    Abstract Within six months only of its existence as result of the ongoing pandemic of COVID-19 caused by the SARSCoV2, various safety measures have been applied in order to offer both protection and attention to the worldwide population. Healthcare professionals have been making great efforts to fight the disease on a hospital care level but paradoxically, acts of aggressive discrimination have been presented towards the health personnel, which mainly include: denying services, threats, and physical damage as well as patrimonial. This paper’s purpose is to describe some of the elements that help understand the mass social discriminative behaviour showing up towards healthcare workers. Healthcare personnel and general population’s mental health states during this pandemic are both described, as well as background checks of aggression towards healthcare workers predating the present pandemic, allowing a bigger picture to be examined, with the intention of establishing new general guidelines to prevent future acts of discrimination.

    A raíz de la pandemia de COVID-19 provocada por el SARS-CoV 2, en tan solo 6 meses de su existencia se han adoptado distintas medidas de protección y atención a la población mundial. Los trabajadores de la salud han realizado grandes esfuerzos para combatir la enfermedad a nivel hospitalario. De forma paradójica, se han identificado actos de discriminación y agresión en contra del personal sanitario, que incluyen la negación de servicios, amenazas, daño físico y patrimonial, principalmente. El propósito de este trabajo es describir algunos elementos que permitan comprender el comportamiento social de discriminación que sufren los trabajadores sanitarios. Se describe el estado de la salud mental en la población general y del personal intrahospitalario durante la pandemia, así como los antecedentes de agresiones al personal de salud previos a esta condición, que permiten tener un panorama general desde distintas perspectivas, con el fin de establecer lineamientos generales útiles para su prevención.

    Abstract Diagnosing intestinal tuberculosis (TB) with uncommon clinical manifestations is often challenging. Here, we report a case of an alcoholic patient who presented with vague symptoms and was later diagnosed with intestinal TB. This patient experienced multiorgan failure causing hemodynamic instability requiring ionotropic support; acute hypoxic respiratory failure managed with non-invasive positive pressure ventilation, hepatic failure, transudative peritoneal effusion, and transudative pleural effusion. These conditions clouded our judgment to pursue colonoscopy for a definite diagnosis and delayed the anti-tuberculosis treatment. When intestinal tuberculosis TB is suspected, the differential diagnosis must be established with other gastrointestinal involving diseases, including mycobacterium avium complex (MAC) and Crohn's disease (CD). MAC can show overlapping features with intestinal TB or coexist with it; Acid-fast stain and tissue culture are the key tests to differentiate these two. In the presence of diagnostic uncertainty between intestinal TB and CD, a therapeutic trial with anti-tuberculous therapy may be warranted.

    Abstract BACKGROUND Posterior reversible encephalopathy syndrome (PRES), also known as reversible posterior leukoencephalopathy, is a neurotoxic state with multiple etiologies characterized by altered mental state, headaches, visual abnormalities, and seizures. This clinico-radiological syndrome is rare, and a high index of suspicion is needed to diagnose, provide adequate treatment, and prevent irreversible neurological sequelae. CASE REPORT We present a case of a woman with end-stage renal disease (ESRD) who presented with acute confusion and non-convulsive seizures and was later diagnosed with PRES. In this case, altered mental status was initially thought to be secondary to uremic encephalopathy. A diagnosis of PRES was subsequently made after she had several sessions of HD without significant improvement in her mental state, prompting magnetic resonant imaging (MRI) for further evaluation. Specific risk factors for PRES, including blood pressure fluctuations, were targeted and she made significant clinical recovery but had residual functional impairment. CONCLUSIONS This case underscores the need for a high index of suspicion, especially in cases with atypical presentation, as delayed diagnosis can lead to suboptimal outcomes.

    Abstract Nowadays, anti-programmed death-1 (PD-1) antibodies are the first-line treatment for metastatic malignant melanoma. An infusion reaction is an adverse event that could occur due to monoclonal antibodies administration and requires prompt diagnosis and treatment. In this article, we report on a case of stage IV malignant melanoma treated with nivolumab, in which a severe infusion reaction occurred, manifesting as flushing and hypotension followed by bronchospasm. The switch to pembrolizumab was both a well-tolerated and effective therapeutic alternative.

    Abstract BACKGROUND Myocarditis is cardiac muscle inflammation caused by infectious or noninfectious agents. Rarely, clozapine, an atypical antipsychotic drug used to treat resistant schizophrenia, has been reported to cause myocarditis, as we report in this case. CASE REPORT A 29-year-old man, who was known to have schizophrenia and was on olanzapine therapy, presented in our Emergency Department with active psychosis, and was subsequently admitted to the psychiatric ward for refractory schizophrenia. He was started on clozapine, which was cross-titrated with olanzapine. On day 20 of being treated with clozapine, he developed a high-grade fever and chest pain. EKG demonstrated new-onset prolonged QT corrected for heart rate (QTc), premature ventricular contractions, ST-T wave changes with an increased ventricular rate, and ventricular bigeminy with elevated troponin and inflammatory markers. Echocardiography showed a reduced left ventricular ejection fraction. Coronary angiography showed normal coronary arteries, low cardiac output, and cardiac index consistent with cardiogenic shock was also observed. Other pertinent laboratory results included negative respiratory viral panel, including COVID-19 PCR, negative blood cultures, and negative stool screen for ova and parasite. Clozapine was discontinued and the patient received management for heart failure with reduced ejection fraction. He improved clinically with return of EKG to normal sinus rhythm and improved left ventricular ejection fraction on repeat echocardiogram. CONCLUSIONS Acute myocarditis can occur due to a myriad of causes, both infectious and noninfectious; thus, determining the lesser-known causes, such as drug-related etiology, is essential to provide appropriate treatment for patients.

    Abstract Tent‐making bats modify leaves to build refuges. Leaf modification involves energetic and defense costs that should be balanced by the benefits of tent‐roosting. The alteration of the leaf's vascular system reduces the tent's life expectancy, so to obtain a benefit, bats are expected to use tents regularly as long as they remain functional and not modify more leaves than necessary. Over 2 yr, we documented the dynamics of tent construction and use by Uroderma convexum and other bat species in the palm Sabal mauritiiformis in a Colombian transitional dry forest. We also assessed tent condition and compared it to nonmodified leaves of approximately the same age in focal palms. Probability of tent use by U. convexum varied between 57 percent during a reproductive period and 4 percent outside of this period. Bats cut the main vein of folioles, partially affecting water transport in the leaf. However, there were no differences between tents and nonmodified leaves in deterioration scores or deterioration rates over 1 yr. During 2 yr, 48 tents were lost for different causes, but this loss was balanced by the construction of 51 new tents. Thus, bats maintained an excess of usable tents. Palm leaves are long‐lived and seem preadapted to sustain damage and remain viable, particularly in species growing in dry environments. We present several hypotheses to explain the advantage of maintaining a tent surplus.

    Abstract Paintings produced spontaneously by patients with neurological lesions represent a fascinating opportunity to analyze some aspects of the underlying disease and involved brain mechanisms. Many cases of artists who have suffered spatial neglect following a neurological disease have been reported in the literature. However, only a few studies evaluating the different subtypes of graphic neglect and aspects related to the construction of perspective (three dimensionality) in works of art have been published. In the present article, we present the case of an artist who, after resection of a central neurocytoma that affected the right thalamo-parietal connections, suffered an impairment of the ability to create perspective in his paintings and involuntary omission of only shapes in the left side of his paintings, although colors and contours were preserved.
