    Abstract Patients with a fused hip can present with various clinical problems, such as contralateral hip arthritis, ipsilateral knee arthritis and degenerative disc disease. There are significant deforming forces at the proximal femur in patients with a fused hip who have ipsilateral subtrochanteric fractures. We report a 66-year-old patient with a spontaneously fused right hip, who sustained an ipsilateral comminuted proximal femoral fracture secondary to a road traffic accident. Optimal operative fixation of this patient posed as a surgical challenge. We discuss the management options in this paper. Intramedullary nailing was used so as to minimise the blood loss and because of its favourable biomechanical characteristics. The patient had postoperative femoral nerve neuropraxia that eventually resolved. In patients with a fused hip who have ipsilateral subtrochanteric fractures, retrograde nailing of such fractures can serve as an optimal treatment option. However, femoral nerve palsy can be a potential complication.

    Abstract This study was focused on investigating the optimum fluidising velocity during the combustion of rice husk in a bench-scale fluidised bed combustor (ID 210mm) to obtain low carbon ash in the amorphous form. When all other parameters are held constant, the optimum fluidizing velocity aids in almost complete combustion, thereby releasing the entrapped carbon for further conversion. This results in ash with consistently low carbon content (less than 2wt%). The range of fluidising velocities investigated was from as low as 1.5U(mf) to as high as 8U(mf). It was found that the optimum fluidising velocity was approximately 3.3U(mf) as the mixing of rice husk with the bed was good with a high degree of penetration into the sand bed. The resulting ash retained its amorphous form with low residual carbon content (at 2.88wt%) and minimal sand contamination as shown by the X-ray diffraction analysis.

    Abstract In two boys aged 13 and 6 years respectively obstipation and a paradoxical diarrhoea developed four days after consumption of unshelled sunflower seeds. The cause was impaction of the unshelled sunflower seeds in the rectum. Both patients required general anaesthesia to remove the sunflower seeds via proctoscopy. It is known that eating unshelled sunflower seeds can cause impaction of the shells in the rectum (causing a bezoar). Removing the seeds under general anaesthesia is practically always necessary. Considering the serious symptoms and the invasive consequences which are not without risks, parents and children should be warned about the danger of eating unshelled sunflower seeds.

    Abstract Background: Among the various operations used for surgical treatment of morbid obesity, adjustable silicone gastric banding (ASGB) is the least invasive. Many good results have been described. During extended follow-up, however, serious complications may occur. We briefly describe our results with ASGB and will focus on three cases of band erosion.
    Methods: From January 1996 to December 1998, 91 patients underwent laparoscopic adjustable gastric banding in our clinic. Follow-up until now is 100%.
    Results: Body Mass Index (BMI) in this series decreased from 44.7 at time of operation to 34.8 at 18 months of follow-up (42 patients). Complications, minor and major, occurred in 27.5%. Three patients are described in which the gastric band migrated and had to be removed operatively.
    Conclusions: Satisfactory weight loss can be established by ASGB. However, serious and potentially lethal complications can occur. In view of the former Angelchik esophageal antireflux prosthesis, abandoned because of its notorious migration, we must be aggressive in evaluating band migration. Thus, we plead for international registration of adjustable silicone gastric banding.
