation programs with universities and hospitals. The disease pathway framework provides a unique foundation for extensive collaboration among multiple stakeholders, through information sharing and delivering high-quality solutions based on the identified problems and customer needs.

    Abstract Background and objective: Findings on the association between testosterone and cardiovascular risk in men have been inconsistent. Previous studies investigated this association based on testosterone level measured at one single point in time (e.g., testosterone level at the baseline visit), which might not be relevant for the future cardiovascular events that occurred many years later, given the dynamic changes of testosterone level over time. Additionally, as compared to patients without prior cardiovascular history, those with prior cardiovascular events are more likely to have pathological changes in coronary or peripheral vasculature, and therefore more susceptible to cardiovascular events when exposed to risk factors. However, whether time-varying testosterone level affects differently between patients with prior cardiovascular events and those without is unknown. The objective of this study was to investigate the association between time-varying testosterone level and risks of myocardial infarction and stroke, and compared the associations stratified by patient’s prior cardiovascular event history. Material and methods: We used data of 376 hypogonadal men from a registry study in Germany with up to 11 years’ follow-up and applied Cox proportional hazards regression models to investigate the proposed association. Results: No association was found between baseline testosterone level and risks of myocardial infarction and stroke. When taking into account the changes of testosterone level over time and including testosterone as a time-varying covariate in the model, the adjusted hazard ratio (HR) for myocardial infarction associated with 1 nmol/L increase in testosterone level was 0.78 (95% CI: 0.64, 0.97); no association between time-varying testosterone level and stroke was found.     Abstract Like other fields of Traditional Medicines, Unani Medicines have been found as an effective medical practice for ages. It is still widely used in the subcontinent, particularly in Pakistan and India. However, Unani Medicines Practitioners are lacking modern IT applications in their everyday clinical practices. An Online Clinical Decision Support System may address this challenge to assist apprentice Unani Medicines practitioners in their diagnostic processes. The proposed system provides a web-based interface to enter the patient's symptoms, which are then automatically analyzed by our system to generate a list of probable diseases. The system allows practitioners to choose the most likely disease and inform patients about the associated treatment options remotely. The system consists of three modules: an Online Clinical Decision Support System, an Artificial Intelligence Inference Engine, and a comprehensive Unani Medicines Database. The system employs advanced AI techniques such as Decision Trees, Deep Learning, and Natural Language Processing. For system development, the project team used a technology stack that includes React, FastAPI, and MySQL. Data and functionality of the application is exposed using APIs for integration and extension with similar domain applications. The novelty of the project is that it addresses the challenge of diagnosing diseases accurately and efficiently in the context of Unani Medicines principles. By leveraging the power of technology, the proposed Clinical Decision Support System has the potential to ease access to healthcare services and information, reduce cost, boost practitioner and patient satisfaction, improve speed and accuracy of the diagnostic process, and provide effective treatments remotely. The application will be useful for Unani Medicines Practitioners, Patients, Government Drug Regulators, Software Developers, and Medical Researchers.

    Abstract Salinity stress poses a significant challenge to agriculture, impacting soil health, plant growth and contributing to greenhouse gas (GHG) emissions. In response to these intertwined challenges, the use of biochar and its nanoscale counterpart, nano-biochar, has gained increasing attention. This comprehensive review explores the heterogeneous role of biochar and nano-biochar in enhancing salt resilience in plants and soil while concurrently mitigating GHG emissions. The review discusses the effects of these amendments on soil physicochemical properties, improved water and nutrient uptake, reduced oxidative damage, enhanced growth and the alternation of soil microbial communities, enhance soil fertility and resilience. Furthermore, it examines their impact on plant growth, ion homeostasis, osmotic adjustment and plant stress tolerance, promoting plant development under salinity stress conditions.
