    Abstract Objective Data regarding the use and effect of hormonal contraceptives on bone mass acquisition during adolescence are contradictory. The present study was designed to evaluate bone metabolism in two groups of healthy adolescents using combined oral contraceptives (COC). Methods A total of 168 adolescents were recruited from 2014 to 2020 in a non-randomized clinical trial and divided into three groups. The COC1 group used 20 μg Ethinylestradiol (EE)/150 μg Desogestrel and the COC2 group used 30 μg EE/3 mg Drospirenone over a period of two years. These groups were compared to a control group of adolescent non-COC users. The adolescents were submitted to bone densitometry by dual-energy X-ray absorptiometry and measurement of bone biomarkers, bone alkaline phosphatase (BAP), and osteocalcin (OC) at baseline and 24 months after inclusion in the study. The three groups studied were compared at the different time points by ANOVA, followed by Bonferroni’s multiple comparison test. Results Incorporation of bone mass was greater in non-users at all sites analyzed (4.85 g in lumbar Bone mineral content (BMC)) when compared to adolescents of the COC1 and COC2 groups, with a respective increase of 2.15 g and loss of 0.43g in lumbar BMC (P = 0.001). When comparing subtotal BMC, the control increased 100.83 g, COC 1 increased 21.46 g, and COC 2 presented a reduction of 1.47 g (P = 0.005). The values of bone markers after 24 months are similar for BAP, being 30.51 U/L (± 11.6) for the control group, 34.95 U/L (± 10.8) for COC1, and 30.29 U/L for COC 2 (± 11.5) (P = 0.377). However, when we analyzed OC, we observed for control, COC 1, and COC 2 groups, respectively, 13.59 ng/mL (± 7.3), 6.44 ng/mL (± 4.6), and 9.48 ng/mL (± 5.9), with P = 0.003. Despite loss to follow-up occurring in the three groups, there were no significant differences between the variables in adolescents at baseline who remained in the study during the 24-month follow-up and those who were excluded or lost to follow-up.     Abstract Abstract Obesity in adolescents has reached epidemic proportions and is associated with the inflammatory response and viral infections. The aim of this study was to understand the profile of inflammatory cytokines and chemokines associated with the inflammatory response and metabolic syndrome (MetS) in obese adolescents with positive serology for adenovirus 36 (ADV36). Thirty-six overweight, 36 obese, and 25 severe obesity adolescents aged 10 to 16 years were included in the study. The following variables were analyzed: sex, age, body mass index (BMI), blood pressure, total cholesterol and fractions, triglycerides, glucose, serum cytokine concentrations, and ADV36 antibodies. Cytokines and chemokines were quantified by cytometry and ADV36 serology was determined by enzyme-linked immunosorbent assay (ELISA). The results showed higher levels of the cytokines interleukin-1beta (IL-1β), IL-6, IL-10 and of the chemokine interferon-gamma-inducible protein 10 (IP-10) in severe obesity adolescents compared to the obese and overweight groups, as well as in the group with MetS compared to the group without this syndrome. The frequency of ADV36-positive individuals did not differ between groups. The findings revealed differences in BMI between the obese and severe obesity groups versus the overweight group in the presence of positivity for ADV36, suggesting an association with weight gain and possibly MetS installation.

    Abstract Introduction Osteoporosis and metabolic syndrome (MetS) are diseases that have serious public health consequences, reducing the quality of life of patients and increasing morbidity and mortality, with substantial healthcare expenditures. Objective To evaluate the impact of MetS on bone mineral density (BMD) and biochemical markers of bone formation and resorption in adolescents with excess weight. Method A descriptive and analytical cross-sectional study was performed that evaluated 271 adolescents of both sexes (10 to 16 years). From the total sample, 42 adolescents with excess weight and the presence of MetS (14%) were selected. A further 42 adolescents with excess weight and without MetS were chosen, matched for chronological age, bone age, and pubertal developmental criteria to those with MetS, for each sex. Anthropometric measurements, blood pressure collection, and biochemical tests were performed in all adolescents, as well as evaluation of BMD and the bone biomarkers osteocalcin (OC), bone alkaline phosphatase (BAP), and carboxy-terminal telopeptide (S-CTx). Results The adolescents with excess weight and MetS exhibited significantly lower transformed BMD and concentrations of BAP, OC, and S-CTx compared to the matched group, except for OC in boys. A negative and significant correlation was observed between total body BMD and BAP (r = -0.55568; p = 0.005), OC (r = -0.81760; p = < .000), and S-CTx (r = -0.53838; p = 0.011) in girls. Conclusion Metabolic syndrome may be associated with reduced bone mineral density and biochemical markers of bone formation and resorption in adolescents with excess weight.

    Abstract Obesity and osteoporosis are important global health problems characterized by increasing prevalence with high impact on morbidity and mortality. The objective of this review was to determine whether excess weight during adolescence interferes with bone mass accumulation. If bone mineral gain can be optimized during puberty, adults are less likely to suffer from the devastating complications of osteoporosis. The increased fracture risk in obese children has also been attributed to a lower bone mass for weight compared to non-obese children. Thus, adiposity present in this age group may not result in the protection of bone mass, in contrast to what has been observed in adults. However, studies involving adolescents have reported both protective and detrimental effects of obesity on bone. The results and mechanisms of these interactions are controversial and have not been fully elucidated, a fact highlighting the extreme relevance of this topic and the need to monitor intervening and interactive variables.

    Abstract The aim of this study was to investigate the effects of excess body fat on bone mass in overweight, obese, and extremely obese adolescents.This study included 377 adolescents of both sexes, ages 10 to 19 y. Weight, height, body mass index (BMI), bone age, bone mineral content (BMC), and bone mineral density (BMD) were obtained by dual-energy x-ray absorptiometry. The results were adjusted for chronological age and bone age. Comparisons according to nutritional classification were performed by analysis of variance, followed by Tukey test. Linear regression models were used to explain the variation in BMD and BMC in the L1–L4 lumbar spinal region, proximal femur, and whole body in relation to BMI, lean mass, fat mass (FM), and body fat percentage (BF%), considering P < 0.05.For all nutritional groups, average bone age was higher than chronological age. In both sexes, weight and BMI values increased from eutrophic to extremely obese groups, except for BMD and BMC, which did not differ among male adolescents, and were smaller in extremely obese than in obese female adolescents (P < 0.01). Significant differences were observed for FM and BF% values among all nutritional groups (P < 0.01). Positive, moderate to strong correlations were detected between BMD and BMC for BMI, lean mass, and FM. A negative and moderate correlation was found between BMC and BF%, and between BMD and BF% at all bone sites analyzed in males and between BF% and spine and femur BMD, in females.The results reveal a negative effect of BF% on bone mass in males and indicate that the higher the BF% among overweight adolescents, the lower the BMD and BMC values.

    Abstract O presente estudo teve como objetivo investigar o processo de abortamento na adolescência, segundo a perspectiva da adolescente. Foi desenvolvido no Hospital Maternidade Santa Izabel, da cidade de Bauru (São Paulo, Brasil), e a metodologia qualitativa buscou compreender a experiência da adolescente no processo de abortamento, construindo um modelo teórico representativo. Utilizou-se como referencial teórico o interacionismo simbólico e como metodológico a grounded theory. Foram entrevistadas dez adolescentes, e a análise dos relatos constatou a descoberta da gravidez, os sentimentos emergentes, o difícil momento da internação, a esperança do retorno ao cotidiano acompanhado pelo apoio dos familiares e a necessidade de mudanças decorrentes de tal experiência. Dos depoimentos emergiram três fenômenos: descobrindo-se grávida, vivenciando o abortamento e reelaborando o plano de vida. A partir da análise do processo, desenvolveu-se a categoria central: "Abortamento na adolescência: da vida à experiência do colo vazio". Conclusão: a ocorrência do abortamento na adolescência causa diferentes sentimentos, podendo atingir as esferas biopsicossociais. The present study aimed to investigate the process of abortion in the adolescence, according to adolescence perspective at Santa Izabel Maternity Hospital, in the city of Bauru - São Paulo, Brazil. The qualitative methodology tried to understand the adolescent's experience on the abortion process, building a theoretical sample that represents it. Symbolic interacionism was used as referential theory and grounded theory as methodological reference. Ten adolescents were interviewed and the analyses of the reports noticed the discovered of pregnancy, emergent feelings, the difficult moment of hospitalization, the hope for a return to quotidian followed by relatives help and the need of changes because of this experience. From the reports three phenomena were observed: find yourself pregnant; living the abortion and remaking of the life plan. From the analyses of the process it was developed the central category: "Abortion on adolescence: from life to the experience of empty lap". The occurrence of abortion in the adolescence causes different feelings and may reach bio psychosocial circles.

    Abstract A puberdade destaca-se como período fundamental para a aquisição de massa óssea. Durante essa fase da vida, a mineralização encontra-se aumentada com taxas de formação óssea superior às de reabsorção. Nesse sentido, o objetivo desta revisão foi investigar a inter-relação da ingestão dietética de cálcio com a mineralização óssea, durante a puberdade. Entre os fatores influenciadores nutricionais, merecem destaque o fosfato e o magnésio, que, juntamente com o cálcio, mantêm a integridade estrutural do esqueleto. A revisão de literatura indicou que os excessos de proteína e de refrigerantes, na alimentação dos jovens, acarretam comprometimento na mineralização óssea, embora os dados sejam ainda contraditórios. Conclui-se que, durante o período da puberdade, é indicado manter o aporte de cálcio em níveis adequados, na perspectiva de maximizar o pico da massa óssea. The puberty is distinguished as the main period for bone mass acquisition. During this phase of the life, the mineralization finds an increase with higher taxes of bone formation compared to re-absorption cycle. In this direction, the objective of this revision was an investigation of the interrelation between the dietary calcium ingestion and bone mineralization during the puberty. Among nutritional factors, deserve prominence phosphate and magnesium, that, jointly with calcium keep the structural integrity of the skeleton. The literature revision indicated that the excesses of protein and carbonated soft drinks in the feeding of the young people cause a jeopardizing in the bone mineralization, even so the data is still contradictory. In conclusion, during the period of the puberty, is indicated to keep the amount of calcium in adjusted levels, with the perspective of bone mass peak maximization.

    Abstract A sociedade atual tem valorizado de forma significativa a aparência alta e esbelta. Essa constituição física tem sido reforçada desde a infância e atinge a população adolescente, que deseja enquadrar-se nos estereótipos, particularmente aqueles veiculados pela mídia. Procurou-se revisar a literatura especializada a respeito dos principais efeitos que o exercício físico exerceria sobre a secreção e atuação do hormônio de crescimento (GH) nos diversos tecidos corporais, durante a infância e adolescência. Através dessa revisão, foi possível verificar que o exercício físico induz a estimulação do eixo GH/IGF-1. Embora muito se especule quanto ao crescimento ósseo ser potencializado pela prática de exercícios físicos, não foram encontrados na literatura científica específica estudos bem desenvolvidos que forneçam sustentação a essa afirmação. No tocante aos efeitos adversos advindos do treinamento físico durante a infância e adolescência, aparentemente, esses foram independentes do tipo de esporte praticado, porém resultantes da intensidade do treinamento. A alta intensidade do treinamento parece ocasionar uma modulação metabólica importante, com a elevação de marcadores inflamatórios e a supressão do eixo GH/IGF-1. Entretanto, é importante ressaltar que a própria seleção esportiva, em algumas modalidades, recruta crianças e/ou adolescentes com perfis de menor estatura, como estratégia para obtenção de melhores resultados, em função da facilidade mecânica dos movimentos. Através dessa revisão, fica evidente a necessidade de realização de estudos longitudinais, nos quais os sujeitos sejam acompanhados antes, durante e após sua inserção nas atividades esportivas, com determinação do volume e da intensidade dos treinamentos, para que conclusões definitivas relativas aos efeitos sobre a estatura final possam ser emanadas. La sociedad actual tiene valorado significativamente la apariencia alta y esbelta. Esta constitución física ha sido reforzada desde la infancia e involucra a la población adolescente que desea encuandrar en estos estereotipos, particularmente en aquellos vínculados a la Media. En este sentido, los profesionales de la salud son continuamente consultados sobre los efectos positivos que el ejercicio físico ejerce sobre el crecimiento longitudinal de los niños y de los adolescentes. Se procura así revisar la literatura respectos a los dos principales efectos que el ejercicio físico ejercería sobre la secreción y la actuación de la hormona de crecimiento (GH) en los diversos tejidos corporales durante la infancia y la adolescencia. A traves de esta revision fue posible verificar que el ejercicio fisico induce una estimulación del eje GH/IGF-1. Ahora bien, lo mucho se especula en cuanto al crecimiento óseo pueda ser potencializado por la práctica de ejercicios físicos no ha sido encontrado en la literatura cientifica especifica estudios bien desarrollados que fortalezcan esta presunción y la afirmen. No obstante a los efectos adversos resultantes del entrenamiento físico durante la infancia y la adolescencia aparentemente este fue independiente del tipo de deporte practicado, y pueden resultar de la intensidad del entrenamiento. Una alta intensidad de entrenamiento parece ocasionar una modulación metabólica importante, como es la elevación de los marcadores inflamatorios y la supresión del eje GH/IGF-1. Entretanto, es importante resaltar que la propia seleción deportiva, en algunas modalidades, recluta niños y/o adolescentes con perfiles de menor estatura, como estrategia para la obtención de mejores resultados, en función de la facilidad mecánica de los movimientos. A través de esta revisión, queda evidente la necesidad de la realización de estudios longitudinales, donde los sujetos sean acompañados antes, durante y después de su inserción en las actividades deportivas, con determinación del volumen y de la intensidad de los entrenamientos, para que las conclusiones definitivas relativas a los efectos sobre la estatura final, puedan ser definidas. Modern society has given a lot of value to being slim and tall. This physical structure has been reinforced since childhood and has been affecting the adolescents desire to fit in this stereotype, especially the ones shown in the media. Because of this, health professionals are frequently questioned about the positive effects that physical exercises perform on children and adolescent's linear growth. Through the specialized literature, this article intended to illustrate the main effects that physical exercise would perform on the secretion of growth hormone (GH) in the several body tissues during childhood and adolescence. Through this reviewing, it was possible to verify that physical exercises induce the stimulation of the GH/IGF-1 axis. Although much is speculated with regard to physical exercise and bone growth, no well-developed studies were found in the specific scientific literature to corroborate this statement. The adverse physical practicing effects during childhood and adolescence, apparently, were independent of the type of the exercise practice, but else a result of the intensity. The high training intensity seems to cause an important metabolic modulation with raise of the inflammatory markers and the suppression of the GH/IGF-1 axis. However, it is important to emphasize that the sportive modality itself selects children and/or adolescents with lower stature as strategy for the attainment of better results in function of the mechanical facility of movements. Through this research, the performance of linear studies is evident, where individuals are followed before, during, and after exercise, with the determination of the training volume and intensity in order to reach definitive conclusions with regard to the effects on the final stature.

    Abstract A adolescência é um período fundamental para a aquisição da massa óssea. Em adolescentes atletas, o pico de massa óssea pode apresentar maior incremento, em virtude do estresse mecânico imposto aos ossos pelo exercício físico praticado. O objetivo desta revisão foi investigar o papel do treinamento esportivo vigoroso e precoce sobre a saúde óssea de atletas adolescentes. Através da revisão da literatura científica, envolvendo adolescentes atletas de diferentes modalidades e de ambos os sexos, é possível inferir que a densidade mineral óssea é potencializada pelos exercícios, quando grupos de atletas são comparados com grupos de controle. Entretanto, muito se discute na literatura quanto à recomendação da intensidade adequada da prescrição de exercício físico para população adolescente, uma vez que, caso o treinamento se torne muito extenuante, os benefícios gerados pela atividade sobre a saúde dos ossos podem ser minimizados ou anulados. Embora muita controvérsia ainda envolva o tema, independente do tipo de esporte praticado, o aumento de intensidade do treinamento deve ser razoável e coerente com as metas, sendo enfatizado treinamento seguro e eficaz para cada uma das faixas de idade e momentos da maturação biológica, independente dos calendários competitivos. En los adolescentes atletas, el pico de masa óssea puede presentar un mayor incremento, en virtud del estres mecánico impuesto a los huesos por el ejercício físico practicado. El objetivo de esta revisión fué investigar el papel del entrenamiento deportivo vigoroso e precoz sobre la salud ósea de los atletas adolescentes. A través de esta revisión de la literatura científica, incluyendo adolescentes atletas de diferentes modalidades y de ambos sexos, es posible inferir que a la densidad mineral ósea es potencializada por los ejercícios, cuando los grupos de atletas son comparados con grupos control. Entretanto, mucho se discute en la literatura en cuanto a la recomendación de la intensidad adecuada de la prescripción de ejercício físico para la población adolescente, una vez que, en caso de que el entrenamiento se torne muy extenuante, los benefícios generados por la actividad sobre la salud de los huesos pueden ser minimizados o anulados. Mismo que mucha controversia exista en torno al tema, independiente del tipo de deporte practicado, el aumento de intensidad del entrenamiento debe ser razonable y coherente con las metas, siendo enfatizado el entrenamiento seguro y eficaz para cada una de las franjas de edad y los momentos de maduración biológica, independientemente de los calendarios deportivos. Adolescence is a fundamental period to gain bone mass. In athletic adolescents, the maximum of bone mass can be achieved at this time. It occurs because of the stress on the bones resulted from physical exercises. The aim of this revision was to investigate the strenuous and premature exercise practice on the bone health on athletic adolescents. Through several research sessions with adolescent athletes of different gender and practicing different sports, it is possible to conclude that the mineral osseous density is potentialized as a result of exercise, when compared to control groups. However, there are numerous discussions as to the adequate intensity of physical exercise for teenagers because, in the case of strenuous training, the advantages to the bones health will be minimal or even lost. Although there is a great deal of controversy on this topic, independent of sports modality, the increase on intensity during practice should be consistent and reasonable with the goal. For different ages and biological levels, safe and efficacious exercises, should be emphasized, independent of the sports calendar.
