    Abstract Engineering failure investigations seek to reconstruct the actual causes of major engineering failures. The investigators need to establish the existence of certain past events and the actual causal relationships that these events bear to the failures in question. In this paper, I examine one method for reconstructing the actual causes of failure events, which I call "feature dependence". The basic idea of feature dependence is that some features of an event are informative about the features of its causes; therefore, the investigators can use the features of a known failure event to reconstruct details of its causes. I make explicit the structure of feature dependence and the evidential basis of its key premises, and show how feature dependence works in the investigation of the American Airlines Flight 191 accident.

    Abstract Potentially toxic elements from geothermal springs can cause significant pollution of the surrounding environment and pose potential risk to the ecosystem. The fate of potentially toxic elements in the water-soil-plant system in the Yangbajain geothermal field on the Tibetan Plateau, China was investigated to assess their impact on the eco-environment. The concentrations of Be, F, As, and Tl were highly elevated in the headwaters of the Yangbajain geothermal springs, and their concentrations in the local surface water impacted by the geothermal springs reached 8.1 μg/L (Be), 23.9 mg/L (F), 3.83 mg/L (As), and 8.4 μg/L (Tl), respectively, far exceeding the corresponding thresholds for surface and drinking water.     Abstract The quality of farmland soils on the Tibetan Plateau is important because of the region's ecological vulnerability and their close link with local food security. Investigation on the pollution status of heavy metal (loid)s (HMs) in the farmlands of Lhasa and Nyingchi on the Tibetan Plateau, China revealed that Cu, As, Cd, Tl, and Pb were apparently enriched, with the soil parent materials being the primary sources of the soil HMs. Overall, the farmlands in Lhasa had higher contents of HMs compared to those in Nyingchi, which could be attributed to the fact that the former were mainly developed on river terraces while the latter were mainly developed on the alluvial fans in mountainous areas. As displayed the most apparent enrichment, with the average contents in the vegetable field soils and grain field soils of Lhasa being 2.5 and 2.2 times higher compared to those of Nyingchi. The soils of vegetable fields were more heavily polluted than those of grain fields, probably due to the more intensive input of agrochemicals, particularly the use of commercial organic fertilizers. The overall ecological risk of the HMs in the Tibetan farmlands was low, while Cd posed medium ecological risk. Results of health risk assessment show that ingestion of the vegetable field soils could pose elevated health risk, with children facing greater risk than adults. Among all the HMs targeted, Cd had relatively high bioavailability of up to 36.2% and 24.9% in the vegetable field soils of Lhasa and Nyingchi, respectively. Cd also showed the most significant ecological and human health risk. Thus, attention should be paid to minimize further anthropogenic input of Cd to the farmland soils on the Tibetan Plateau.

    Abstract Rationale: Previous studies have found that the main treatment of sinus arrest is pacemaker treatment. It is rare to have 12 s of sinus arrest after radiofrequency ablation, and whether a permanent pacemaker is implanted immediately in this case is not described in the guidelines.
    Patient concerns: A 76-year-old male patient with persistent atrial fibrillation (AF) developed sinus arrest lasting 12 s in the early morning of the fourth day after using radiofrequency ablation for pulmonary vein isolation.
    Diagnosis: The patient was diagnosed with AF and sinus arrest.
    Interventions: The patient received cardiopulmonary resuscitation, intravenous injection of atropine 1 mg, and intravenous infusion of isoproterenol 1mg and immediately recovered consciousness thereafter. Approximately, 1.5 h later, the patient underwent surgery to install a temporary pacemaker in the right femoral vein.
    Outcomes: The patient had repeated episodes of sinus arrest after the implantation of a temporary pacemaker. After 3 weeks, the patient stabilized and was discharged. The patient was followed up for 1 year and did not experience any recurrence of sinus arrest or AF.
    Lessons: We consider the potential for postoperative myocardial edema, injury to the sinoatrial node during the procedure, propafenone poisoning, and autonomic dysfunction as contributors to the occurrence of sinus arrest after radiofrequency ablation. When sinus arrest occurs after radiofrequency ablation, we can choose the appropriate treatment according to the patient's condition.

    Abstract Background: Branchio-Oto-Renal (BOR) Syndrome is a rare autosomal disorder with a wide variety of clinical manifestations and a high degree of heterogeneity. Typical clinical manifestations of BOR syndrome include deafness, preauricular fistula, abnormal gill slits, and renal malformations. However, atypical phenotypes such as congenital hip dysplasia, congenital heart anomaly or facial nerve paresis are rare in BOR syndrome, and this might be easily misdiagnosed with other congenital disorders.
    Case presentation: We report a 5-month-old boy of BOR syndrome with "congenital heart defects and proteinuria" as clinical features. Initially, as this case mainly presented with symptoms of recurrent respiratory infections and was found to be with congenital heart disease and proteinuria at the local hospital, but he only was diagnosed with congenital heart disease combined with pulmonary infection and anti-infective and supportive treatment was given. Subsequently, during the physical examination at our hospital, left side preauricular pit and branchial fistulae on the right neck were found. Subsequent evaluation of auditory brainstem response and distortion product otoacoustic emission were revealed sensorineural hearing impairment. Results of renal ultrasonography showed small kidneys. Genetic analysis revealed a microdeletion at chromosome 8q13.2-q13.3 encompassing EYA1 gene, this patient was finally diagnosed with BOR syndrome. Then, this patient received transcatheter patent ductus arteriosus closure and hearing aid treatment. Proteinuria, renal function and hearing ability are monitoring by nephrologist and otologist. The patient is currently being followed up until 3 months after discharge and his condition is stable.
    Conclusion: Careful physical examination, detailed history and the implementation of diagnostic laboratory tests can reduce the incidence of misdiagnosis. Genetic sequencing analysis of patients is a key guide to the differential diagnosis of BOR syndrome.

    Abstract Despite enormous advances in the treatment of cardiovascular diseases, including I/R injury and heart failure, heart diseases remain a leading cause of mortality worldwide. Inositol-requiring enzyme 1 (IRE1) is an evolutionarily conserved sensor endoplasmic reticulum (ER) transmembrane protein that senses ER stress. It manages ER stress induced by the accumulation of unfolded/misfolded proteins via the unfolded protein response (UPR). However, if the stress still persists, the UPR pathways are activated and induce cell death. Emerging evidence shows that, beyond the UPR, IRE1 participates in the progression of cardiovascular diseases by regulating inflammation levels, immunity, and lipid metabolism. Here, we summarize the recent findings and discuss the potential therapeutic effects of IRE1 in the treatment of cardiovascular diseases.

    Abstract Background: Due to the heterogeneity of the phenotype of Hereditary spherocytosis (HS) patients, some patients may have rare clinical complications such as biliary obstruction and ultra-high bilirubinemia.
    Case presentation: A 8-y-old boy presented to the emergency with complaints of anemia for 6 years and worsened abdominal pain and scleral yellowing of the skin for 2 days. Physical examination showed tenderness in the middle and upper abdomen and splenomegaly. Abdominal CT revealed biliary obstruction. Genetic analysis revealed a de novo mutation in the gene ANK1, HS with biliary obstruction was diagnosed. The surgery of bile duct exploration and T-tube drainage, and splenectomy were performed successively. This patient was followed up for 13 months after splenectomy, and his condition was stable.
    Conclusion: The diagnosis of HS is not clinically difficult, and once a patient with HS is diagnosed, regular follow-up management and standardized treatment are required. Genetic testing is also needed to screen for other genetic disorders that may co-exist in patients with HS who do not have a good efficacy or who have a long-term chronic onset of jaundice.
