    Abstract Introduction: Three-in-one block and Fascia Illiaca Compartment Block (FICB) are two peripheral nerve block techniques that target the femoral nerve, obturator nerve and lateral femoral cutaneous nerve in a single injection. Both nerve blocks are used to provide anaesthesia and analgesia to the lower limb for various surgical procedures. The use of ultrasonography in peripheral nerve blocks helps to visualise the nerve, needle, and the distribution of the drug in real-time. It, thus, shortens the time of onset of sensory block, decreases performance time, and lowers the required drug doses, and finally increases chances of a favourable outcome from the nerve block. Aim: To compare the postoperative analgesic efficiency of ultrasound-guided 3-in-1 block with FICB in patients undergoing lower limb orthopaedic surgeries under General Anaesthesia (GA). Materials and Methods: A randomised clinical study was conducted at a tertiary care hospital. One hundred and fifty patients were randomly allocated to two groups, 3-in-1 block group or FICB. Both groups received the respective blocks after surgery before extubation. Postoperatively, Visual Analog Scale (VAS) scores were evaluated hourly for first 6 hours and 2 hourly thereafter until rescue analgesia was instituted. Time period from giving the block to giving rescue analgesia was noted as duration of analgesia. Injection diclofenac 1.5 mg/kg Intravenous (IV) was given when VAS value reached ≥4. Results: On analysis of 150 patients, divided into 3-in-1 block group (N=75) and FICB Group(N=75); mean age: 53.29±8.69 years; the VAS scores at 2 to 18 hours, 20 hours, and 22 hours after performing the blocks were significantly less in the 3-in-1 block group compared to FICB group. Also, the mean time (hours) for first rescue analgesia in FICB group (3.49±0.53) was earlier compared with 3-in-1 block group (7.35±0.51). Conclusion: A 3-in-1 block provides effective and prolonged postoperative analgesia in comparison to FICB.

    Abstract Introduction: Women undergoing elective Lower Segment Caesarean Section (LSCS) delivery under spinal anaesthesia usually experience anxiety and stress due to unpleasant operative environment. Music therapy medicine has been found to relieve anxiety and stress besides decreasing pain and stable cardiorespiratory parameters. Aim: To assess the effect of perioperative music therapy medicine on pain and cardiorespiratory parameters in women undergoing elective LSCS under spinal anaesthesia. Materials and Methods: The present study was case-control study in which 60 consecutive parturient women, planned for elective LSCS delivery under spinal anaesthesia, were randomly divided into music and non music groups of 30 each. Demographic characteristics of women were recorded. The ‘music group’ received preselected music three times for 20 minutes each along with standardised analgesia protocol. The ‘non music group’ received standardised analgesia protocol only. Respiratory and haemodynamic parameters were assessed during perioperative period. Postoperative pain was assessed using Visual Analogue Scale (VAS) scores in addition to comparisons of time for rescue analgesia between study and control groups. Statistical differences were derived using Mann-Whitney U-test. Results: Demographic characteristics of women were statistically insignificant. Significant differences were observed in Heart Rate (HR) at 30, 45 and 60 minutes Postanaesthesia Care Unit (PACU) (p-value 0.0278, 0.0151 and 0.02852, respectively) and Respiratory Rate (RR) at 60 minutes PACU (p-value 0.04884). This study found beneficial effect of music on pain as assessed by VAS in the postoperative period. Beneficial effect of music on pain observed in study group vs. controls at 1 hour (0.64 vs. 1.51), 2 hours (1.88 vs. 2.76) and 3 hours (3.07 vs. 3.47) of PACU stay (p-value 0.0003, 0.00152 and 0.02444, respectively).     Abstract Binders syndrome is an uncommon congenital facial deformity. It is a rare form of maxillofacial dysplasia with several nasal and mid-face deformities. There has been no literature about anaesthetic concerns regarding this uncommon disorder. Binder's syndrome is associated with difficult airway and difficulty in nasal intubation. Difficult airway cart must be kept ready, both during intubation and extubation. In addition, it is associated with mental retardation, cervical spine abnormalities and dental problems. As perioperative physicians, we must be aware of this anomaly and its peri-operative implications. We hereby describe the anaesthetic considerations in a case of Binder's syndrome in a young female patient presenting for nasal reconstruction.

    Abstract Geriatric medicine is a full-fledged speciality nowadays. Anaesthesia for geriatric patients is quite challenging. Elderly patients with serious cardiac problems are presenting for various surgeries, warranting the anaesthesiologist to formulate tailor-made peri-operative plan of management. We hereby present a case of an elderly male patient, posted for bilateral inguinal hernioplasty having a permanent pacemaker in situ in DDDR (dual-chamber rate-modulated) mode. The patient, on preoperative examination, was found to have a complete atrioventricular (AV) block on electrocardiogram (ECG) and electrophysiology study showed degenerative AV conduction disease and symptomatic complete AV (atrio-ventricular) block. The pacemaker mode changed to VOO (asynchronous ventricular pacing) mode preoperatively. Central neuraxial blockade was given. Beat-to-beat haemodynamic monitoring was instituted with invasive arterial catheter-transducer system. The patient tolerated the procedure of bilateral inguinal hernioplasty well and vital parameters were maintained throughout the operation. Phenylephrine infusion was given briefly to maintain the blood pressure (MAP > 65mmHg). After the operation, patient was shifted to the intensive care unit (ICU), pacemaker was reprogrammed to DDDR mode, and vigilant monitoring was done. The patient was shifted to the ward on the 2nd post-operative day. This case highlights that low-dose spinal anaesthesia along with vigilant monitoring is a reasonable choice for elderly patients with permanent pacemakers coming for elective surgeries.
