    Abstract In 2016, as a trainee doctor, I wrote a paper on changing the culture of medicine. I felt the medical system was broken back then, and it seems even less functional now, with higher rates of burnout, dropout and staff shortages nationally. As a result of a lack of resources, it feels impossible to provide the care to our communities expected of us, making our work challenging and disheartening. Until all stakeholders acknowledge the systemic issues faced by our workforce, service outcomes and physician well-being may not improve. We need to collaborate and innovate to reform the healthcare system taking a multifaceted, evidence-based approach, implementing an appropriate balance of systemic change and interventions to support individual well-being. As we collectively work towards these changes, tragic optimism may spur physicians to develop meaning and purpose despite the inevitable challenges. This may serve as the motivation and fuel required to survive and sustain our practice but also thrive working in careers of value.

    Abstract As sites of turbulence and transformation, cities are machines for forgetting. And yet archiving and exhibiting the presence of the past remains a key cultural, political and economic activity in many urban environments. This book takes the example of Berlin over the past four decades to chart how the memory culture of the city has responded to the challenges and transformations thrown up by the changing political, social and economic organization of the built environment. The book focuses on the visual culture of the city (architecture, memorials, photography and film). It argues that the recovery of the experience of time is central to the practices of an emergent memory culture in a contemporary overexposed city, whose spatial and temporal boundaries have long since disintegrated

    Abstract Owing to climate change related uncertainties and anticipated population growth, different parts of the world (particularly urban areas) are experiencing water shortages or flooding and security of fit-for-purpose supplies is becoming a major issue. The emphasis on decentralized alternative water supply systems has increased considerably. Most of the information on such systems is either scattered or focuses on large scale reuse with little consideration given to decentralized small to medium scale systems. Alternative Water Supply Systems brings together recent research into the available and innovative options and additionally shares experiences from a wide range of contexts from both developed and developing countries. Also covered are the technical, social, financial and institutional aspects associated with decentralized alternative water supply systems. These include systems for greywater recycling, rainwater harvesting, recovery of water through condensation and sewer mining

    Abstract The objective of this study was to review the evidence relating to treatment duration and the surgery first approach (SFA) in managing class III skeletal discrepancies to determine whether there is sufficient evidence to show that it results in a shorter duration of treatment compared to a conventional orthodontics first approach (OFA). Analytical and observational studies that compared both approaches and recorded treatment duration as an outcome were included. MEDLINE, EMBASE, CENTRAL and Dental Oral Sciences Source databases were searched along with a review of reference lists. Risk of bias was assessed using the Risk of Bias in Non-randomised Studies of Interventions (ROBINS-I) tool. Eleven observational studies met the inclusion criteria. No experimental studies were identified. The SFA was reported to have a shorter duration of treatment in all studies. All included studies showed a significant risk of bias, particularly in relation to selection of participants, allocation to groups and in the measurement of treatment duration. There is no quality evidence to suggest that using a surgery first approach to orthognathic treatment of class III skeletal patterns results in a shorter duration of treatment.

    Abstract This article explores working-class women's experiences of political activism in the Labour Party in the 1930s. The article focuses upon the relationships formed with leaders, the bonds with fellow women, and the emotional fulfilment politics could bring, rather than considering the policies and campaigns which drew women into the party. It suggests how working-class women performed a political self which was shaped by but distinctive from a domestic self. Official political party materials from across Britain are drawn upon to uncover how working-class women in the years after equal franchise was won carved out a political space and the meanings of activism. Party members' private and public portrayals of women's activism are considered. Although the focus is upon working-class women's political experiences, the conclusions drawn here have implications for how the creation of a political self can be considered in studies of other political parties and voluntary organizations.

    Abstract Introduction: Before the COVID-19 pandemic, a 1-week in-person Clinical Ultrasound Course was taught in African nations as part of a U.S. Department of State-funded program that supports and trains African peacekeepers serving with the United Nations and African Union. In order to maintain active engagement with host nations despite the travel restrictions due to the COVID-19 pandemic, portions of the course were taught virtually in 2021 to providers in Ghana, Senegal, and Rwanda. An abbreviated course was delivered covering the Focused Assessment with Sonography in Trauma (FAST) trauma exam and vascular access. The goal of this study is to assess the effectiveness of the Clinical Ultrasound Course while taught in a virtual classroom.
    Materials and methods: Thirty-six participants enrolled in the program. Participants completed a pre-course survey before the training. Training consisted of a pre-recorded lecture followed by hands-on ultrasound instruction. After the training, participants completed a post-course survey. Eight participants were excluded from the study because they did not complete both surveys. Survey questions assessed the participants' comfort with ultrasound mechanics and the FAST exam. Participant responses were measured utilizing a visual analog scale.
    Results: Based on survey results, all participants gained a better understanding of the indications and limitations of the FAST exam from the virtual Clinical Ultrasound Course. All participants also felt more comfortable using ultrasound in clinical practice than they did before the course. Ghanaian and Senegalese participants showed greater improvement in all areas studied than Rwandan participants. This is likely due to the fact that the Rwandan hospital system had prior ultrasound training and a higher baseline understanding than their counterparts. The Rwandan hospital system had previously completed the in-person sessions of the Clinical Ultrasound Course and therefore had some institutional knowledge, while the Ghanaians and Senegalese took the course for the first time during this study.
    Conclusion: Virtual delivery of the Clinical Ultrasound Course was successful. Participants felt more comfortable in all aspects of ultrasound taught during the course and indicated that they were more likely to use ultrasound in clinical practice. This demonstrates that virtual ultrasound teaching is a viable option for international educational programs in the future.
