    Abstract More than 190,000 patients have been fitted with an indwelling nasogastric tube in Taiwan. Nasogastric tube feeding is a strategy for temporary or long-term feeding and for preventing aspiration pneumonia in patients with dysphagia. However, aspiration pneumonia is a common complication in patients fitted with a nasogastric tube. Displacement of the nasogastric tube, inappropriate length of the placed nasogastric tube resulting gastroesophageal reflux, inappropriate feeding posture and speed, and excessive gastric residual volume resulting in vomiting are important risk factors associated with aspiration pneumonia. In this article, the literature and clinical practice experiences were reviewed to identify methods used to confirm the length and positioning of nasogastric tubes. We further explored strategies for preventing oral-care and tube-feeding-induced aspiration pneumonia during the period of indwelling nasogastric tube. These strategies included adjusting the patient's positions for receiving tube feedings, feeding techniques, methods and frequency of gastric residual volume measurement, and conditions for stopping nasogastric feeding. This exploration was intended to provide nursing staff with relevant knowledge and skills for preventing aspiration pneumonia in patients with indwelling nasogastric tube to improve patient safety.

    Abstract In the case of small-cell lung carcinoma, the highly metastatic nature of the disease and the propensity for several chromatin modifiers to harbor mutations suggest that epigenetic manipulation may also be a promising route for oncotherapy, but histone deacetylase inhibitors on their own do not appear to be particularly effective, suggesting that there may be other regulatory parameters that dictate the effectiveness of vorinostat's reversal of histone deacetylation. Recent discoveries that serotonylation of histone H3 alters the permissibility of gene expression have led to renewed attention to this rare modification, as facilitated by transglutaminase 2, and at the same time introduce new questions about whether this modification belongs to a part of the concerted cohort of regulator events for modulating the epigenetic landscape. This review explores the mechanistic details behind protein serotonylation and its possible connections to the epigenome via histone modifications and glycan interactions and attempts to elucidate the role of transglutaminase 2, such that optimizations to existing histone deacetylase inhibitor designs or combination therapies may be devised for lung and other types of cancer.

    Abstract Background and aims: The present study aimed to investigate the predictive ability of selected adiposity indices, such as body mass index (BMI), waist-to-hip ratio (WHR), waist circumference (WC), and waist-to-height ratio (WHtR), for new-onset hypertension in metabolically healthy Taiwanese adults. The study also sought to establish sex-specific cutoff points for these indices and to analyze the risk of new-onset hypertension, taking into account sex and age.
    Methods: This prospective cohort study utilized the Taiwan Biobank database to examine metabolically healthy participants aged between 20 and 65 at baseline. Four adiposity indices, namely BMI, WHR, WC, and WHtR, were calculated and used to predict new-onset hypertension over 4 years. Receiver operating characteristics (ROCs) and areas under the curve (AUCs) were used to evaluate the effectiveness of the parameters in predicting new-onset hypertension over 4 years. Sex-specific cutoff points were identified and used to assess the risk of new-onset hypertension.
    Results: This study analyzed 13,375 participants over 4.28 years. The incidence of new-onset hypertension was 17.65%. The new-onset rate of hypertension was 34.39% in men and 65.61% in women. Adiposity indices effectively predict new-onset hypertension, with WHtR having the highest predictive value (i.e., AUC) for both sexes. The classification of participants into low and high categories for each adiposity index was based on sex-specific cutoff points, and the risk of new-onset hypertension was assessed according to sex and age. This study found that high adiposity indices predicted a significantly higher risk of new-onset hypertension in metabolically healthy adults. The risk was equal for both sexes. Young women had a higher risk of new-onset hypertension than middle-aged women when they were further categorized. All risk ratios of the indices in young women were over two-fold and significant.
    Conclusion: According to the sex-specific cutoff point, high adiposity indices had a higher predictive value for new-onset hypertension in metabolically healthy Taiwanese young women.

    Abstract Highlights: • Introduce a data augmentation strategy to expand the required different morphological data during the training and learning phase, and improve the algorithm's feature learning ability for complex and diverse tumor morphology CT images.• Design attention mechanisms for encoding and decoding paths to extract fine pixel level features, improve feature extraction capabilities, and achieve efficient spatial channel feature fusion.• The deep supervision layer is used to correct and decode the final image data to provide high accuracy of results.• The effectiveness of this method has been affirmed through validation on the LITS, 3DIRCADb, and SLIVER datasets.
    Background: Accurately extracting liver and liver tumors from medical images is an important step in lesion localization and diagnosis, surgical planning, and postoperative monitoring. However, the limited number of radiation therapists and a great number of images make this work time-consuming.
    Objective: This study designs a spatial attention deep supervised network (SADSNet) for simultaneous automatic segmentation of liver and tumors.
    Method: Firstly, self-designed spatial attention modules are introduced at each layer of the encoder and decoder to extract image features at different scales and resolutions, helping the model better capture liver tumors and fine structures. The designed spatial attention module is implemented through two gate signals related to liver and tumors, as well as changing the size of convolutional kernels; Secondly, deep supervision is added behind the three layers of the decoder to assist the backbone network in feature learning and improve gradient propagation, enhancing robustness.
    Results: The method was testing on LITS, 3DIRCADb, and SLIVER datasets. For the liver, it obtained dice similarity coefficients of 97.03%, 96.11%, and 97.40%, surface dice of 81.98%, 82.53%, and 86.29%, 95% hausdorff distances of 8.96 mm, 8.26 mm, and 3.79 mm, and average surface distances of 1.54 mm, 1.19 mm, and 0.81 mm. Additionally, it also achieved precise tumor segmentation, which with dice scores of 87.81% and 87.50%, surface dice of 89.63% and 84.26%, 95% hausdorff distance of 12.96 mm and 16.55 mm, and average surface distances of 1.11 mm and 3.04 mm on LITS and 3DIRCADb, respectively.
    Conclusion: The experimental results show that the proposed method is effective and superior to some other methods. Therefore, this method can provide technical support for liver and liver tumor segmentation in clinical practice.

    Abstract The 18 mm thick Q235A low carbon steel plates were welded via the ultra-narrow gap metal active gas arc welding (ultra-NGMAGW) and narrow gap submerged arc welding (NGSAW), and the microstructure and mechanical properties of the welded joints' area were characterized. The results showed that there is acicular ferrite (AF) in the weld zone of the joint obtained via the ultra-NGMAGW. The AF grains are fine and have a great difference in growth direction, resulting in high local dislocation density. However, there is no AF in the welded joint obtained via the NGSAW. Using numerical simulation analysis of the temperature field distribution and the thermal cycle curve in the welding process of the ultra-NGMAGW, it was found that the mechanism of microstructure evolution is that during the welding process of the ultra-NGMAGW, the heat input is low, the cooling rate is quick, and the residence time in the high temperature region is short. Therefore, pearlite with coarse grains is basically not formed. AF nucleates in different directions with inclusions as the core. The tensile strength of the weld joint obtained via the ultra-NGMAGW is 643 MPa, which corresponds to 139% of that of the base metal, and 132% of that obtained via the NGSAW. The ultra-NGMAGW joints exhibited better tensile strength and higher microhardness than the NGSAW joints, which is mainly due to the existence of AF.

    Abstract Objective: To improve the diagnostic yield of prostate biopsy, which we can achieve by puncture more sites and number of cores, another way to obtain more tissue is to take longer tissue strips. In this study, we evaluated the effect of strip length on cancer diagnosis by needle biopsy and derived a cutoff value of strip length to improve cancer detection.
    Methods: The pathological reports of 754 patients with suspected prostate cancer who underwent transperineal prostate biopsy were retrospectively analyzed. The age, serum prostate specific antigen (PSA), prostate volume, Gleason score and tissue strip length were analyzed. The length of the tissue strip was compared between the biopsy positive patients and the biopsy negative patients, and the patients were divided into group A(biopsy positive group)and group B(biopsy negative group), respectively. Statistical analysis of tissue strip lengths was performed to determine cutoff values for biopsy length quality.
    Results: A total of 10 556 tissue strips were obtained from 754 patients, and 45.1 % of the patients were pathologically diagnosed as prostate cancer. The median length of the tissue strip was 10.5 (9.5, 12.5) mm, the median age was 69 (64,75) years, the median PSA was 12.4 (8.6, 20.8) μg/L, and the median prostate volume was 44.8 (30.5, 64.4) ml. The median length of tissue strips in group A and group B was 11 (10,13) mm and 10 (9,12) mm, respectively. Receiver operating characteristic (ROC) curve analysis was performed on the length of tissue strips in all cases, and the cutoff value of quality assurance was 11.8mm, the area under curve (AUC) was 0.82, and the cut-off value of quality assurance was 11.8mm. Sensitivity 71.4%, specificity 73.8%(P<0.001).
    Conclusions: In transperineal prostate biopsy, the cancer detection rate of tissue strips may increase with length. The results of ROC analysis showed that 11.8 mm was used as the cut-off value for quality assurance.

    Abstract Abstract: A 57-year-old man presented with odynophagia for 1 week was referred for FDG PET/CT scan to rule out recurrent hypopharyngeal cancer. The FDG PET/CT showed hypermetabolic lesions in hypopharyngeal area and adjacent cervical spine with pneumorrhachis, the presence of intraspinal air, on attenuation CT images, which might indicate a life-threatening infection. An emergency MRI confirmed the presence of cervical spondylodiscitis with an epidural abscess. The patient rapidly progressed to quadriplegia and difficulty voiding on the same day as the PET/CT scan, leading to emergent operation. The patient received antibiotics treatment and discharged 4 months later without evidence of cancer recurrence.
