    Abstract Clinical Encounters in Sexuality makes an intervention into the fields of clinical psychoanalysis and sexuality studies, in an effort to think about a range of issues relating to sexuality from a clinical psychoanalytic perspective. This book concentrates on a number of concepts, namely identity, desire, pleasure, perversion, ethics and discourse. The editors, Noreen Giffney and Eve Watson, have chosen queer theory, a sub-field of sexuality studies, as an interlocutor for the clinical contributors, because it is at the forefront of theoretical considerations of sexuality, as well as being both reliant upon and suspicious of psychoanalysis as a clinical practice and discourse. The book brings together a number of psychoanalytic schools of thought and clinical approaches, which are sometimes at odds with one another and thus tend not to engage in dialogue about divisive theoretical concepts and matters of clinical technique. Traditions represented here include: Freudian, Kleinian, Independent, Lacanian, Jungian, and Relational. The volume also stages, for the first time, a sustained clinical psychoanalytic engagement with queer theory. By virtue of its editorial design, this book aims to foster a self-reflective attitude in clinical readers about sexuality which historically has tended toward reification

    Abstract Approximately 5,700 people are diagnosed with myeloma each year in the UK. The standard of care is to receive an autologous stem cell transplant after completion of induction therapy. There are no specific dietary recommendations for people with myeloma, however they are at risk of malnutrition due to symptoms and side effects of treatments. This report describes the journey of a 73-year-old male diagnosed with immunoglobulin A (IgA) lambda myeloma in April 2021. The patient lost 23% of his body weight during 6 months of systemic anti-cancer treatment (SACT), resulting in postponing his transplant twice due to reduced fitness. This report describes an effective, although late, multidisciplinary intervention which was successful for the patient who managed to reestablish a healthy weight and good quality of life. The patient received his transplant in January 2023. This case highlights two important aspects of patient care that should not be underestimated in dietetic clinical practice: early screening and multidisciplinary collaboration. Monitoring the nutritional status of patients and providing early nutrition support can prevent hospital admissions, treatment delays and reduce the associated costs. Multidisciplinary teamwork can improve patient care and clinical outcomes, and it is fundamental to strengthen communication and collaboration among clinical disciplines.

    Abstract Craniofacial reconstruction requires robust bone of specified geometry for the repair to be both functional and aesthetic. While native bone from elsewhere in the body can be harvested, shaped, and implanted within a defect, using either an in vitro or in vivo bioreactors eliminates donor site morbidity while increasing the customizability of the generated tissue. In vitro bioreactors utilize cells harvested from the patient, a scaffold, and a device to increase mass transfer of nutrients, oxygen, and waste, allowing for generation of larger viable tissues. In vivo bioreactors utilize the patient's own body as a source of cells and of nutrient transfer and involve the implantation of a scaffold with or without growth factors adjacent to vasculature, followed by the eventual transfer of vascularized, mineralized tissue to the defect site. Several different models of in vitro bioreactors exist, and several different implantation sites have been successfully utilized for in vivo tissue generation and defect repair in humans. In this review, we discuss the specifics of each bioreactor strategy, as well as the advantages and disadvantages of each and the future directions for the engineering of bony tissues for craniofacial defect repair.

    Abstract Binocular summation, a well-known phenomenon in letter acuity measurement, refers to the improvement in visual performance when viewing with both eyes compared to one eye alone. The present study aims to assess the relationship in binocular summation between high and low contrast letter acuities, and examine whether baseline measure (binocular summation at either high or low contrast) is predictive of the change in binocular summation between contrast conditions. Corrected high and low contrast letter acuities were assessed monocularly and binocularly in 358 normal vision observers aged 18-37 years using Bailey-Lovie charts. All observers had high contrast acuities (both monocular and binocular) of 0.1 LogMAR or better and no known eye disease. Binocular summation was calculated as the difference in LogMAR between the better eye acuity and binocular acuity. We found that binocular summation was present at both contrast levels (0.044 ± 0.002 LogMAR for high and 0.069 ± 0.002 LogMAR for low contrast) with higher magnitude of summation at low contrast, and declined with increasing interocular difference. There was a correlation in binocular summation between high and low contrast. The difference in binocular summation between the two contrast levels was found to be correlated with the baseline measurement. Using common commercially available letter acuity charts, we replicated the findings on binocular acuity summation in normally sighted young adults for both high and low contrast letters. Our study revealed a positive relationship in binocular acuity summation between high and low contrast, and an association between a baseline measure and the change in binocular summation between contrast levels. These findings may serve as a reference in clinical practice and research when high and low contrast binocular summations are measured in assessing binocular functional vision.
