    Abstract Background and objective: Early detection and grading of Diabetic Retinopathy (DR) is essential to determine an adequate treatment and prevent severe vision loss. However, the manual analysis of fundus images is time consuming and DR screening programs are challenged by the availability of human graders. Current automatic approaches for DR grading attempt the joint detection of all signs at the same time. However, the classification can be optimized if red lesions and bright lesions are independently processed since the task gets divided and simplified. Furthermore, clinicians would greatly benefit from explainable artificial intelligence (XAI) to support the automatic model predictions, especially when the type of lesion is specified. As a novelty, we propose an end-to-end deep learning framework for automatic DR grading (5 severity degrees) based on separating the attention of the dark structures from the bright structures of the retina. As the main contribution, this approach allowed us to generate independent interpretable attention maps for red lesions, such as microaneurysms and hemorrhages, and bright lesions, such as hard exudates, while using image-level labels only.
    Methods: Our approach is based on a novel attention mechanism which focuses separately on the dark and the bright structures of the retina by performing a previous image decomposition. This mechanism can be seen as a XAI approach which generates independent attention maps for red lesions and bright lesions. The framework includes an image quality assessment stage and deep learning-related techniques, such as data augmentation, transfer learning and fine-tuning. We used the architecture Xception as a feature extractor and the focal loss function to deal with data imbalance.
    Results: The Kaggle DR detection dataset was used for method development and validation. The proposed approach achieved 83.7 % accuracy and a Quadratic Weighted Kappa of 0.78 to classify DR among 5 severity degrees, which outperforms several state-of-the-art approaches. Nevertheless, the main result of this work is the generated attention maps, which reveal the pathological regions on the image distinguishing the red lesions and the bright lesions. These maps provide explainability to the model predictions.
    Conclusions: Our results suggest that our framework is effective to automatically grade DR. The separate attention approach has proven useful for optimizing the classification. On top of that, the obtained attention maps facilitate visual interpretation for clinicians. Therefore, the proposed method could be a diagnostic aid for the early detection and grading of DR.

    Abstract Background: The purpose of this study was to evaluate the long-term oncological progression pattern of locally advanced rectal cancer patients with post-neoadjuvant nodal metastatic disease (ypN+) and correlate potential prognostic features associated with proven radiochemoresistant nodal biology.
    Methods: Individual patient data (100 variables) from a 20-year consecutive single-institution multidisciplinary experience (1995-2015), delivering multimodal therapy to rectal cancer patient candidates for radical treatment, including a neoadjuvant component and surgical resection with or without intraoperative radiotherapy followed by optional adjuvant chemotherapy. The ypN+ disease data was registered in the context of initial staging categories post-neoadjuvant T status (ypT).
    Results: Data on 487 patients showed histologically confirmed diagnoses of metastatic nodal disease in 108 specimens (ypN+, 22.1). There was a significant age difference (
    Conclusions: The presence of nodal metastases (ypN+) after neoadjuvant therapy containing long-course pelvic irradiation severely impacts the long-term outcome for patients with locally advanced rectal cancer and correlates with multiple clinical and therapeutic variable metrics. Implementation of local and systemic therapies should be adapted and intensified in relation to the finding of ypN+ category in surgical specimens.

    Abstract Pregnant women are especially vulnerable to respiratory diseases. We aimed to study seroconversion rates during pregnancy in a cohort of consecutive pregnancies tested in the first and third trimesters and to compare the maternal and obstetric complications in the women who seroconverted in the first trimester and those who did so in the third. This was an observational cohort study carried out at the Hospital Universitario de Torrejón, in Madrid, Spain, during the first peak of the COVID-19 pandemic. All consecutive singleton pregnancies with a viable fetus attending their 11-13-week scan between 1 January and 15 May 2020 were included and seropositive women for SARS-CoV2 were monthly follow up until delivery. Antibodies against SARS-CoV-2 (IgA and IgG) were analyzed on stored serum samples obtained from first- and third-trimester routine antenatal bloods in 470 pregnant women. Antibodies against SARS-CoV-2 were detected in 31 (6.6%) women in the first trimester and in 66 (14.0%) in the third trimester, including 48 (10.2%) that were negative in the first trimester (seroconversion during pregnancy).     Title translation Prevalence of analgesic consumption in the northern of Portugal.
    Abstract Introduction: Analgesic drugs are frequently used in developed countries and their in take seems to be rising, despite the associated risks. Counselling about the appropriate and safe use of this medication should be based on the characterization of their consumption. The goal of this study was to evaluate the prevalence and to characterize analgesic consumption patterns in northern Portugal, analysing related socio demographic factors.
    Methods: This multicentric, observational, transversal and analytic study took place between May 2005 and May 2007, based on the lists of 45 family doctors in northern Portugal. The authors used a simple randomised sample of 4500 patients who answered a personal questionnaire. The chi-square test was used to compare proportions and the T student test and ANOVA were used to compare distributions. A significance level of 0.05 was adopted. In order to control possible confounders, a multivariate analysis was made.
    Results: The questionnaire was answered by 3492 adults; 30,8% (n=1077, IC 95% 29,3-32,4) of them had taken analgesics in the last fifteen days. Non steroidal anti-inflammatory drugs (NSAID) and salicilates were the most frequently consumed (66%), followed by non opioids (46,6%). Opioids were consumed by 2,7% of the analgesic users, of which the most prevalent pattern of use was the "frequent consumption" (more than three ti mes per week). Most of the patients with a "frequent consumption" pattern used them for more than three months. Age and gender were in dependent risk factors for analgesic in take; women took three times more analgesics than men and consumption rose 1,02 ti mes per year.
    Discussion: The methodological variability found between this and other studies limited the comparison of our results with theirs. The only study with similar methods was performed in Sweden and found a higher prevalence (35%) of analgesic intake. In studies that only evaluated NSAID consumption, the results were similar to those of this study. Most of the stu dies were concordant about the higher analgesic in take by women, but not with its rise with age.
