    Abstract The reconstruction of electrons and photons in CMS depends on topological clustering of the energy deposited by an incident particle in different crystals of the electromagnetic calorimeter (ECAL). These clusters are formed by aggregating neighbouring crystals according to the expected topology of an electromagnetic shower in the ECAL. The presence of upstream material (beampipe, tracker and support structures) causes electrons and photons to start showering before reaching the calorimeter. This effect, combined with the 3.8T CMS magnetic field, leads to energy being spread in several clusters around the primary one. It is essential to recover the energy contained in these satellite clusters in order to achieve the best possible energy resolution for physics analyses. Historically satellite clusters have been associated to the primary cluster using a purely topological algorithm which does not attempt to remove spurious energy deposits from additional pileup interactions (PU). The performance of this algorithm is expected to degrade during LHC Run 3 (2022+) because of the larger average PU levels and the increasing levels of noise due to the ageing of the ECAL detector. New methods are being investigated that exploit state-of-the-art deep learning architectures like Graph Neural Networks (GNN) and self-attention algorithms. These more sophisticated models improve the energy collection and are more resilient to PU and noise, helping to preserve the electron and photon energy resolution achieved during LHC Runs 1 and 2. This work will cover the challenges of training the models as well the opportunity that this new approach offers to unify the ECAL energy measurement with the particle identification steps used in the global CMS photon and electron reconstruction.

    Abstract Compression fractures of thoracolumbar vertebra are the most common vertebral fracture.1 Associated with osteoporosis, the compression can progress until reach vertebra plana and cause a kyphosis.2 The reduction of the fresh fracture and restoration of the height are the purposes of the kyphoplasty.3 We present a modified technique adapted for the treatment of vertebral plana using a mechanical flexible lamellar vertebral body reduction device (Tektona, Spinart).4 We present the case of a 77-year-old woman with back pain after a fall. The radiological investigations show a compressive fracture type AO A1 of T7. After an initial conservative management, the patient presented persistence of the pain with a visual analog scale of 9/10 and progression of the fracture at the 2-week follow-up. On the x-ray, focal kyphosis with vertebra plana was observed. A kyphoplasty for pain management and kyphosis correction was performed. The patient consented to the procedure. A bipedicular approach to the vertebra was performed. The correction of the height was done using 2 mechanical devices. The lamellar device allowed a progressive height restoration of the fracture. On one side, the cannula was used as a support to maintain the correction during the contralateral cementing. The postoperative imaging confirmed a maintained vertebral restoration with kyphosis correction. At 1 year, the patient reported a pain at 1 of 10 on the visual analog scale. In our opinion, this technique is adapted for fresh osteoporotic vertebral compression fractures. Surgical experience in osteoporotic fracture treatment and kyphoplasty is recommended before the management of vertebra plana.

    Abstract Introduction/aims: Lumbosacral radiculopathy (LR) is a common disorder. Neuromuscular ultrasound (NMU) is a rapidly evolving technique for the investigation of peripheral nerve and muscle disorders, but studies using NMU in LR are lacking. The aim of the present study was to investigate ultrasonographic neuromuscular changes distant from root compression in patients with subacute to chronic compressive LR with motor impairment.
    Methods: Patients with unilateral subacute to chronic L4, L5, or S1 radiculopathy with motor impairment and confirmed by magnetic resonance imaging were included. The sciatic and femoral nerve cross-sectional areas (CSA), the CSA of lower limb muscles, and muscle fasciculation detection rate were assessed using a pre-specified neuromuscular ultrasound evaluation with blinded side-to-side comparison.
    Results: Of the 18 included patients, 66% were male and the mean age was 51 years. Overall, 16.7% had L4, 55.5% L5, and 27.8% S1 radiculopathy, mostly due to disc herniation (83%). Sciatic nerve CSA of the symptomatic side was increased (61.4 mm
    Discussion: NMU evaluation in patients with symptomatic subacute to chronic LR revealed sciatic nerve enlargement distant from nerve root compression and higher fasciculation rates. These structural findings on NMU might be due to an axonal repair mechanism and an inflammatory response with endoneurial edema induced by ongoing nerve damage and potentially reflect progressive axonal loss.

    Abstract Purpose: Spontaneous intracranial or intrathecal hypotension (SIH) is an underdiagnosed phenomenon predominantly presenting with low cerebrospinal fluid (CSF) pressure and postural headache in the setting of CSF leak. Extrathecal CSF collections causing compression of the spinal cord or nerve roots present an even rarer subset of this disease. We aim to describe this pathology in a comprehensive manner while illustrating with a case of our own.
    Methods: We present a literature review on spinal idiopathic pseudomeningoceles and their neurological implications illustrated with a case of an anterior compressive pseudomeningocele between C2 and D7. Further investigations through a myelography and myelo-CT were able to postulate a CSF leak through a discogenic osteophytic microspur at the level C5-C6.
    Results: Spinal manifestations are uncommon in cases of idiopathic or spontaneous CSF leak, occurring in about 6% of patients, but myelopathy and radiculopathy involving all spinal segments do occur. In contrast to the cranial complaints, the spinal manifestations usually are not positional and are caused by mass effect from an extradural CSF collection.
    Conclusion: The utility of multiple imaging modalities such as dynamic myelography and the use of epidural blood patches and fibrin glue polymers should be explored, and surgery is an option if the symptoms persist despite other measures.

    Abstract Introduction: In recent years, an increasing trend in the incidence of melanoma has been observed in Europe. Although early diagnosis and prompt intervention with local resection often results in positive outcomes, conversely, metastatic disease is still clinically challenging with a poor prognosis and a 5-year survival of around 30%. The growing awareness of melanoma biology and of antitumor immune responses has allowed the development of novel therapies targeted at specific molecular alterations occurring at advanced stages. This real-world analysis examined patients with melanoma in Italy, focusing on treatment patterns, outcome, time to discontinuation (TTD), and resource consumption.
    Methods: Two retrospective observational analyses were conducted for BRAF+ patients with metastatic melanoma and those with a positive sentinel lymph node biopsy in an adjuvant setting, retrieving data from the administrative databases covering 13.3 million residents. The cohort melanoma BRAF+ in metastatic setting comprised 729 patients with targeted therapy (TT) (n = 671 with TT as first line and 79 as second line).
    Results: Median TTD was 10.6 months in first line and 8.1 months in second line. Median overall survival from the start of first TT line was 27 months and was 11.8 months for patients with brain metastasis. In the dabrafenib plus trametinib patients, main healthcare resource consumption tended to increase in the presence of brain metastasis. The cohort with a positive sentinel lymph node biopsy under adjuvant therapy (n = 289) included 8% patients treated with dabrafenib plus trametinib or tested BRAF+, 5% BRAF wild-type, and 10% under immunotherapy.
    Conclusion: Our findings provided an overview on TT utilization on metastatic melanoma patients in real clinical practice and highlighted an increased burden in brain metastatic patients.

    Abstract Background: This real-world analysis evaluated drug utilization focusing on wastage and healthcare costs for treatment of patients with advanced breast cancer (aBC) hormone receptor-positive (HR+)/human epidermal growth factor receptor-2 negative (HER2-) in Italy.
    Methods: A retrospective analysis was conducted on administrative data covering about 13.3 million health-assisted individuals. Across January/2017-June/2021, all patients with HR+/HER2-aBC were identified by ≥ 1 prescription for cyclin-dependent kinase 4/6 inhibitors (CDK 4/6i). Cost analysis was performed and updated referring to the prices of November 2021.
    Results: Overall, 3,647 HR+/HER2-aBC patients were included (2,627 palbociclib treated, 729 ribociclib treated, and 291 abemaciclib treated). After 12 months of follow-up, 35% of palbociclib patients had a dose reduction (on average 8.9 wasted pills/patient), 44.7% of abemaciclib patients had a dose reduction (on average 6.7 wasted pills/patient), 22.1% of ribociclib patients had a dose reduction (no wasted pills). Therapy wastage added up to 528,716€ for palbociclib-treated patients (524€/patient) and 5,738€ in abemaciclib-treated patients (151€/patient). No wastage was attributed to ribociclib.
    Conclusions: Dose reduction was associated with drug wastage in palbociclib and abemaciclib-treated patients, but not in ribociclib-treated ones. These findings might be helpful to policy decision-makers who, for healthcare strategies implementation, among several variables should consider the possible restraining of drug wastage.

    Abstract Aims: During the Year 2013 we took in charge at our Hospital in Lugano 21 patients suffering by aneurysmatic lesions, with SAH or in elective surgery. We evaluate retrospectively the outcome between patients who underwent DSA and who didn’t. Furthermore, we evaluated correlation between aneurysm size and location and outcome.
    Methods: We collected retrospectively all the data about the patients treated in 2013, suffering by aneurysmatic lesions. We then compared patients who underwent endovascular diagnostic or treating procedures, with those patients who didn’t. We compared then outcome at the discharge from the Intensive Care Unit, according to the mRanking. In those patients who had worst outcome, we evaluate a correlation between the greatest diameter of the aneurysm and the negative outcome, using MIP sequences on the angio-CT scan.
    Discussion: In 2013, we took in charge 21 patients, who suffered by aneurysmatic lesions, whose 15 had acute stroke with SAH and 6 no ruptured aneurysms. In 19 cases (89%) we were able to perform a treatment. Patients with good clinical presentation at the onset (WFNS I-II), had better outcome, with mRanking 1 in 89% at the discharge. In patients with WFNS >II, the outcome was at contrary with exitus in 75% of the cases. Localization of the aneurysms showed that the most common lesions were on the left MCA (M1-M2), by which suffered the 33% of patients. Lesions with worst outcome had been those on the ACoA, detected on 14% of patients only, but with exitus in 66% of cases. Preoperative Digital Suppressed Angiography (DSA) was performed in 53% of patients. In the remaining 47% of cases, we performed surgical treatment with just angio-CT scan. The mRanking 1 was the result in 55% of patients who underwent DSA, while patients who didn't had good outcome in 50% of cases. About outcome in treated patients, good outcome was achieved in 75% of patients with coiling and 45% in those who underwent craniotomy. Finally, 78% of patients with poor outcome (mRanking >3) had aneurysms with the greatest diameter >15 mm, whose 100% the patients who died (also those who didn’t receive any treatment).
    Conclusion: There wasn’t a great difference in good outcome between patients who underwent preoperative DSA and those who didn't (55% vs. 50%). Patients treated in endovascular way had a great benefit, with 75% of mRanking 1 at discharge. Patients with poor outcome had greater diameter of the aneurysm (75% >15 mm), or ACoA aneurysms (exitus in 66%).

    Abstract This real-world analysis investigated patients with follicular lymphoma in Italy receiving three or more treatment lines (≥3L), focusing on therapeutic pathways with their rebounds on healthcare resource consumptions and costs. Data were retrieved from administrative databases from healthcare entities covering about 13.3 million residents. Adults diagnosed with follicular lymphoma were identified between January 2015 and June 2020, and among them 2434 patients with ≥3L of treatment during the data availability interval (January 2009 to June 2021) were included. Of them, 1318 were in 3L, 494 in 4L and 622 in ≥5L. A relevant proportion of patients (12-32%) switched to a later line within the same calendar year. At 3-year follow-up (median), 34% patients died. Total mean annual expenses were euro 14,508 in the year preceding inclusion and rose to euro 21,081 at 1-year follow-up (on average euro 22,230/patient/year for the whole follow-up), with hospitalization and drug expenses as weightiest cost items. In conclusion, the clinical and economic burden of follicular lymphoma increases along with later treatment lines. The high mortality rates indicate that further efforts are needed to optimize disease management.

    Results: AKI was reported in 12.8% of patients, 50% of whom were stage 1 (KDIGO criteria), while 25.7% of the cohort developed AKD. Patients with baseline estimated Glomerular Filtration Rate (eGFR) < 90 ml/min showed a higher incidence of AKD (36.2% vs 17.7%). Hypertension, baseline eGFR, and therapy with Renin-angiotensin-aldosterone system inhibitors proved to be significant factors associated with both AKI and AKD.
    Conclusion: AKI and AKD are not rare complications of high-dose cisplatin, but an appropriate prevention strategy and accurate monitoring of patients during treatment could lead to a reduction of the burden of these conditions.
