    Abstract Abstract Background Hydatid disease is a parasitic infection which is endemic in multiple regions across the globe including Indian subcontinent. Although it commonly affects the liver and other solid organs, isolated involvement of the heart is relatively uncommon. Cardiac hydatidosis can be confidently diagnosed on echocardiography, but cross-sectional imaging by computed tomography (CT) or magnetic resonance imaging (MRI) provides accurate information about its location, extent, and multiplicity. Multiple case studies of cardiac hydatid disease have been reported in the literature. However, cardiac hydatidosis with extension into pulmonary vasculature has not been reported commonly. This case report highlights this rare association, with stress upon the altered surgical management. Case presentation A 50-year-old patient presented with atypical chest pain to the cardiology department of a tertiary care hospital where he underwent multiple blood tests as well as electrocardiography (ECG) and echocardiography as part of the routine examination. Echocardiography revealed a large, multicystic mass invading the posterolateral wall of left ventricle (LV) with no internal vascularity and was provisionally diagnosed as hydatid disease. Subsequent CT study confirmed a large multicystic lesion with the presence of another smaller lesion invading the left atrial appendage (LAA). In order to evaluate the vascular and mediastinal invasion, cardiac MRI was then performed which revealed invasion of right pulmonary artery (PA), right superior pulmonary vein (PV) as well as compression of superior vena cava (SVC) by the smaller lesion. A final diagnosis of cardiac hydatidosis with invasion into right PA and right superior PV was made. Treatment plan for the patient changed, who then underwent an additional reconstruction of right PA and right superior PV after cyst removal.     Abstract Intraductal papillary neoplasm of the bile duct (IPN-B) is a rare preinvasive intraductal pathology of the biliary tract. It should be differentiated from other more common benign or malignant causes of biliary obstruction and dilatation such as calculi or cholangiocarcinoma because the management and prognosis of this condition differs significantly. This case report describes a case of IPN-B in a 45-year-old female patient who presented with non-specific complaints of chronic abdominal pain without jaundice for three months.

    Abstract Background: Pregnancy which is not terminated within the boundaries of legal definition in India is called an unsafe abortion. Objective: To observe the proportion of unsafe abortions among total abortions in 15–49-year-old ever married women across India between 1990 and 2020. Materials and Methods: A comprehensive search was conducted in National Library of Medicine's PubMed database, IndMED, TRIP, Web of Science, and Google Scholar for articles published between January 1, 1990, and February 29, 2020. A combination of medical subject headings and free text word like abortion, induced, unsafe, women 15–49 years, prevalence, and India were used. Each article was assessed for quality using Joanna Briggs Institute Critical Appraisal Tool. The proportion of subjects classified as having unsafe abortion was the outcome of meta-analysis. Analysis was done using Comprehensive Meta-Analysis version 2.2. All pooled estimates were calculated using DerSimonian–Laird random-effects model and reported as a proportion with 95% confidence intervals. Results: The pooled estimate of the mean prevalence of unsafe abortion in India was 37.56% (confidence interval: 20.68–54.44). There was a significant heterogeneity between the studies (I2 = 99.98% and Cochran's Q = 113538.65, df = 18, P < 0.001). Conclusion: Even after several amendments in Medical Termination of Pregnancy Act 1971 till 2020, unsafe abortions are highly prevalent in entire India, irrespective of high-performance or low-performance states. In urban India, the prevalence of unsafe abortions is comparatively higher than rural areas.

    Abstract Background and Aims: Faith healing is a method of treating illnesses through the exercise of faith rather than medical methods. The current study was done with the objective to find out demographic variables of faith healers, various conditions for which the people approach them, various practices and processes used and to look for scope of integrated approach to healthcare with faith healers. Settings and Design: RHTC village was a sub center village of PHC Morad. Hence, for the study, all the villages which comes under PHC Morad were selected. The design of the study was a cross-sectional study. Methods and Material: The study was a cross sectional study where faith healers were interviewed based on a semi structured questionnaire comprising of both close and open-ended questions. Statistical Analysis Used: Absolute numbers. Results: Different reasons for which the persons usually came for ranged from supernatural possession, unemployment, family problems etc., and various different processes were used by these healers to solve the problems included giving charms and amulets, personal sacrifices like “baddha” (Nischay/praan) and many others. Conclusion: The study concludes that faith healers in many conditions and situations became the first point of contact as the faith of the community in these faith healers is deeply rooted.

    Abstract Introduction: Dissatisfaction with job is invariably linked with decreased work performance. Discontent provider is more likely to face difficulty in caring for his patients and provide quality medical care. Thus, provider dissatisfaction results in patient dissatisfaction which deleteriously affects health outcomes. Aim: The study was undertaken with the objective of finding determinants of job satisfaction amongst Primary Health Centre (PHC) doctors in Anand, Gujarat, India. Materials and Methods: The present cross-sectional analytical study was conducted amongst 50 PHC Medical Officers (MO), MBBS and Ayurveda, Yoga and Naturopathy, Unani, Siddha, Homeopathy (AYUSH) of Anand, Gujarat, India. Self-administered questionnaire method was used to obtain the responses from the participants. Job satisfaction was seen in four different heads/dimensions like “Workload”, “Relationships with Staff”, “Personal Difficulties” and “Availability of Infrastructure”. Some questions were in Likert scale and some questions were of dichotomous type. All the individual heads were summated to get a final score. Total scores ranged from 6-36. Categorical analysis of all the individual variables was also done. Face validity and consensual validity, and content validity of the questionnaire was checked. The data entry was done in excel and analysed using SPSS version 15.0. Qualitative as well as quantitative analysis was done. Mann-Whitney U test, and chisquare test were applied and values were calculated. Fisher’s exact test was also applied. Results: Out of total 50 MO, 33 (66.0%) were MBBS doctors and the rest were BAMS and BHMS doctors. Overall job satisfaction score was found to be significantly different amongst two groups (p=0.037). The two groups (MBBS and AYUSH) were statistically different in the workload scores (p=0.008). Conclusion: Overall job satisfaction scores were poor amongst PHC doctors.     Abstract Summary: Background: It is critical to identify high-risk groups among children with COVID-19 from low-income and middle-income countries (LMICs) to facilitate the optimum use of health system resources. The study aims to describe the severity and mortality of different clinical phenotypes of COVID-19 in a large cohort of children admitted to tertiary care hospitals in India. Methods: Children aged 0–19 years with evidence of SARS-CoV-2 infection (real time polymerase chain reaction or rapid antigen test positive) or exposure (anti-SARS-CoV-2 antibody, or history of contact with SARS-CoV-2) were enrolled in the study, between January 2021 and March 2022 across five tertiary hospitals in India. All study participants enrolled prospectively and retrospectively were followed up for three months after discharge. COVID-19 was classified into severe (Multisystem Inflammatory Syndrome in Children (MIS-C), severe acute COVID-19, ‘unclassified’) or non-severe disease. The mortality rates were estimated in different phenotypes. Findings: Among 2468 eligible children enrolled, 2148 were hospitalised. Signs of illness were present in 1688 (79%) children with 1090 (65%) having severe disease. High mortality was reported in MIS-C (18.6%), severe acute COVID-19 (13.3%) and the unclassified severe COVID-19 disease (12.3%). Mortality remained high (17.5%) when modified MIS-C criteria was used. Non-severe COVID-19 disease had 14.1% mortality when associated with comorbidity. Interpretation: Our findings have important public health implications for low resource settings. The high mortality underscores the need for better preparedness for timely diagnosis and management of COVID-19. Children with associated comorbidity or coinfections are a vulnerable group and need special attention. MIS-C requires context specific diagnostic criteria for low resource settings. It is important to evaluate the clinical, epidemiological and health system-related risk factors associated with severe COVID-19 and mortality in children from LMICs. ...

    Abstract The study was carried out in 200 male volunteers. They were divided into two groups. The study group was exposed to noise levels of more than 80 dB(A) for more than 8 hours a day for a period of 6 months, working in the steel and hammer industry, whereas the control group was working under normal conditions. The mean age of subjects was 33.33 + 0.867 years and the mean noise level to which they were exposed was 90.34 + 0.781 dB(A). Various autonomic function tests were carried out in both the groups and results were analyzed using Z test. Heart rate was recorded on cardiofax ECG machine and blood pressure (BP) was recorded using sphygmomanometer. The tests depicted significant increase in the mean resting heart rate and the heart rate response to standing (P=0.000), 30:15 ratio (P=0.002), the valsalva ratio (P=0.017), the % change in diastolic BP response to standing (P=0.000) and valsalva maneuver (P=0.000), the systolic BP and diastolic BP after cold pressor test (P=0.000) in study group as compared to the control group. The significant higher results in study group may be attributed to increased sympathetic activity. Thus, noise presents as a significant health hazard. It is recommended that maximum allowable duration of exposure should be reviewed and strictly followed.

    Abstract The present study was conducted on 12 albino rabbits of either sex and weighing between 1-1.5kg to see the influence of coconut oil and soyabean oil on serum TSH levels and weight gain for a period of 12 weeks. The rabbits were divided into 2 groups of six each. Rabbits in group 1 were fed on coconut oil and in group 2 were fed on soyabean oil in addition to their standard diet. At the end of 12 weeks we found that rabbits fed on soyabean oil had significant increase in TSH levels (p= 0.003) and gained more weight (p=0.000) when compared to rabbits fed on coconut oil.
