Abstract Context: The world is aging rapidly through “demographic transition.” The aging leads to CI Refers as Cognitive Impairment (CI) – a risk factor for dementia. A community-based cross-sectional study was conducted among the elderly aged ≥50 years. Objectives: The objective of the study is to estimate the prevalence of cognitive dysfunction among the elderly and to identify the associated sociodemographic variables. Settings and Design: This was a community-based cross-sectional study in rural and urban field areas of a medical college. Subjects and Methods: Participants aged ≥50 years were interviewed using a structured questionnaire and screened for cognitive dysfunction using Gujarati version of Mini-Mental Status Examination. A score of 23 out of 30 was taken as the cutoff. Written informed consent was obtained from participants. Statistical Analysis Used: Univariate and multivariate analyses were done using SPSS version 17 and Epi Info version 6 to identify significant variables. Results: Of 560 participants, 140 (25%) had CI. Rural (27.6%) and female (29.8%) prevalence was higher than urban (18.5%) and male (19.1%) prevalence. On multivariate analysis, age ≥60 years (odds ratio [OR]: 2.98) and illiteracy (OR: 39.8) had significant positive association with outcome; being employed (OR: 0.18), living with spouse (OR: 0.07), and living with spouse and children (OR: 0.08) had significant negative association with outcome. Conclusions: CI – a precursor of dementia – has serious clinical and public health consequences. Awareness generation and capacity building of primary health-care workers and family caregivers are core control strategies.

Abstract Introduction: Kangaroo Mother Care (KMC) and optimal feeding in Low Birth Weight (LBW) infants are evidence based cost effective interventions which if properly implemented across all facilities can help to save millions of newborn. The study, focussing on status of KMC services in Gujarat State, has not been conducted so far. Objectives: The intent of present study was to ascertain the infrastructure, facilities, policies and practices related to Kangaroo Mother Care at special newborn care units (SNCUs) of various regions of Gujarat and to suggest strategies for effective delivery of KMC services. Methods: A pre tested, semi structured and validated questionnaire including general information about SNCU, baseline data about KMC protocols and practices of each SNCU etc. was developed. The information was collected from all 40 Special Newborn Care Units (SNCUs) of Gujarat State of India. The data analysis was performed using EpiInfo 7 Statistical Software developed by Centres for Disease Control and Prevention (CDC), Atlanta. Simple frequency tables were used for analysis. Results: About 81% of the facilities reported that newborns with birth weight less than 2000 grams, either stable or with non-serious medical conditions were eligible for receiving KMC. None of the facilities were providing continuous KMC more than 12 hours. Thirty (81.08%) SNCUs had an earmarked space for performing KMC either in the form of separate ward or identified corner in the ward. Conclusion: The present study revealed inadequate Kangaroo Mother Care services in selected newborn care units of Gujarat State of India. The services, especially basic facilities like drinking water, food, toilet and hand washing was not available in some of the facility.

Abstract Context: Early undergraduate exposure to research helps in producing physicians who are better equipped to meet their professional needs especially the analytical skills. Aim: To assess the effectiveness and acceptability of small group method in teaching research methodology. Setting: Sixth semester medical undergraduates (III MBBSpart1) of a self-financed rural medical college. Materials and Methods: The workshop was of two full days duration consisting of daily two sessions by faculty for 30 minutes, followed by group activity of about four hours and presentation by students at the end of the day. A simple 8 steps approach was used. These steps are Identify a Problem, Refine the Problem, Determine a Solution, Frame the Question, Develop a Protocol, Take Action, Write the Report and Share your Experience. A Pre-test and post-test assessment was carried out using a questionnaire followed by anonymous feedback at the end of the workshop. The responses were evaluated by blinded evaluator. Results: There were 95 (94.8%) valid responses out of the 99 students, who attended the workshop. The mean Pre-test and post-test scores were 4.21 and 10.37 respectively and the differences were found to be significant using Wilcoxon Sign Rank test (p<0.001). The median feedback score regarding relevance, skill learning, quality of facilitation, gain in knowledge was four and that of experience of group learning was 5 on a Likert scale of 1-5.There were no significant differences between male and female students in terms of Pre-test, post-test scores and overall gain in scores. Conclusion: Participatory research methodology workshop can play a significant role in teaching research to undergraduate students in an interesting manner. However, the long term effect of such workshops needs to be evaluated.

Abstract Introduction: Dissatisfaction with job is invariably linked with decreased work performance. Discontent provider is more likely to face difficulty in caring for his patients and provide quality medical care. Thus, provider dissatisfaction results in patient dissatisfaction which deleteriously affects health outcomes. Aim: The study was undertaken with the objective of finding determinants of job satisfaction amongst Primary Health Centre (PHC) doctors in Anand, Gujarat, India. Materials and Methods: The present cross-sectional analytical study was conducted amongst 50 PHC Medical Officers (MO), MBBS and Ayurveda, Yoga and Naturopathy, Unani, Siddha, Homeopathy (AYUSH) of Anand, Gujarat, India. Self-administered questionnaire method was used to obtain the responses from the participants. Job satisfaction was seen in four different heads/dimensions like “Workload”, “Relationships with Staff”, “Personal Difficulties” and “Availability of Infrastructure”. Some questions were in Likert scale and some questions were of dichotomous type. All the individual heads were summated to get a final score. Total scores ranged from 6-36. Categorical analysis of all the individual variables was also done. Face validity and consensual validity, and content validity of the questionnaire was checked. The data entry was done in excel and analysed using SPSS version 15.0. Qualitative as well as quantitative analysis was done. Mann-Whitney U test, and chisquare test were applied and values were calculated. Fisher’s exact test was also applied. Results: Out of total 50 MO, 33 (66.0%) were MBBS doctors and the rest were BAMS and BHMS doctors. Overall job satisfaction score was found to be significantly different amongst two groups (p=0.037). The two groups (MBBS and AYUSH) were statistically different in the workload scores (p=0.008). Conclusion: Overall job satisfaction scores were poor amongst PHC doctors. It was observed that doctors were unsatisfied on account of facilities in terms of good infrastructure, support services, good ...

Abstract "Background: Undernutrition continues to be underlying cause of morbidity and premature mortality among children in India. The primary goal of our study was to investigate the prevalence of child undernutrition and applying Svedberg theory to construct Composite Index of Anthropometric Failure (CIAF). Methods: We used cross-sectional anthropometric data for 372 child-mother pairs which were selected using two stage sampling. Anthropometric measurements was used to determine if children were low weight for age (underweight), low height for age (stunting) and low weight for height (wasting) based on WHO child growth standards and later used for construction of CIAF. Epi Info 7 and Anthro software used to calculate Z-scores and statistical analysis. Results: Based on CIAF, the prevalence of undernutrition was 60.5%. Of the children included in the study, 42.7% found to be underweight, 15% being found to have wasting and 50% showing sign of stunting. Using underweight as the only criterion for identifying undernourished children may underestimate the true prevalence by as much as 17.5%. Conclusion: Undernutrition still an important public health problem in this region. Use of WHO Z-score system recommended for identifying all aspects of undernutrition."

Abstract It would be worth to consider socio-demographic characteristics of the patients on DOTS (Directly Observed Treatment Short course), as the therapy requires a long term adherence. Material & Methods: Design: Cross-sectional study, Participants: 100 diagnosed TB patients on DOTS, from all TB units (25 from each TU) of Anand District, who had reported 3 weeks or later to health care center, after the onset of symptoms of TB. Information was filled up in a pretested questionnaire and the data was analyzed. Results: Among 100 TB patients (68 males and 32 females), 75% of the subjects had an education below 9th standard. 46% of subjects were laborer. 65% were self-dependent. Most of the respondents (75%) preferred government facility. Conclusion: The socio-demographic characteristics of DOTS patients are found to be of such levels that could have a pivotal role in the treatment success. They also need to be addressed under the program in such a manner that those characteristics don’t prove to be a barrier to treatment success.
