Abstract Introduction: Bedaquiline (BDQ), a diarylquinoline class of antimicrobial, is one of the latest anti-mycobacterial agents to be developed in several decades. Despite the drug being a great hope for the Drug Resistant Tuberculosis (DR-TB) patients, previous studies have raised alarm about BDQ-induced QT prolongations of serious clinical implication. Unfortunately, knowledge about adverse drug reaction of BDQ on Indian patients remains limited. Therefore, dedicated research focused on safety of BDQ in Indian population can provide valuable insight. Aim: To assess the short-term safety of BDQ on Indian DR-TB patients. Materials and Methods: This prospective observational study was conducted over a period of one year on 49 DR-TB patients under BDQ therapy. Data of all the DR-TB patients from the first 14 days of BDQ therapy were enrolled in the study. All adverse events during this period were closely observed and recorded. Electrocardiography (ECG) were recorded daily during this period. From the observed QT value, a ‘corrected QT’ (QTc) value was calculated using Fridericia’s formula (QTcF). Values above 440 ms were noted as prolonged QTcF and values >500 ms were given a special consideration. Results: Total 49 patients were recruited in the present study, with mean age of 38.63±1.63 years. A total of 124 reports of adverse events or symptoms were recorded during the 14 days in-hospital period. Nausea was the most commonly reported complaint (n=33) followed by headache (n=30) and arthralgia (n=28). A total of 278 observations of prolonged QTcF values (>440 ms) was noted out of 686 ECG recordings. The mean QTcF values among day 1, day 7 and day 14 showed statistically significant difference {p=0.01, 95% CI (Confidence Interval)}. Moreover, a mean increase of 14.2% was observed in the QTcF values between day 1 and day 14. There were a total of 69 observations of QTcF value more than 500 ms. The incidence of such value was maximum on day 14 (n=9). The QTcF values were found to follow three distinct trends: a) ...

Abstract Introduction: Ocular tuberculosis has diverse presentations. As microbiological confirmation is difficult, a high index of suspicion is required. The present study replenishes the gap of existing knowledge regarding ocular tuberculosis and facts related to antitubercular drug related toxicity. Aim: To evaluate the magnitude and outcome of ocular tuberculosis along with antitubercular drug related ocular toxicity, with special reference to ethambutol toxicity. Materials and Methods: This cohort study was conducted from May 2018 to April 2019 at Outpatient Department of Ophthalmology, Burdwan Medical College and Hospital, West Bengal, India. Total 170 newly diagnosed tuberculosis patients underwent comprehensive ophthalmic examination for any signs of ocular tuberculosis or development of antitubercular drug related ocular toxicity. They were treated and followed up to the completion of Anti-Tubercular Therapy (ATT). The statistical software Statistical Package for the Social Sciences (SPSS) version 20.0 was used for the analysis. Results: Out of total 170 study participants, 110 (64.7%) were male, mean age was 35.65±15.49 years. Ocular tuberculosis was found in 12 (7.05%) patients. Four patients had choroidal tuberculoma, two had Eales disease, four had choroiditis, two had anterior and intermediate uveitis. All the ocular tubercular lesions responded to ATT. Total 7 (4.1%) patients developed ethambutol-induced ocular toxicity. Following stoppage of ethambutol and treatment with corticosteroids there was significant improvement in colour vision, perimetry and Visual Evoked Potential (VEP) findings in 3 (42.9%) patients. Unfortunately, 4 (57.1%) patients developed optic atrophy. Conclusion: Comprehensive ophthalmic examination should to be done in all tuberculosis patients to detect and treat co-existing ocular tuberculosis as well as early detection of antitubercular drug toxicity. Withholding of the offender can result in visual recovery

Abstract Background: Silicosis is a slowly progressive chronic occupational lung disease, developed after a prolong period of exposure to high concentration of silica dust.
Methods: In this longitudinal study, we enrolled old and new silicosis patients (n=19; 8 jewellery polishers, 11 from other occupations) seen at our Pulmonary Medicine Department from June 2009 to December 2012 to document the course of illness as per their occupational exposure.
Results: Six of the eight jewellery polishing workers had developed silicosis within five years of exposure, while six of the 11 other workers with other occupational exposure had developed silicosis after exposure of 10 years or more. Mean duration of exposure was significantly less among jewellery polishing workers compared to other workers (3.4 +/- 1.7 versus 9.3 +/- 4.1; p=0.001). Mean duration of illness (months) (14.9 +/- 5.8 versus 28.5 +/- 16.5; p=0.040) were significantly less among the jewellery polishing workers compared to other workers. At the end of the study period, all eight jewellery polishing workers with silicosis had died while four of the 11 patients with other occupational exposure had died.
Conclusion: Silicosis among jewellery polishing workers was found to be more severe and progressive compared to silicosis due to other occupational exposures, in our study.
