    Abstract For contagious diseases like measles a successful immunization program can result in herd protection. Small outbreaks may still occur but fade out soon, because the possibilities for the pathogen to spread in the 'herd' are very small. This implies that people who refuse to participate in such a program will still benefit from the protection it offers, but they don't do their part in maintaining protection. Isn't that a case of freeriding-and isn't that unfair towards all the people who do collaborate? If so, that might be considered an additional ground for making vaccination mandatory or compulsory. In this paper I argue that vaccination refusal can be considered as freeriding, but that this might not be unfair. The public good of herd protection is a peculiar public good because it supervenes on private benefits that are enjoyed by all who do opt for vaccination. For vaccinated individuals, the additional benefit of herd protection comes about, as it were, for free, and hence they can't complain that others benefit without sharing in the burdens. There are however still other grounds for making vaccination compulsory or at least for seeing refusal as a morally wrong choice.

    Abstract Tobacco discouragement, smoking cessation and tobacco endgame policies are sometimes criticised for being unduly paternalistic: governments should respect citizens' freedom and not take over their individual responsibility for healthy behaviour. In this commentary, I argue that very strict tobacco policies can be justified on multiple grounds, including the harm principle, the public good of maintaining a healthy society, and the reduction of health inequities. The moral reasons governments have to protect people against the harms of smoking do not limit or infringe upon the responsibility of each individual to take care of her own health: responsibility for healthy behaviour is not a zero-sum game.

Small outbreaks may still occur but fade out soon, because the possibilities for the pathogen to spread in the ‘herd’ are very small.     Abstract Environmental pollution and greenhouse gas emissions that contribute to climate change have adverse impacts on global health. Somewhat paradoxically, health care systems that aim to prevent and cure disease are themselves major emitters and polluters. In this paper we develop a justification for the claim that solidaristic health care systems should include sustainability as one of the criteria for determining which health interventions are made available or reimbursed - and which not. There is however a complication: most adverse health effects due to climate change do occur elsewhere in the world. If solidarity would commit us to take care of everyone's health, worldwide, it might imply that solidaristic health system cannot justifiably restrict universal access to their own national populations. In response we explain health solidarity is to be considered as a moral ideal. Such an ideal does not specify what societies owe to whom, but it does have moral implications. We argue that ignoring sustainability in political decision making about what health care is to be offered, would amount to betrayal of the ideal of solidarity.

    Abstract In contrast-enhanced echography, the simulation of nonlinear propagation of ultrasound through a population of oscillating microbubbles imposes a computational challenge. Also, the numerical complexity increases because each scatterer has individual properties. To address these problems, the Iterative Nonlinear Contrast Source (INCS) method has been extended to include a large population of nonlinearly responding microbubbles. The original INCS method solves the Westervelt equation in a four-dimensional spatiotemporal domain by generating increasingly accurate field corrections to iteratively update the acoustic pressure. The field corrections are computed by the convolution of a nonlinear contrast source with the Green's function of the linear background medium. Because the convolution integral allows a coarse discretization, INCS can efficiently deal with large-scale problems. To include a population of microbubbles, these are considered as individual contrast point sources with their own nonlinear response. The field corrections are computed as before, but now, in each iteration, the temporal signature of each contrast point source is computed by solving the bubble's Marmottant equation. Physically, each iteration adds an order of multiple scattering. Here, the performance of the extended INCS method and the significance of multiple scattering is demonstrated through various results from different configurations.

    Abstract Planetary health is not only a new field of research and practice; it is also a moral ideal. What are the implications for medicine and health care? In this article, we argue that within this ideal, health of humans, animals and also nature are worthy of protection for their own sake. These values can reinforce each other, but they may also conflict. We formulate a general framework that provides some direction for ethical reflection. Next we discuss implications of the ideal of planetary health for zoonotic disease outbreaks, for environmental sustainability of health care, and for global health and solidarity in the face of climate change. Planetary health demands much from health care, and this will also further exacerbate existing policy dilemmas.

    Abstract Nutrition, as a science, is facing challenges. While issues regarding obesity, chronic diseases, and sustainability are becoming more pressing, nutrition science is encountering limitations regarding novel insights, trust, and social relevance. In order to move forward, we need to innovate the field and explore new perspectives. Current nutrition research has mainly employed a reductionist approach, which has been very successful in the past. However, reductionism shows limitations when addressing the problems we face today. The addressed weaknesses of reductionism include (1) the questionable assumption that nutrients and calories are exchangeable between foods, (2) the tendency of reductionism to oversimplify reality, which has consequences for complex concepts such as health and nutrition, and (3) the focus on details, which could undermine the aim of nutrition science: creating optimal dietary guidelines for the promotion of health and prevention of disease. Holism offers an alternative perspective that could complement these limitations, on the condition that they are similar enough on an ontological and epistemological level. Holistic approaches to health appear in eastern philosophies (ayurveda), but also in modern western nutrition approaches (dietary patterns). These two holistic approaches can complement reductionism in the following ways: (1) Holistic approaches like ayurveda and dietary patterns provide different nutritional knowledge by considering multiple factors that affect food’s health potential, in addition to only nutrients and calories. Some of these factors include food processing, food matrix/structure, food combinations, food compatibility, and nutrient interaction. (2) Holism can complement the reductionistic tendency to oversimplify reality by including subjective, individual, and holistic aspects of health into nutrition research and embracing the complexity of food-chronic disease relationships.
