    Abstract This book contributes to the empirical literature on economic and human development from five different perspectives: the first chapter provides a new statistical test for bimodality of densities with an application to income data. The second chapter analyzes the worlds cross-country distribution of income and challenges the so called Twin Peaks-claim. The third chapter focuses on the world income distribution and resulting implications for poverty reduction, pro-poor growth and the evolution of global inequality. The fourth chapter estimates the welfare effects of recently negotiated Economic Partnership Agreements between the EU and African countries.     Abstract BACKGROUND: Diabetes is an important health burden in Indonesia. However, diabetes management and treatment remain poor, with most people with diabetes in Indonesia not achieving the recommended blood glucose levels. Peer education may have particular potential in low-income settings in complementing diabetes care without being a large additional strain on the health system. METHODS/DESIGN: This cluster randomized controlled trial aims to identify the effect of the implementation of peer education for patients with type 2 diabetes on diabetes-related outcomes in Aceh, Indonesia, which will complement the diabetes treatment provided at primary-care health posts (puskesmas). Altogether, 29 puskesmas were recruited in Banda Aceh and Aceh Besar, each of which was randomly assigned to either the control or the intervention group. Then, 534 people with diabetes were identified and recruited through their respective puskesmas. The intervention consists of up to two peer education groups per puskesmas, which are led by previously trained people with diabetes. Peer education sessions are held every month for 18 months, with follow-up data being collected 9 and 18 months after the first peer education session. The main objective is to improve diabetes management and the health behavior of participants receiving peer education to reduce their average blood glucose levels as measured by glycated hemoglobin (HbA1c) levels. Secondary outcomes are the effects of peer education on lipid levels, waist circumference, blood pressure, quality of life, treatment adherence, diabetes knowledge, physical activity, and dietary diversity. Data sources for the measurement of outcomes include patient and health facility surveys and biomarker measurements. An economic evaluation will be conducted to assess the cost-effectiveness of the intervention. DISCUSSION: This trial will contribute to the evidence on the effectiveness and cost-effectiveness of peer education in improving diabetes management in a low-income setting in Indonesia and in other comparable contexts.

    Abstract We propose a novel son preference measure that relates the preference to a specific child. We find child-specific son preference to be more common among later born children and in families with fewer sons. Using the novel measure and an interaction instrumental variables approach, we estimate a penalty in early mental functions for unwanted girls of 0.7 standard deviations. This penalty appears to be partially driven by discrimination against girls and partially by pampering of boys. Children's health and parental inputs do not mediate the effect from son preference to mental development. Our findings highlight the relevance of parents' attitudes for a nurturing home environment and healthy brain development.

    Abstract We study the effect of the Safe Childbirth Checklist (SCC) - a tool developed by the WHO to improve the quality of delivery care - on a range of provider- and patient-level outcomes. We conducted a clustered pair-wise matched randomized controlled trial among 166 health providers in two districts of Pakistan. This included primary and secondary health facilities as well as non-facility based rural health workers. We do not find positive effects on health outcomes, but on the adherence to some essential delivery practices, mostly to those conducted during the patient's admission to the delivery ward. We also find increased rates of referrals to higher-level facilities.

    Abstract This study collected evidence on the use of early sown wheat varieties and complementary zero tillage technologies in Northwest India. Detailed information on farmers' knowledge, adoption decisions, personal experience, and perceptions of early sown wheat and zero tillage technologies were collected at the household level using different survey tools. Additional information on agricultural practices during the Rabi Season 2021/22 were collected at the plot level and geocoded. Overall, the dataset comprises responses from 1206 wheat farmers in 70 villages across 7 districts in Punjab and Haryana that were collected between September and October 2022. The villages were selected using stratified random sampling based on a sampling frame of 1722 communities that had been identified as predominantly wheat growing areas based on remote-sensing data from satellite images. The dataset provides rich information that may be used for assessing the diffusion and impact of recently developed wheat varieties designed for early sowing, identifying barriers to the wider adoption of these technologies, and informing policy making aimed at improving adoption and usage decisions of agricultural innovations.

    Abstract Despite ongoing calls for a more even global distribution of COVID-19 vaccines, there remains a great disparity between high- and low-income countries. We conducted representative surveys among the adult populations in the United States (N = 1,000) and Germany (N = 1,003) in June 2021 to assess public opinion in these countries on the distributive justice of COVID-19 vaccines. We conducted two instances of analytic hierarchy processes (AHP) to elicit how the public weighs different principles and criteria for vaccine allocation. In further discrete choice experiments, respondents were asked to split a limited supply of vaccine doses between a hypothetical high-income and a hypothetical low-income country. AHP weights in the United States and Germany were 37.4% (37.2–37.5) and 49.4% (49.2–49.5) for “medical urgency”, 32.7% (32.6–32.8) and 25.4% (25.2–25.5) for “equal access for all”, 13.7% (13.6–13.8) and 13.3% (13.2–13.4) for “production contribution”, and 16.3% (16.2–16.4) and 12.0% (11.9–12.1) for “free market rules”, respectively, with 95% CI shown in parentheses. In the discrete choice experiment, respondents in the United States and Germany split available vaccine doses such that the low-income country, which was three times more populous than the high-income country, on average received 53.9% (95% CI: 52.6–55.1) and 57.5% (95% CI: 56.3–58.7) of available doses, respectively. When faced with a dilemma where a vulnerable family member was waiting for a vaccine, 20.7% (95% CI: 18.2–23.3) of respondents in the United States and 18.2% (95% CI: 15.8–20.6) in Germany reduced the amount they allocated to the low-income country sufficiently to secure a vaccine for their family member. Our results indicate that the public in the United States and Germany favours utilitarian and egalitarian distribution principles of vaccines for COVID-19 over libertarian or meritocratic principles. This implies that political decisions favouring higher levels of redistribution would be supported by public opinion in these two countries.

    Abstract Mesenchymal stromal cells (MSCs) and their extracellular vesicles (EVs) exert profound anti-inflammatory and regenerative effects in inflammation and tissue damage, which makes them an attractive tool for cellular therapies. In this study we have assessed the inducible immunoregulatory properties of MSCs and their EVs upon stimulation with different combinations of cytokines. First, we found that MSCs primed with IFN-γ, TNF-α and IL-1β, upregulate the expression of PD-1 ligands, as crucial mediators of their immunomodulatory activity. Further, primed MSCs and MSC-EVs, compared to unstimulated MSCs and MSC-EVs, had increased immunosuppressive effects on activated T cells and mediated an enhanced induction of regulatory T cells, in a PD-1 dependent manner. Importantly, EVs derived from primed MSCs reduced the clinical score and prolonged the survival of mice in a model of graft-versus-host disease. These effects could be reversed

    Abstract This study gathered evidence from Germany and the United States on public opinion towards fair distribution of COVID-19 vaccines across the world. Analytical Hierarchy Process and discrete choice experiments were used for this purpose. The sample is nationally representative of adults (aged 18 and above) for both countries using quotas on age, gender, education, state, and COVID-19 vaccination rates at the time of the fieldwork (25 May 2021 to 26 June 2021). Overall 1,003 responses in Germany and 1,000 in the United States were collected.
