Abstract This paper presents two cases of acute postoperative upper airway obstruction following ventral bulla osteotomy (VBO) in cats. The first cat underwent a unilateral left-sided VBO for a suspected inflammatory polyp. The second cat underwent a single-session bilateral VBO procedure for bilateral otitis media. In the first case, immediate re-intubation and a gradual lightening of the anaesthetic plane resolved the clinical signs; in the second case, the patient deteriorated and went into acute cardiorespiratory arrest and received cardiopulmonary resuscitation. Both patients recovered well and were discharged home 3 days after surgery. Both cases were reported to show no further clinical signs on postoperative follow-up 3 weeks and 4 months after surgery, respectively. Upper airway obstruction should be regarded as a potential complication of VBO in cats.

Abstract The purpose of the biomonitoring project in the Nature Conservancy in the Czech Republic is to obtain information on long-term development of some selected living components of ecosystems in protected areas. A survey of methods was conducted to select the monitoring areas as well as to study birds, epigean, vegetation and water communities (a minimal programme). A system for the monitoring areas and data storage and processing (to gather a body of evidence) was established. Simultaneously, close linkage with abiotic factor monitoring is planned. The project has been carried out by the Czech Institute for Nature Conservation.

Abstract Complicated case, which after several surgical interventions ended up with an acquired esophageal stricture is presented. The possibility of reflux esophagitis should be kept in mind during postoperative period, especially in primarily troubled patients. An early management of reflux esophagitis could prevent such a condition successfully and help taking full control of a patient after surgery. This report describes etiology of the problem, follows its dynamics, as well as evaluates treatment options suggested in textbook against options available and those finally applied after consulting the owner. Authors try to point out cardial incompetence as one of the most important etiological factors. Despite some early remissions, authors finally succeeded to extend the diameter in the site of the stricture from 5 mm to over 15 mm within seven sessions performed in the period of six months, improving functional outcome.

Abstract Palliative surgery for advanced-stage prostatic cancers was tested with regard to survival rate and complications in a prospective randomised clinical study of dogs. Currently, therapeutic approaches have a grave long-term prognosis in clinically significant prostatic cancer. Of 167 dogs with prostatic disorders, 24 were diagnosed with prostatic cancer. Eleven dogs underwent subtotal intracapsular prostatectomy, while in 10 dogs total prostatectomy was performed. The remaining three dogs were euthanased at their owner's request. Dogs treated by subtotal intracapsular prostatectomy and those treated by total prostatectomy were followed until their death. It was found that dogs treated by subtotal intracapsular prostatectomy survived 5·63 times longer (mean [sd] 112·0 [63·03] days) than those treated by total prostatectomy (19·9 [10·67] days) (P<0·01). Moreover, a significant decrease in postoperative complications after subtotal intracapsular prostatectomy was recorded, especially with regard to urinary incontinence. It was concluded that, in the authors' facility, treatment of prostatic cancer by subtotal intracapsular prostatectomy was superior to that by total prostatectomy, with respect to both postoperative survival and serious complications.

Abstract Various prostatic disorders are described in older intact dogs. Progression of disease commences quite often in dogs aged 5.5 years or older. Clinical symptoms are transient in the beginning, and are difficult to recognize. In fact, patient feels very little, if any, discomfort as troubles develop subsequently. As an exception to the rule there is acute inflammation, symptoms of which are severe, sometimes even life threatening. Diagnosis not meticulously established, when procedure is not met properly, (e.g. recognition of prostatitis without identification of infectious agent) this may result in development of chronic disease, which is often hard to manage. Rectal palpation should be part of routine examination in all older dogs, while correct diagnostic protocol should follow. By means of precise diagnostic protocol, we are able to identify the problem early, often in the curable stage of disease; therefore we are able to maintain fertility of the patient, as well. So far, described prostatic disorders are benign hyperplasia (BPH), squamous metaplasia, prostatic inflammation, both acute and chronic, prostatic cysts and abscesses and neoplasia.

Abstract Objectives: To validate a porcine model of ruptured abdominal aortic aneurysm (rAAA) repair.
    Design: Experimental study.
Methods: Ten experimental and five sham-operated pigs were studied. Instrumentation for cardiac output (CO) measurement, regional blood flow (renal-REN and portal-PORT) and blood sampling (inferior vena cava (IVC), renal and portal vein) was done. Microcirculation was visualised sublingually and in ileostoma.
Protocol: simulation of rAAA with bleeding (mean arterial pressure (MAP) 45 mmHg) and increased abdominal pressure (25 mmHg) for 4 h; 2 h of infrarenal clamp with shed blood retransfusion; 11 h of post-surgery care.
Results: Six experimental pigs completed the protocol and are presented. Bleeding decreased CO to 95%, PORT to 80% and REN to 10% of baseline. From clamping on CO and PORT increased above baseline whereas REN (47%) with creatinine clearance remained compromised till the end. Microcirculation was affected more in ileum than sublingually. Approximately threefold increase in cytokines (tumour necrosis factor-α (TNF-alpha), interleukin (IL)-6 and IL-10) and oxidative stress markers (thiobarbituric acid-reactive substances (TBARs) and 4-hydroxy-2-trans-nonenal (HNE) was observed. Only mild increase in IL-6 and TBARs was observed in sham-operated animals. Organ histology did not reveal differences between groups.
Conclusions: This near-lethal model of rAAA induced expected severe deterioration of haemodynamics and metabolism accompanied with a moderate inflammatory and oxidative stress response.
