    Abstract Vitamin D deficiency increases the risk of adverse pregnancy outcomes. The high prevalence of vitamin D deficiency worldwide requires the analysis of scientific publications to make recommendations for the prevention, management and treatment of vitamin D deficiency in pregnant women. The purpose of the research was to explore the relationship between pathology and pregnancy outcomes with serum 25-hydroxycholecalciferol levels and vitamin D supplementation. A literature search was performed for systematic literature reviews with meta-analyses published between January 2018 and February 2023. 42 publications were selected for further analysis. 24 meta-analyses evaluated associations between serum 25-hydroxycholecalciferol levels on the one hand and the course and outcome of pregnancy on the other. 18 meta-analyses focused on the preventive effect of vitamin D preparations on the development of obstetric and paediatric pathology. Vitamin D deficiency was identified to increase the probability of spontaneous abortion (OR=1.6 (1.11; 2.3), preterm labour (OR=1.33 (1.15; 1.54)), especially in multiple pregnancies (OR=2.59 (1.35; 4.95)), pre-eclampsia (OR 1.58 (1.39-1.79)), anaemia of pregnancy (OR=1.61 (1.41; 1.83), postpartum depression (OR=3.67 (1.72; 7.85), autism spectrum disorders in early childhood (OR=1.54 (1.12; 2.1). High vitamin D levels reduce the risk of gestational diabetes (RR=0.87 (0.79; 0.97)), and low birth weight (RR=0.65 (0.48; 0.86)). The target level of 25-hydroxyvitamin D during pregnancy is a serum concentration of more than 30 mg/ml. Vitamin D supplementation during pregnancy at a dose of 2000 IU or higher is preventive for pre-eclampsia, insulin resistance, and the development of bronchial asthma in early childhood. Vitamin D screening is indicated for all pregnant women. Dosages of vitamin D preparations should be determined individually, considering laboratory tests and risk factors.

    Abstract Objective: The purpose was to evaluate the effectiveness of a non-invasive prenatal test (NIPT) using mass parallel sequencing (MPS) to detect trisomy 13, 18, 21 and fetal sex chromosome abnormalities in maternal blood samples by isolating freely circulating foetal extracellular DNA (eDNA), and to develop an algorithm for prenatal screening.
    Material and methods: The research methods used included blood sampling from patients, isolation of eDNA, determination of DNA concentration and quality, library preparation for sequencing, MPS using an Illumina HiSeq2000, positive and negative control samples, monitoring, and analysis of results using the distributed algorithms platform based on calculations of z-value and the average absolute deviation. Pregnant women were divided into two groups based on gestational age at sampling, group 1; 9-14 weeks and group 2; 15-27 weeks.
    Results: A total of 377 pregnant women were included with a mean (range) age of 33 (23-44) years. The mean gestational age at the time of blood sampling in group 1 was 11 (9-14) weeks, and in group 2 was 21 (15-27) weeks. In the first group, three cases of trisomy 18 chromosomes were detected in patients aged 43 years old, and female children were subsequently born with Edwards syndrome. In the second group, one case of trisomy 21 was detected in a patient aged 36 years and the pregnancy was terminated at 25 weeks.
    Conclusion: The analysis of freely circulating foetal eDNA was a sensitive method for detecting chromosomal abnormalities. The study has a practical significance, since the NIPT for frequent aneuploidy considerably exceeds the effectiveness of traditional screening methods and allows identifying chromosomal disorders starting from the 9th week of the gestation period.

    Abstract Background: Vitamin D is involved in numerous reactions in the human body, as well as in the regulation of about 3% of the human genome. Research shows that insufficient concentration of vitamin D in the blood is found in 40-50% of the population. The total intake of vitamin D through food is very low, only 20% of the total daily needs. This paper shows importance of vitamin D supplementation in different periods of woman's life.
    Objective: To estimate average plasma levels of vitamin D3 in all researched group and establish which group should be supplemented with vitamin D3.
    Methods: The study included 182 women, divided in appropriate groups according to age, who came regularly for annual gynecological check-ups. Total concentration of 25 (OH) vitamin D in plasma was determined by a competitive immunochemical test.
    Results: The average value of vitamin D3 plasma level in patients aged 18 to 45 was 24,409 ng/ml, in the patients aged 46 to 55 years was 27,519 ng/ml and in the patients aged 56 and over 31,960 ng/ml. The results obtained indicate a vitamin D shortage in the first two groups of patients and an adequate vitamin D3 levels in third group of patients. Vitamin D supplementation is required in patients aged 18 to 55 years.
    Conclusion: Vitamin D shortage is present in two groups of patients. Because of complex role that this vitamin plays in numerous reactions in human body adequate supplementation is necessary.

    Abstract Background: Uncontrolled type 2 diabetes mellitus (T2DM) is widely recognized as a significant risk factor for the emergence of cardiovascular events. Early risk assessment, especially for coronary artery disease, is crucial to starting therapeutic strategies to lower cardiovascular risk.
    Objective: To assess cardiovascular risk in patients with type 2 diabetes mellitus.
    Methods: 64 patients were divided into groups according to age, gender, disease duration, glucoregulation, and applied treatment. The SCORE table was used to quantify cardiovascular risk.
    Results: Our research showed that cardiovascular risk in patients with diabetes mellitus increases with age (rho = 0.458; p = 0.0001; p <0.05) and is higher in men (rho = -0.417; p <0,0001). It has been found that patients with a longer duration of diabetes mellitus have a higher cardiovascular risk (rho = 0.266; p = 0.032). Patients with better glucoregulation had a slightly lower cardiovascular risk, but correlation was statistically insignificant. No statistically significant correlation was observed between applied therapy and cardiovascular risk.
    Conclusion: Good control and treatment of T2DM is of crucial importance for reducing cardiovascular risk.

    Abstract Background: Uterine Artery Embolization (UAE) and Magnetic Resonance guided High Intensity Focused Ultrasound (MRgHIFU) are two noninvasive treatments for uterine leiomyoma.
    Methods: This systematic review, following PRISMA guidelines, analyzed the effectiveness of two treatments by comparing percent fibroid volume shrinkage immediately after the procedure and after 3, 6, 12 and 24 months of follow-up and also assessed and compared common complications following treatment. The search utilized Science Direct, PubMed, MEDLINE, Google Scholar and BioMed Central databases, selecting manuscripts published during the period 2000 and 2020. Studies with premenopausal patients with previous treatments for uterine leiomyoma and/or with other pelvic diseases were excluded.
    Results: Twenty-nine papers satisfied inclusion and exclusion criteria. Results were pooled and stratified by treatment and follow-up time. Weighted fibroid volume percent shrinkage after UAE was statistically significantly greater than MRgHIFU at 6, 12, and 24 months follow-up times. However, UAE had statistically significantly more complications, such as pain, nausea and vomiting. However, this study cannot conclude that UAE is more effective than MRgHIFU due to confounding factors.

    Abstract Endometriosis is a heterogeneous, complex, and still challenging disease, due to its epidemiological, etiological and pathogenic, diagnostic, therapeutic, and prognosis characteristics. The classification of endometriosis is contentious, and existing therapies show significant variability in their effectiveness. This study aims to capture and describe clusters of women with endometriosis based on their comorbidity. With data extracted from electronic records of primary care, this study performs a hierarchical clustering with the Ward method of women with endometriosis with a subsequent analysis of the distribution of comorbidities. Data were available for 4055 women with endometriosis, and six clusters of women were identified: cluster 1 (less comorbidity), cluster 2 (anxiety and musculoskeletal disorders), cluster 3 (type 1 allergy or immediate hypersensitivity); cluster 4 (multiple morbidities); cluster 5 (anemia and infertility); and cluster 6 (headache and migraine). Clustering aggregates similar units into similar clusters, partitioning dissimilar objects into other clusters at a progressively finer granularity-in this case, groups of women with similarities in their comorbidities. Clusters may provide a deeper insight into the multidimensionality of endometriosis and may represent diverse "endometriosis trajectories" which may be associated with specific molecular and biochemical mechanisms. Comorbidity-based clusters may be important to the scientific study of endometriosis, contributing to the clarification of its clinical complexity and variability. An awareness of those comorbidities may help elucidate the etiopathogenesis and facilitate the accurate earlier diagnosis and initiation of treatments targeted toward particular subgroups.

    Abstract Infertility is a problem that affects millions of couples worldwide and has a significant impact on their quality of life. The recently introduced "Fertility Quality of Life Questionnaire (FertiQoL)" quickly became a gold standard for evaluation of the quality of life of patients suffering from infertility. The aim of this study was to determine the quality of life of Kazakhstani women coping with infertility problems by FertiQoL and assess the validity of the questionnaire. This cross-sectional study involved women of reproductive age undergoing an in vitro fertilization (IVF) cycle at a large IVF center in Kazakhstan in the period from 1 September 2020 to 31 September 2021. A total of 453 women out of 500 agreed to participate in the study, and the response rate was 90.6%. The overall Core FertiQoL was 56.95 ± 14.05, and the Treatment FertiQoL was 66.18 ± 11.13 points.     Abstract The Cucurbita genus has been widely used in traditional medicinal systems across different countries. In this study, we aimed to investigate the chemical composition, antioxidant properties, enzyme inhibitory, and cytotoxic effects of methanol and aqueous extracts obtained from the aerial parts, seeds, and fruit shells of Cucurbita okeechobeensis. Antioxidant properties were assessed using various chemical methods, including radical quenching (DPPH and ABTS), reducing power (CUPRAC and FRAP), metal chelation, and phosphomolybdenum assays. The extracts' enzyme inhibitory effects were tested against cholinesterase, amylase, glucosidase, and tyrosinase, whereas different cancer cell lines were used for the cytotoxicity study.     Abstract Objectives. Abnormal uterine bleeding (AUB) is a common complaint of women in different age groups, and endometrial biopsy is widely used to investigate the underlying causes. The aim of this observational study was to assess factors influencing pain in patients undergoing endometrial biopsy for AUB. Methods. Pain intensity before, during, and after Pipelle sampling was evaluated using the numerical rating scale (NRS), where “0” represents no pain at all, “10”—the worst pain ever possible. Pain rating was categorized as 1−6—mild to moderate, 7 and above as severe pain. Results. The study included 160 women who underwent Pipelle biopsy. The median age in the cohort was 42 (34−48) years, 18.1% of women were postmenopausal, 56.3% were either overweight or obese, 30% were nulliparous and 80% reported urban residency. The median pain score during the procedure was 2 (0−4). Pain scores of 5 (4−7) were reported with the junior gynecologist and 2 (0−4) in the senior gynecologist (p < 0.0001). Conclusion. The pain was found to have a strong association with the type of provider performing the endometrial sampling procedure. This fact suggests the need for a personalized approach and that psychological or informational interventions should be scheduled before the procedure to decrease pain and increase satisfaction.
